Osteoporotic vertebral compression fractures and vertebral augmentation.

Vertebral fractures account for ~27% of all osteoporotic fractures in both men and women. The economic burden is substantial and growing: osteoporosis is expected to affect 14 million people by the year 2020. There is substantial morbidity associated with osteoporotic vertebral compression fractures (VCFs) including decreased quality of life, reduced pulmonary function, and increased mortality. Relatively recent additions to the treatment armamentarium include vertebral augmentation using vertebroplasty and kyphoplasty. Numerous retrospective and case studies demonstrate short-term efficacy an

[Prevalence of osteoporotic fractures in Tunisian women].

PURPOSE OF THE STUDY: Osteoporotic fractures are an important public health problem due to significant morbidity and mortality and the socioeconomic burden. In Tunisia, the impact of such fractures is often underestimated due to lack of epidemiological data. We evaluated the prevalence of these fractures in a sample population at risk in order to better ascertain the amplitude of this problem in our country. MATERIAL AND METHODS: This prospective study involved 2,000 menopaused women aged 50 years or more. The sample was randomly selected among the population of Manouba, an urban administrati

Kyphoplasty for treatment of osteoporotic vertebral fractures.

Cement reinforcement for the treatment of osteoporotic vertebral fractures is efficient mean with high success in pain release and prevention of further sintering of the reinforced vertebrae; however, the technique does not allow to address the kyphotic deformity. Kyphoplasty was designed to address the kyphotic deformity and help to realign the spine. It involves the percutaneous placement of an inflatable bone tamp into a vertebral body. Restoration of VB height and kyphosis correction is achieved by inflation of the bone tamp with liquid. After deflation, a cavity is created that eases the

Intravertebral clefts in osteoporotic vertebral compression fractures.

OBJECTIVE: To describe the characteristics, radiographic appearance, and significance of intravertebral clefts in vertebral compression fractures (VCFs) of patients with osteoporosis presenting for vertebroplasty. METHODS: This was a prospective radiographic study of 50 consecutive patients with 82 VCFs who underwent vertebroplasty at a tertiary referral center. Patients underwent imaging preoperatively with standing lateral and supine cross-table lateral radiographs and magnetic resonance imaging (MRI). Standing lateral radiographs were also obtained postoperatively. Clefts were defined at t

Percutaneous treatment of osteoporotic spinal compression fractures.

Osteoporotic spinal compression fractures are a significant health risk for elderly females, as well as other patient groups. These fractures are associated with an increased risk of mortality in comparison with age-matched controls. Recent techniques of minimally invasive surgical treatment show promising early results with respect to pain reduction and restoration of function, with low procedural complication rates.

Direct medical costs attributable to osteoporotic fractures.

Osteoporotic fractures are a major cause of morbidity in the elderly, the most rapidly growing segment of our population. We characterized the incremental direct medical costs following such fractures in a population-based cohort of men and women in Olmsted County, Minnesota. Cases included all County residents 50 years of age and older with an incident fracture due to minimal or moderate trauma between January 1, 1989 and January 1, 1992. For each case, a control of the same age (+/- 1 year) and sex who was attended in the local medical system in the same year was identified. Total incrementa

Detection of osteoporotic sacral fractures with radionuclides.

Osteoporotic sacral fractures usually occur in elderly patients as a result of mild trauma. Clinical symptoms range from localized sacral tenderness to neurologic problems attributable to sacral nerve root irritation or cauda equina compression. Although the radiographic diagnosis is difficult to establish, bone scans show a characteristic H-shaped pattern of radionuclide uptake across the sacrum and sacroiliac joints. Four cases of osteoporotic sacral fracture with confirmation by computed tomography are included in this report.

Obesity and Osteoporotic Fractures: Solving the Paradox.

Obesity is a global health issue linked to conditions like metabolic syndrome and heart disease, but its relationship with fracture risk is complex. While higher body mass index (BMI) is associated with increased bone mineral density (BMD) due to mechanical loading, it does not uniformly protect against fractures. Obese individuals face higher risks of proximal humerus and ankle fractures, along with propensity for increased falls and sarcopenic obesity. Higher fat mass, particularly visceral adiposity, correlates with lower bone quality and increased fracture risk, independent of BMD. Obesity

Identifying osteoporotic vertebral endplate and cortex fractures.

Osteoporosis is the most common metabolic bone disease, and vertebral fractures (VFs) are the most common osteoporotic fracture. A single atraumatic VF may lead to the diagnosis of osteoporosis. Prevalent VFs increase the risk of future vertebral and non-vertebral osteoporotic fracture independent of bone mineral density (BMD). The accurate and clear reporting of VF is essential to ensure patients with osteoporosis receive appropriate treatment. Radiologist has a vital role in the diagnosis of this disease. Several morphometrical and radiological methods for detecting osteoporotic VF have been

Determinants of mortality risk following osteoporotic fractures.

PURPOSE OF REVIEW: Increased mortality risk is accepted for hip and vertebral fracture. Recent data suggest that other fracture types have also been linked to excess mortality. This article reviews the existing evidence on the pattern and determinants of postfracture mortality. RECENT FINDINGS: The pattern of mortality over time following hip and vertebral fractures has recently been clarified. Nonhip nonvertebral fractures at major, and even minor sites in older individuals have also been associated with excess mortality. Studies have revealed the higher excess mortality in men and in younge

[Diagnostics and treatment of osteoporotic vertebral fractures].

Osteoporosis is a systemic skeletal disease characterized by low bone mass and microarchitectural deterioration of bone tissue, with vertebral compressive fractures (VCFs) that may occur as a consequence. Vertebral compression fractures (VCFs) can lead to severe acute and chronic pain, impaired mobility, reduced quality of life and an increased risk of mortality due to decreased mobility and pulmonary dysfunction. When painful VCFs do come to clinical attention, they are typically treated with optimal pain management (OPM). Although the natural course of pain due to vertebral fractures decreas

Vertebral augmentation in osteoporotic and osteolytic fractures.

PURPOSE OF REVIEW: To discuss recent advances in percutaneous minimally invasive vertebral augmentation techniques used in the treatment of osteolytic and osteoporotic compression fractures. RECENT FINDINGS: Recent data have shown that outcomes in patients with vertebral compression fractures treated with vertebral augmentation techniques have been encouraging. Several studies demonstrated the positive effects of vertebral augmentation on maintenance of postoperative pain reduction at long-term follow-up. There is more evidence on the relationship between vertebral augmentation and adjacent s

[Osteoporotic fractures in pregnancy - conjunction of factors?].

Pregnancy-associated osteoporosis is rare and self--limiting. We report two clinical cases, with beginning of back pain during a 1st pregnancy. In the 1st case, the pain became severe after a fall, and she had other risk factors of fracture, like low calcium intake, and a treatment with thyroid hormones for weight loss. In the 2nd case, we did not find any. Both were diagnosed of osteoporosis by bone densitometry and radiology (multiple vertebral compression fractures). They had a favorable progression with life-style measures, calcium and vitamin D supplementation, and oral biphosphonate in t

Osteoporotic fractures: a brain or bone disease?

Osteoporosis is a skeletal disorder that predisposes individuals to increased risk of fracture. However, most osteoporotic fractures occur in women who do not meet criteria for osteoporosis. Hence, bone density, by itself, is a relatively poor predictor of fracture. Age and age-related factors are now recognized as increasingly important in determining fracture risk. Osteoporotic fractures are associated with increased disability and mortality, suggesting that osteoporosis may be a clinical manifestation of an underlying disease process affecting multiple systems. The systems affected, the mus
