Mental health quality and outcome measurement and improvement in Norway.

PURPOSE OF REVIEW: To describe and discuss results from scientific articles in 2008-2009 on outcome and quality and improvement of these in the mental health services in Norway. RECENT FINDINGS: Good outcome is documented for several types of interventions and patient groups such as information campaigns to reduce duration of untreated psychosis, dynamic psychotherapy with transference interpretations for patients with less mature object relations, group and individual therapy for patients with personality disorders, parent training to reduce conduct problems in small children and opioid main

Anesthesia and outcome after partial hepatectomy for adult-to-adult donor transplantation.

PURPOSE OF REVIEW: The worldwide adoption of live liver donors as a source of donor organs for transplantation has been curtailed by the very real risk of complications in otherwise healthy people. Our objective in this review is to outline the perioperative management of the live liver donor for adult-to-adult transplantation. RECENT FINDINGS: The incidence and severity of complications following live liver donation is extremely variable, and reporting needs to be standardized if we are to improve the perioperative management and outcomes. Agreed definitions would clarify the incidence and s

The forest and the trees: relational and specific factors in addiction treatment.

AIMS: Increased expectations for the use of evidence-based methods in addiction treatment have fueled a debate regarding the relative importance of 'specific' versus 'common' factors in treatment outcome. This review explores the influence of these factors on addiction treatment outcome. METHODS: The authors review and link findings from four decades of research on specific and general factors in addiction treatment outcome research. FINDINGS: Although few would argue that what one does in addiction treatment is immaterial, outcome studies tend to find small to no difference when specific tr

Systematic review of cardiac rehabilitation guidelines: Quality and scope.

Cardiac rehabilitation is a comprehensive model of secondary prevention proven to reduce mortality and morbidity. The World Health Organization is developing a Package of Rehabilitation Interventions for implementation by ministries of health as part of universal healthcare across the continuum. Through a systematic review, we sought to identify the best-quality cardiac rehabilitation guidelines, and extract their recommendations for implementation by member states. A systematic search was undertaken of academic databases and guideline repositories, among other sources, through to April 2019,

The process and outcomes of a multimethod needs assessment at a homeless shelter.

Many factors contribute to homelessness, including extreme poverty, extended periods of unemployment, shortages of low-income housing, deinstitutionalization, and substance abuse. As a result, the needs of people who are homeless are broad and complex. This needs assessment used literature reviews, review of local documents and reports, participant observation, locus groups, and reflective journals to guide the development of an occupational performance skills program at one homeless shelter in south Florida. Through these methods, the role of occupational therapy was extended beyond direct se

Focus is key: Panic-focused interpretations are associated with symptomatic improvement in panic-focused psychodynamic psychotherapy.

Objective: This study examines whether, in panic-focused psychodynamic psychotherapy (PFPP), interpretations of conflicts that underlie anxiety (panic-focused or PF-interpretations) are specifically associated with subsequent panic disorder (PD) symptom improvement, over and above the provision of non-symptom-focused interpretations. Method: Technique use in Sessions 2 and 10 of a 24-session PFPP protocol was assessed for the 65 patients with complete outcome data randomized to PFPP in a two-site trial of psychotherapies for PD. Sessions were rated in 15-min segments for therapists' use of PF-

Use of Objective Metrics in Dynamic Facial Reanimation: A Systematic Review.

IMPORTANCE: Facial nerve deficits cause significant functional and social consequences for those affected. Existing techniques for dynamic restoration of facial nerve function are imperfect and result in a wide variety of outcomes. Currently, there is no standard objective instrument for facial movement as it relates to restorative techniques. OBJECTIVE: To determine what objective instruments of midface movement are used in outcome measurements for patients treated with dynamic methods for facial paralysis. DATA SOURCES: Database searches from January 1970 to June 2017 were performed in Pub

Deep brain stimulation parameters for dystonia: A systematic review.

