What Functional Outcomes Can Be Expected With Osseointegrated Prostheses in Transfemoral Amputations?

BACKGROUND: Individuals who have undergone transfemoral amputation often have difficulty donning and utilizing a conventional socket prosthesis. Osseointegrated prostheses may alleviate certain issues, potentially resulting in better outcomes, but few prospective studies have either evaluated these outcomes or quantified the purported benefits. QUESTIONS/PURPOSES: (1) Did patient-reported outcome measures improve between baseline and 2-year follow-up among those who underwent osseointegration surgery after transfemoral amputation? (2) What was the frequency and severity of early complications

The Role of Culture and Masculine Honor Ideology in Substance Use and Outcome Expectancies.

Background: A cultural construct that has rarely been investigated in substance use research is the culture of honor. In honor cultures, masculine honor ideology (the expectation that men display toughness, defend their manliness, and protect female family members) is prevalent and can influence people's health behaviors. In this study, we predicted that substance use will be positively associated with masculine honor ideology among South Asian Americans (an understudied honor culture), because as a risky health behavior, substance use can solidify one's reputation of invincibility and fearles

The modulation of reward expectancy on the processing of near-miss outcomes: An ERP study.

A near-miss is a situation in which a gambler almost wins but falls short by a small margin, which motivates gambling by making it feel like success is within reach. Existing research has extensively investigated the influence of contextual information on near-miss outcome processing; however, the impact of reward expectancy has received limited attention thus far. To address this gap, we utilized the wheel of fortune task and event-related potential technique (ERP) to quantify the electrophysiological responses associated with gambling outcomes at different levels of reward expectancy. Expectations and outcomes of varying treatment strategies for CML presenting during pregnancy.

Diagnosing chronic myeloid leukaemia (CML) during pregnancy is rare. Tyrosine kinase inhibitors (TKIs) have traditionally been contraindicated owing to their teratogenicity. Management decisions should consider the risks to mother and foetus of uncontrolled disease and teratogenic medications. Further cases are required to build upon the paucity of current literature. We report 22 cases of CML diagnosed during pregnancy from 2002 to date. Twenty-one pregnancies resulted in healthy babies and one patient miscarried. Some patients remained untreated throughout pregnancy but the majority received

Measuring General Expectations of Advanced Stage Treatment Outcomes in Parkinson's Disease.

BACKGROUND: Recent research suggests that a significant number of those who receive advanced treatments for Parkinson's disease (PD) do not report improvements for some symptoms, which may relate to their pre-treatment expectations. It is important that expectations of treatment are measured and discussed prior to advanced treatment. OBJECTIVE: The primary aim of this study was to develop a measure of treatment expectations of two advanced-stage treatments in PD, deep brain stimulation (DBS), and Levodopa/Carbidopa Intestinal Gel (LCIG). A secondary aim was to explore potential predictors of

Outcomes in gastroschisis: expectations in the postnatal period for simple vs complex gastroschisis.

OBJECTIVE: To provide generalizable estimates for expected outcomes of simple gastroschisis (SG) and complex gastroschisis (CG) patients from a large multi-institutional cohort for use during counseling. STUDY DESIGN: A retrospective study of 394 neonates with gastroschisis at 11 children's hospitals from January 2013 to March 2017 was performed. Analysis by Fisher's exact tests and Wilcoxon rank sum tests were performed. Outcomes of complex and simple gastroschisis are reported. RESULT: There were 315 (80%) SG and 79 (20%) CG. CG had increased time from birth to closure (6 vs 4.4 days), clo

Liver Transplantation: Patient Selection, Perioperative Surgical Issues, and Expected Outcomes.

Liver transplant rates are at an all-time high, with nearly 8000 liver transplants in 2015. Despite the increasing number of liver transplants performed per year, there is a widening gap of supply and demand on limited donor resources. Patient selection is a complex but necessary process to evaluate patients who will benefit from liver transplant while minimizing futile transplants. Efforts should also continue to focus on minimizing perioperative complications resulting in retransplantations and more targeted immunosuppression to minimize side effects and prolong patient survival.

Threat expectancy bias and treatment outcome in patients with panic disorder and agoraphobia.

BACKGROUND AND OBJECTIVES: Previous studies suggest that patients with panic disorder and agoraphobia (PD/A) tend to overestimate the associations between fear-relevant stimuli and threat. This so-called threat expectancy bias is thought to play a role in the development and treatment of anxiety disorders. The current study tested 1) whether patients with PD/A (N = 71) show increased threat expectancy ratings to fear-relevant and fear-irrelevant stimuli relative to a comparison group without an axis I disorder (N=65), and 2) whether threat expectancy bias before treatment predicts treatment ou

Infant and perinatal outcomes of triplet pregnancy in Auckland: better than expected?

