Immune rejection of orthopedic tissue allograft scoping review: Are we missing a cause of graft/procedural failure? Current concepts.

Allograft transplants are commonly used in orthopedics. Unlike other transplanted tissues, orthopedic tissue allografts are not human leukocyte antigen (HLA)-matched. The clinical impact of the immune response to allografts is poorly understood. Recent experience with allograft failures and reactions to anterior cruciate ligaments (ACL), medial patellofemoral ligament (MPFL), osteochondral, and meniscus transplants raises concern. This scoping review examines the literature on immunologic rejection of orthopedic tissue allograft using PubMed, Embase, Web of Science, and Scopus for studies desc

Design optimization procedure for an orthopedic insole having a continuously variable stiffness/shape to reduce the plantar pressure in the foot of a diabetic patient.

Foot ulcers and lower-limb amputations are among the major problems in diabetic patients. Orthopedic insoles can reduce the risk of diabetic foot ulcers in patients through pressure redistribution on the bottom of the foot. The purpose of this study was to propose an optimization method to design the dedicated insoles for diabetic patients in order to decrease the maximum plantar pressure. At first, a three-dimensional finite element model of bones, ligaments and soft tissue of a diabetic patient's foot was created using CT scan images. Then, the foot plantar pressure was calculated by means o

Surgeon Experience Matters: An Exact Matched Analysis of TLIF Outcomes Demonstrates No Difference in Surgical Outcomes Between Experienced Neurosurgeons and Orthopedic Surgeons.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To compare surgical and medical complications profile between neurosurgeons and orthopedic surgeons after transforaminal lumbar interbody fusion (TLIF) procedures. BACKGROUND: Studies comparing the impact of spine surgeon specialty (neurosurgery vs. orthopedic spine) on TLIF outcomes have been inconclusive and failed to control for operative learning curves and surgical maturity. Orthopedic spine surgeons perform fewer spine procedures in residency, although these differences may be attenuated by mandatory fellowship before starting practi

Orthopedic disorders of the knee in hemophilia: A current concept review.

The knee is frequently affected by severe orthopedic changes known as hemophilic arthropathy (HA) in patients with deficiency of coagulation factor VIII or IX and thus this manuscript seeks to present a current perspective of the role of the orthopedic surgeon in the management of these problems. Lifelong factor replacement therapy (FRT) is optimal to prevent HA, however adherence to this regerous treatment is challenging leading to breakthrough bleeding. In patients with chronic hemophilic synovitis, the prelude to HA, radiosynovectomy (RS) is the optimal to ameliorate bleeding. Surgery in pe
