Miniscrew-assisted Orthodontic Correction of Gummy Smile in a Patient with Vertical Maxillary Excess and Lip Hypermobility.

This case report describes the successful nonsurgical correction of a gummy smile in a 23-year-old female patient with vertical maxillary excess and hypermobile lips using miniscrew-assisted orthodontic mechanotherapy. Excessive gingival display was reduced by applying controlled forces to the upper front teeth and miniscrew-assisted intrusion. The treatment also improved the patient's facial profile by promoting counterclockwise jaw rotation, normalizing her skeletal imbalance. In addition, the hyperactive lip movements during smiling were minimized, likely due to neuromuscular adaptation and

Orthodontic correction of anterior open bite using skeletal anchorage: systematic review and meta-analysis.

The purpose of this study was to investigate the scientific evidence on the short- and long-term effects of orthodontic correction of anterior open bite (AOB) using skeletal anchorage (SA). Clinical studies on the use of SA for AOB in patients with permanent dentition, or at least 12 years of age, were searched. Short- and long-term (≥2 years) outcomes were collected. Mean differences were calculated from pooled data. Twenty-four eligible articles with a total of 362 subjects were selected for inclusion in the meta-analysis. There was a significant increase in overbite (3.88 mm, P < 0.001) and

Occlusal plane rotation and orthodontic decompensation: influence on the outcome of surgical correction of class III malocclusion.

PURPOSE: The aim of this retrospective multicenter study is to evaluate the influence of surgical manipulation of the upper occlusal plane (UOP) and orthodontic decompensation on the outcome of class III orthognathic surgery. METHODS: Incisor inclinations, occlusal plane inclination as well as skeletal and soft tissue changes were assessed in lateral cephalograms of 85 class III patients who had previously undergone orthognathic surgery. Fourteen linear and eight angular measurements were performed on each radiograph at the beginning of treatment (T0), before surgery (T1) and at the end of tr

A 29-year-old female patient with unilateral condylar hyperplasia (UCH) of the left side presented with facial asymmetry, maxillary transverse occlusal plane (MXTOP) cant, posterior open bite, and Class III relationship. Treatment consisted of proportional condylectomy of the left condyle for management of UCH, and fixed orthodontic treatment with intrusion of the left maxillary molars to correct the MXTOP cant and remaining chin point deviation (CPD). Proportional condylectomy with a 14-mm resection of the left condylar head improved the CPD from 11.5 mm to 7.8 mm and resolved the posterior o

Orthodontic decompensation and correction of skeletal Class III malocclusion with gradual dentoalveolar remodeling in a growing patient.

An 8-year-old girl with a skeletal Class III malocclusion was treated in 2 phases. Maxillary expansion and protraction were carried out as the early intervention. However, her maxillary hypoplasia and mandibular hyperplasia deteriorated with age. The phase 2 comprehensive treatment began with proper mechanics when she was 12 years old with growth potential. In the maxillary arch, an auxiliary rectangular wire was used with a round main wire and an opening spring to create space for the impacted teeth and to bodily move the anterior teeth forward. Decompensation of mandibular incisors and corre

Patterns of incisor root resorption before and after orthodontic correction in cases with anterior open bites.

External root resorption is a frequent iatrogenic consequence of orthodontic treatment, particularly in the maxillary anterior teeth. Since resorption also occurs is a normal function of aging and since altered behaviors such as bruxism and chronic nailbiting accelerate resorption even in the absence of treatment, it was hypothesized that the long-term orthopedic forces of tongue thrusting leading to anterior open bites would also enhance the rates of clastic activity. In a series of 32 adolescents with open bites, the roots of permanent maxillary central incisors were significantly shorter an

Orthodontic correction of a skeletal Class II malocclusion with severe gummy smile by total intrusion of the maxillary dentition.