AIM: There were two aims to the study: (1) to provide local outcome data that would be useful in counselling prospective parents of triplets; and (2) to address the deficit in accurate contemporary data on neurodevelopmental outcome and neonatal morbidity for those infants weighing less than 1500 g at birth. METHODS: We reviewed the outcome of triplet pregnancies born at National Women's Hospital / Auckland City Hospital (Auckland, New Zealand) for 1995-2005 inclusive. For this study triplet pregnancy was defined as a pregnancy beyond 20 weeks leading to registration of at least one birth. R

Treatment expectancy affects the outcome of cognitive-behavioral interventions in chronic pain.

Patients' initial beliefs about the success of a given pain treatment are shown to have an important influence on the final treatment outcome. The aims of the paper are to assess determinants of patients' treatment expectancy and to examine the extent to which treatment expectancy predicts the short-term and long-term outcome of cognitive-behavioral treatment of chronic pain. This study employs the data of 2 pooled randomized clinical trials evaluating the effectiveness of cognitive-behavioral interventions for 171 patients with fibromyalgia and chronic low back pain. Triplets: outcomes of expectant management versus multifetal reduction for 127 pregnancies.

OBJECTIVE: Our purpose was to compare outcomes of women with triplet gestations conceived via assisted reproductive technology who chose expectant management or multifetal pregnancy reduction. STUDY DESIGN: We performed a retrospective review of all women who initiated assisted reproductive technology cycles from August 1995 through July 1997 with ultrasonographic documentation of triplets exhibiting fetal heart tones at 9 weeks of gestation (N = 127). Patients were then uniformly referred to a maternal-fetal medicine specialist and to 3 centers offering multifetal pregnancy reduction. RESUL

Maternal and perinatal outcome of expectant management of premature rupture of membranes in the midtrimester.

The purpose of this clinical investigation was to determine the maternal and perinatal results of continuing pregnancy in 118 consecutive patients with premature rupture of the membranes at 16 to 26 weeks. The mean gestational age at diagnosis of premature rupture of the membranes was 23.1 +/- 2.7 weeks, with a median of 23.5. The interval from rupture to delivery ranged from 1 to 152 days, with a mean of 13. There was no correlation between gestational age at the time of rupture and the latency period. Thirty-five patients received tocolytic agents and 24 received steroids. Forty-eight percen

Total knee Arthroplasty: Patient expectations and outcomes in a resource constrained setting.

OBJECTIVE: Knee osteoarthritis significantly impacts the quality of life, particularly in advanced stages. Total knee arthroplasty (TKA) is a common treatment, but 20-30% of patients remain dissatisfied despite clinical improvements. This study evaluates TKA outcomes using patient-reported outcome measures (PROMs) in a low-middle-income country context. METHODOLOGY: A prospective cohort study from Aga Khan University Hospital of 70 patients with end-stage knee osteoarthritis undergoing TKA was conducted from May, 2023 to December, 2023, Improvement in Knee injury and Osteoarthritis Outcome Sc

Expectations before periacetabular osteotomy and relation to post-operative outcomes and satisfaction.

A periacetabular osteotomy (PAO) seeks to improve native hip function in patients with pre-arthritic dysplasia. Fulfilment of expectations has been shown to influence post-operative satisfaction in spine surgery and knee arthroplasty, but has not been investigated for PAOs. This study analyses relationships between preoperative patient expectations, patient-reported outcomes, and patient satisfaction in young adults undergoing PAO. Patients undergoing PAO in 2012-2014 at a tertiary care centre were asked to complete a preoperative survey indicating their expectations on a Likert scale (not imp

Duration of insomnia and success expectancy predict treatment outcome of iCBT for insomnia.

INTRODUCTION: Identifying prognostic factors of treatment outcome may assist in customizing an intervention to a patient's needs. Hence, we conducted a secondary analysis of data from a randomized controlled trial to investigate the effectiveness of an internet-based cognitive behavioral therapy for insomnia (iCBT-I) to find patient characteristics that may predict the change of insomnia severity after treatment. MATERIALS AND METHODS: In this exploratory analysis involving 94 chronic insomnia patients, we examined the predictive value of several self-reported measures, medical history, and s
