OBJECTIVE: To compare vitamin D status represented by serum 25-hydroxyvitamin D (25-OHD) levels in a group of patients with primary hyperparathyroidism, stratified by clinical manifestations.
METHODS: We studied 22 patients (18 women and 4 men) with primary hyperparathyroidism--5 patients with severe osteitis fibrosa cystica, 10 symptomatic patients with active renal stone disease without overt bone disease, and 7 asymptomatic patients. Bone mineral density (BMD) measurements (T-scores) were done at the lumbar spine, femoral neck, and distal radius, and laboratory data for the three subgroups…

Primary hyperparathyroidism is rarely found in the pediatric population. Delays in diagnosis due to vague symptoms can lead to severe disease manifestations such as osteitis fibrosa cystica. We report a rare case of primary hyperparathyroidism in a 13-year-old girl who presented with a pathological femur fracture and numerous brown tumors.

The case is described of a woman who died with a functioning parathyroid carcinoma 19 years after removal of two parathyroid tumours, considered at the time to be benign. Following operation hyperparathyroidism subsided, with a short period of hypocalcaemia, and severe osteitis fibrosa cystica healed. Five years before death progressive renal failure developed with normal and later raised serum calcium levels. At necropsy a parathyroid carcinoma infiltrated the thyroid at the site of the previous operation and there was a solitary hepatic metastasis. A remaining parathyroid gland was nodularly…

A 52 year old male, without medical history, had painful shins for several months. X-ray showed lytic bone reaction and blood results showed a primary hyperparathyroidism. The diagnosis was osteitis fibrosa cystica, a rare, benign bone lesion. Increased parathyroid hormone leads to high osteoclastic bone turnover causes osseous deformities and fractures.

For thirty years, primary hyperparathyroidism is recognized as a frequent endocrine disease which is, most often, "non symptomatic" that is without nephrolithiasis or osteitis fibrosa cystica. Our knowledge in the pathophysiology has greatly grown and diagnosis of primary hyperparathyroidism is frequently easy. The only curative treatment is surgery and the indications have been codified by several consensus conferences.

Facial skeletal changes associated with hyperparathyroidism assume 3 radiographic patterns: osteitis fibrosa cystica, fibrous dysplasia, and leontiasis ossea. The 3rd pattern is unique to renal osteodystrophy. We report a case of uremic leontiasis ossea with CT images illustrating significant hypertrophy of the jaws with serpiginous tunneling within the bone and poor visualization of the cortical bone.

BACKGROUND: A growing number of patients with primary hyperparathyroidism (PHPT) are diagnosed in the absence of symptoms following routine biochemical screening. However, in some countries, overt manifestations and osteitis fibrosa cystica (OFC) still dominate the clinical profile of PHPT patients.
METHODS: We retrospectively studied clinical and biochemical manifestations of 47 consecutive patients with primary hyperparathyroidism who were treated with parathyroidectomy from October 1993 to June 2005.
RESULTS: Mean age was of 51.3 years. Our sample included 40 women (85%) and 7 men (15%). …

Parathyroid cysts rarely cause primary hyperparathyroidism. In most cases, the resultant hypercalcemia is mild and detected before any significant skeletal disease develops. We report a patient with severe hypercalcemia, a synchronous brown tumor (osteitis fibrosa cystica) of the maxilla, and a large benign functional parathyroid cyst. The unusual patient presentation and management are described and illustrated. The pertinent literature is reviewed.

Primary hyperparathyroidism (PHPT) is rare in children and may present with skeletal deformities. The present case report describes the case … e expansile, cystic lesions with a 'soap-bubble' appearance. A histopathological analysis confirmed osteitis fibrosa cystica (brown tumor). Radiofrequency ablation of the adenoma normalized the calcium and PTH levels, with the near-complete radiological resolution of lesions and the marked clinical improvement of genu valgum. The present case report highlights a rare pedi …

A case of long-standing idiopathic hypoparathyroidism that remained asymptomatic during pregnancy is described in a 25-year-old primipara. Clinical signs including carpopedal spasm, convulsions, and acute psychosis developed after the patient gave birth to a healthy infant. In contrast to previous reported cases, no signs of osteitis fibrosa cystica were found in the infant. This is believed to be the first such case described.

A middle-aged farmer and a female student presented with osteitis fibrosa cystica with nephrocalcinosis in the former and nephrolithiasis in the latter due to primary hyperparathyroidism. Hypercalcaemia was present in both cases and neck exploration revealed a large solitary parathyroid adenoma which was excised. Because primary hyperparathyroidism is less common in tropical countries doctors may be less familiar with the early manifestations of the disease. The number of patients thus undiagnosed and untreated is unknown.

BACKGROUND: Primary hyperparathyroidism is an endocrinopathic condition characterized by hypersecretion of parathyroid hormone. Excess parathyroid hormone results in an altered state of osseous metabolism involving bone resorption and tissue change known as osteitis fibrosa cystica, which is the end stage of primary hyperparathyroidism. Osteitis fibrosa cystica is associated with the development of brown tumors, which are rare because hyperparathyroidism is now usually diagnosed and treated before symptoms develop. Brown tumors are rarely the first symptom of hyperparathyroidism and can occasi…

Brown tumor is a localized form of osteitis fibrosa cystica, being part of the hyperparathyroid bone disease. It rarely is the first manifestation of hyperparathyroidism, since nowadays, the diagnosis is made at an asymptomatic or minimally symptomatic stage. We present a case of a left superior maxillar brown tumor as the first manifestation of primary hyperparathyroidism due to a parathyroid adenoma. A parathyroidectomy was performed, and there was a regression of the bone lesion, without the need of performing other local surgical procedures.

The differential diagnosis for an isolated lytic mastoid lesion is broad, encompassing various conditions requiring careful consideration. These include granulomatous disorders such as Langerhans cell histiocytosis and sarcoidosis, neoplastic processes like multiple myeloma, leukemia, lymphoma, and metastases, primary bone diseases such as Paget's disease, fibrous dysplasia, and osteitis fibrosa cystica, as well as infectious causes like osteomyelitis. In this report, we present a patient with otalgia and an isolated lytic mastoid lesion.