A 22-year-old woman visited the hospital complaining of lip protrusion, crowded teeth, and a gummy smile. The clinical examination showed a convex profile with a hyperdivergent mandible and a severe gummy smile in both anterior and posterior regions. The unstable mandible position was considered during treatment planning, and a targeted mechanic system was carefully designed. Temporary skeletal anchorage devices in the posterior dental region and a transpalatal arch were introduced as anchorage for the intrusion of the entire maxillary dentition and controlled retraction of the anterior teeth.

Correction of a gummy smile and lip protrusion by orthodontic retreatment with lingual appliances and temporary skeletal anchorage devices.

A 28-year-old woman desired orthodontic retreatment for lip protrusion and excessive gingival display in both the anterior and posterior areas on full smiling. She had previously undergone an extraction orthodontic treatment for correction of open bite. She was diagnosed with skeletal Class Ⅱ hyperdivergence. To mimic LeFort Ⅰ maxillary impaction surgery, posterosuperior movement of the maxillary whole dentition was planned, and bodily distalization of both the maxillary and the mandibular whole dentitions to improve lip protrusion. A combination of lingual appliances, 2 appropriately fabricat

Utilizing virtual surgical planning and orthognathic surgery to correct severe facial asymmetry without orthodontic treatment.

Combining double-jaw surgery and orthodontic treatment to correct dentofacial asymmetry is a standard procedure but time consuming and costly. In order to provide alternative solutions, previewing the possible results with precise virtual surgical planning (VSP) system can help clinicians select patients suited for correcting facial asymmetry surgically without orthodontic treatment. In our experiences, after rigid or semirigid fixation of the maxilla, using intermaxillary fixation (IMF) without mandibular bony fixation is another key. This kind of fixation can minimize the chances of changing

Correction of the occlusal and functional sequelae of mandibular condyle fractures using orthodontic mini-implant molar intrusion.

We report on the non-surgical management of an adult female whose bilateral mandibular condylar fractures had resulted in a clockwise (posterior) mandibular rotation, limitation of mandibular movements and increased occlusal loading on the molar teeth. She refused maxillary surgery and was treated with a minimally-invasive approach, involving orthodontic fixed appliances and mini-implant intrusion of the maxillary molar teeth. This provided both occlusal and functional improvements, including a significant increase in the inter-incisal distance, which were stable after one year of retention.

Orthognathic surgery in patients with craniofacial syndrome. I. A 5-year overview of combined orthodontic and surgical correction.

We studied orthognathic correction of dentofacial deformities in patients with craniofacial syndromes (Crouzon, Apert, and Treacher Collins) during a 5-year period. The number of patients treated within this time period was 17 with Crouzon syndrome, 12 with Apert syndrome, and six with Treacher Collins syndrome. Individual follow-up was 2.5 years. Bimaxillary operation, sometimes with genioplasty, has been done after preoperative orthodontic treatment of 1.5-2.5 years. Clinical and radiographic cephalometric evaluations of the orthodontic results were made. The functional and aesthetic results

[Corrective movement of a palatal impacted canine tooth: methodology and outcome of a combined oral surgery/orthodontic procedure].

In 24 patients, 31 palatal impacted maxillary canines were brought in position using the same surgical and orthodontic concept. After temporal surgical exposure a little silver chain was attached at the crown of the canine, the mucoperiostal flap was repositioned, and the canine was moved into its position by innovative orthodontic treatment means. The combined method is described intensively. With respect to the applied technique the results indicate that according to the criteria of periodontal status, vitality, bone support, root resorption and occlusion no differences exist to free erupted

A Retrospective Cohort Study Comparing the Surgery-First and Orthodontic-First Protocols for Correcting Skeletal Class II Malocclusion.

BACKGROUND: The surgery-first approach (SFA) with mandibular anterior subapical osteotomy (MASO) is increasingly utilized to overcome the limitations of conventional preoperative orthodontic decompensation and achieve earlier correction of facial deformities in skeletal class II patients. However, it also caused greater surgical trauma, and there are currently insufficient clinical data available for analyzing the surgical outcome. METHODS: We conducted a retrospective study to compare the skeletal stability, overall treatment duration, and incidence of major complications between the SFA and
