Use positional screws for Le Fort I osteotomy fixation: technical note.

In recent years, several studies related to fixation systems have been published, but few suggest any variations of the Le Fort I osteotomy technique, and the use of plates and screws placed along the canine and zygomatic pillar are common. The 20-year-old patient with Crouzon syndrome presented with severe hypoplasia of the maxillary, mandibular prognathism, and class III facial pattern. The patient underwent orthognathic surgery and high Le Fort I osteotomy with the fixation of eight positional screws. The aim of this paper is to describe a different technique for the fixation in high maxill

Postoperative maxillary stability after Le Fort I osteotomy using a u-HA/PLLA system: three-dimensional analysis by surface superimposition based on virtual Le Fort I osteotomy.

The postoperative stability achieved with Le Fort I osteotomy (LFI) using bioabsorbable systems remains controversial. A new method - multipoint measurement method - was devised for detailed three-dimensional examination of postoperative stability following LFI, and the stability after LFI when using SuperFIXSORB-MX made of u-HA/PLLA was investigated. Thirty-one patients who underwent LFI using SuperFIXSORB-MX were evaluated retrospectively. The patients were divided into four malocclusion types: open bite, mandibular retrognathia, mandibular protrusion, and facial asymmetry. Seven maxillary r

Simultaneous Le Fort III and Le Fort I Osteotomies for Rare Midfacial Hypoplasia in a Patient with 15q11.2 Microdeletion-A Multidisciplinary Approach.

15q11.2 microdeletion syndrome, also known as Burnside-Butler syndrome, is a rare partial autosomal monosomy associated with a spectrum of neurodevelopmental, cognitive, and behavioral disorders. Due to its atypical occurrence and limited research, craniofacial manifestations linked to this microdeletion remain poorly documented. This study presents a novel case of severe midface hypoplasia in a patient with 15q11.2 microdeletion syndrome, treated with a 2-stage surgical approach, and a review of the literature for simultaneous Le Fort III/I osteotomies. The first phase involved simultaneous L

Indications and limitations of CAD/CAM splints in Le Fort I osteotomy.

The surgical outcome of planned Le Fort I osteotomy (LFI) is important for successful orthognathic surgery. The aim of this study was to evaluate the accuracy of LFI using a CAD/CAM intermediate splint by performing a three-dimensional (3D) comparison of the planned and postoperative maxillary positions. This retrospective study evaluated 31 patients who underwent LFI with a CAD/CAM intermediate splint. The patients were classified into three skeletal malocclusion types: Class III, Class II, and facial asymmetry. Five maxillary reference points and two axes were measured using computed tomogra

Hemostatic Efficacy of Oxidized Regenerated Cellulose Powder in Le Fort 1 Osteotomy.

A challenging aspect of Le Fort I osteotomy is bleeding control. Osteotomy techniques, devices, drugs, and anesthetic management have been reported to reduce bleeding; however, there are no reports on the use of hemostatic agents. We aimed to evaluate the hemostatic efficacy of a new topical absorbent hemostatic agent, Surgicel Powder, consisting of oxidized regenerated cellulose (ORC). We reviewed the records of 40 patients who underwent Le Fort I surgery for jaw deformities, with or without cleft lip and palate. Twenty of the 40 patients did not have cleft lips or cleft palates (CLCP); the r

[A safe technique of Le Fort I osteotomy in bilateral cleft patients].

Le Fort I osteotomy is often needed in bilateral cleft patients. The usual approach to the maxillae may compromise the premaxillary vascularisation. We present a subperiosteal exposure preserving two paramedian vascular pedicles that respect premaxillary blood-supply.

Navigation surgery for Le Fort 1 osteotomy in a fibrous dysplasia patient.

Orthognathic surgery is sometimes performed for fibrous dysplasia to correct malocclusion or facial asymmetry. However, Le Fort 1 osteotomy for this disease is difficult because of severe anatomical abnormality. Computer-assisted surgery is a rapidly developing technique in oral and maxillofacial surgery that is helping to ensure the safety of the surgery. We report a case of polyostotic craniofacial fibrous dysplasia in which two-jaw orthognathic surgery was performed using a navigation system with the Le Fort 1 osteotomy procedure. A 29-year-old woman presented with swelling and asymmetry on

Le Fort I osteotomy for extensive juvenile nasopharyngeal angiofibroma: a retrospective study.

INTRODUCTION: Juvenile nasopharyngeal angiofibroma (JNA) is a rare, nonencapsulated, benign neoplasm typically diagnosed in adolescent boys. Surgery is the usual treatment modality for JNA. The optimal surgical procedure should allow maximal exposure of the tumor for complete excision with minimum morbidity. One possible surgical approach is Le Fort I maxillary osteotomy. The aim of this study was to review our experience with the Le Fort I osteotomy and to investigate the feasibility of this approach for extensive JNA invaded into pterygopalatine and infratemporal fossae. METHODS: We retrosp

[Hybrid osteosynthesis in orthognathic surgery: 28 cases of Le Fort I osteotomy].

INTRODUCTION: The use of resorbable materials in orthognathic surgery is presently the object of numerous clinical and experimental studies. Their main drawbacks are their lack of rigidity and induced chronic inflammatory reactions. The purpose of this study was to propose and to test an original system of hybrid osteosynthesis associating a single titanium plate with three resorbable plates in Le Fort I osteotomies in the absence of maxillary expansion, or two titanium plates with two resorbable plates in case of maxillary expansion. METHODS: Our retrospective study concerned 28 patients ope

Anatomy of the Le Fort I segment: Are arterial variations a potential risk factor for avascular bone necrosis in Le Fort I osteotomies?

PURPOSE: Osteotomies of the Le Fort I segment are routine operations with low complication rates. Ischemic complications are rare, but can have severe consequences that may lead to avascular bone necrosis of the Le Fort I segment. Therefore the aim of this study was to investigate the blood supply and special arterial variants of the Le Fort I segment responsible for arterial hypoperfusion or ischemic avascular necrosis after surgery. MATERIAL AND METHODS: The arterial anatomy of the Le Fort I segment's blood supply using 30 halved human cadaver head specimens was analyzed after complete diss