INTRODUCTION: Programming of globus pallidus pars interna (GPi) deep brain stimulation (DBS) systems for dystonia is complex because clinical benefits are often gradual. Some groups have advocated starting DBS with higher electrical parameters, whereas others have suggested the opposite. This variability in programming, even within each dystonia subtype, makes it challenging to compare outcomes and program the generators. To determine how variable DBS for dystonia stimulation parameters are, we performed a systematic literature review. METHODS: A comprehensive systematic literature search for

Reentry interventions that address substance use: A systematic review.

Justice-involved individuals with substance use problems have heightened risk of relapse and recidivism after release from incarceration, making reentry a critical time to provide evidence-based treatments (EBTs) for substance use; however, the extent to which reentry interventions incorporate EBTs for substance use is unclear. This systematic review identified studies of reentry interventions in the past 10 years that address substance use, assessed whether EBTs were used, and explored which interventions were effective in reducing substance use and recidivism postrelease. Eligible studies in

The role of mastoidectomy in outcomes following tympanic membrane repair: a review.

OBJECTIVES/HYPOTHESIS: The role of mastoidectomy performed with tympanoplasty for tympanic membrane perforations in the absence of cholesteatoma remains controversial. Some authorities suggest that outcomes are improved by the pressure buffering contributed by a well-aerated mastoid. It is unclear, however, whether or not clinical evidence demonstrates a benefit by the addition of a mastoidectomy. STUDY DESIGN: Review of peer-reviewed publications. METHODS: The literature on tympanoplasty and mastoidectomy was reviewed, examining the success of tympanic membrane repair, control of drainage,

Medical photography: a reliable and objective method for documenting the results of reconstructive surgery of pectus excavatum.

Patients with a pectus excavatum frequently experience medical problems such as dyspnoea, fatigue and palpitations. After surgical intervention most patients are alleviated of their symptoms. Pectus excavatum can easily be diagnosed by a proper inspection of the chest. In the Atrium Medical Center photographic documentation of the deformation of the chest is part of the treatment protocol before and after surgical correction of a pectus excavatum. In addition to the normal photographic routine two special recording techniques are applied: measuring the depth of the excavation with a rule, and

Treatment of early onset hair pulling as a simple habit.

The authors evaluated the effects of response prevention, a treatment previously shown to be effective for routine thumb sucking and suggested to be effective for early onset trichotillomania, applied to hair pulling in a 2-year-old. Response prevention was used alone in two settings (bedtime and naptime) and combined with a brief time out in another (daytime). The authors also used a novel assessment, weight of hairs pulled, and the results indicated complete cessation of hair pulling. Corresponding photographic evidence indicated complete regrowth of hair lost to pulling. These results add t

How successful is screening for congenital disease of the hip?

The efficacy of neonatal screening for congenital disease of the hip is reviewed in relation to currently accepted criteria. There is poor validity of the tests used, imperfect understanding of the natural history of the condition, and absence of information on the outcome of treatment following screening compared to that following conventional diagnosis. The benefits of screening, although probably real, are therefore difficult to quantify. Strategies for increasing the efficiency of screening (maximizing the benefit:cost ratio) by mass screening after the neonatal period or by instituting ro

Are Positive Interventions Always Beneficial?

Can people improve their lives by smiling more, trying to have a better posture, and by thinking about good memories? Can individuals become more successful by deliberatively engaging in positive actions and thoughts? Do people feel better by following recommendations from naïve psychology? In the present article we discuss these questions, noting that although some popular interventions thought to be universally beneficial (e.g., inductions of happiness, self-affirmation, empowerment, self-distancing) can sometimes yield positive outcomes, at other times the outcomes can also be negative. Tak

[Predictors of treatment duration for inpatients with mental disorders--a systematic literature review].

OBJECTIVES: In a systematic literature review we examined patient-related predictors of inpatient treatment duration for mental disorders. METHODS: The databases Medline, Embase, Psyndex, PsycINFO and EBMR were systematically reviewed for studies in the years 1990-2009 regarding treatment duration of inpatients with ICD-10 diagnoses F3-F6. Also, their methodological quality was evaluated. RESULTS: Twenty-one studies examining 28 predictors (sociodemographic, clinical) were included. Longer treatment durations were found for patients living alone and with higher levels of initial symptom seve
