Empty oval window associated with ossicular malformation: Our experience and a review of the literature.

PURPOSE: Congenital malformations of the middle ear, particularly those affecting the ossicular chain and stapes, are uncommon yet significant causes of conductive hearing loss. Here, we report a unique case of stapes malformation with an empty oval window and provide a review of the literature to contextualize our findings. METHODS: We present the detailed diagnostic and therapeutic course of a 21-year-old female with unilateral conductive hearing loss, focusing on imaging, intraoperative findings, and management strategy. A comprehensive literature review (January 1950-January 2025) was und

Vestibulotomy above a severely displaced facial nerve.

CONCLUSIONS: Vestibulotomy above a severely displaced facial nerve represents a new surgical approach to achieve serviceable hearing. The lack of facial nerve injury and the potential for hearing restoration make this procedure feasible in patients who would otherwise be marginal or poor surgical candidates. OBJECTIVE: To investigate the feasibility of vestibulotomy above a severely displaced facial nerve. MATERIAL AND METHODS: Eight patients with severe congenital conductive hearing loss underwent vestibulotomy above a severely displaced facial nerve and hearing reconstruction between Janua

The facial nerve coursing across the promontory with a persistent stapedial artery.

A case of congenital bilateral conductive deafness is described. Some years ago absence of the oval window was found in the left ear. Exploratory tympanotomy of the right ear revealed a large persistent stapedial artery while the facial nerve took its course from the geniculate ganglion straight down across the promontory anteriorly of the stapedial artery.

Infrasound transmission in the human ear: Implications for acoustic and vestibular responses of the normal and dehiscent inner ear.

The transmission of infrasound within the human ear is not well understood. To investigate infrasound propagation through the middle and inner ear, velocities of the stapes and round window membrane were measured to very low frequencies (down to 0.9 Hz from 2000 Hz) in fresh cadaveric human specimens. Results from ear-canal sound stimulation responses show that below 200 Hz, the middle ear impedance is dominated by its stiffness term, limiting sound transmission to the inner ear. During air-conduction, normal ears have approximately equal volume velocities at the oval (stapes) and round window

Temporal bone histopathologic findings in oculoauriculovertebral dysplasia. Goldenhar's syndrome.

The right temporal bone of a 6-month-old patient with oculoauriculovertebral dysplasia (Goldenhar's syndrome) was examined histopathologically. The most striking abnormalities were deformity of the auricle, atresia of the external auditory canal, severe malformation of middle ear structures, and incomplete development of the oval window. No inner ear abnormalities were identified in this case.

Endoscopic Stapes Surgery.

The management of stapes fixation using transcanal endoscopic ear surgery is a viable alternative approach. The advantages of transcanal endoscopic ear surgery are improved visualization and illumination of the stapes and oval window regardless of the size and geometry of the external auditory canal. Footplate work and prosthesis placement pose significant but surmountable challenges using transcanal endoscopic ear surgery secondary to the lack of depth perception. Several recent studies demonstrate the efficacy and safety of transcanal endoscopic ear surgery.

Correlations between click-ABR / PTA outcomes and temporal bone malformation characteristics in patients with congenital aural atresia.

OBJECTIVE: To evaluate correlations between click auditory brainstem response (c-ABR) / pure tone audiometry (PTA) hearing thresholds and high-resolution computed tomography (HRCT) temporal bone abnormalities in congenital aural atresia (CAA) patients and identify key anatomical predictors of hearing loss. METHODS: This retrospective study included 64 CAA patients (74 ears). Auditory assessments comprised c-ABR (performed in infancy) and PTA (conducted in school-aged children), with temporal bone HRCT completed during the same period as PTA. Patients with prior surgical interventions were exc

Surgical anatomy and pathology of the round window.

A study of the surgical anatomy and pathology of the round window was performed on a total of 292 serially sectioned temporal bones. The surgical approach to the round window is discussed with consideration of the anatomical findings. In the pathological study obliteration of the round window niche occurred as a congenital anomaly, in otosclerosis, chronic otitis media and as an end result in suppurative labyrinthitis. Clinical reports seem to indicate that obliteration of the window is associated with significant hearing loss, contrary to the findings in experimental animals. In chronic otiti

Perilymphatic fistula with no visible leak of fluid into the middle ear: a new method of intraoperative diagnosis using electrocochleography.

A new method for the intraoperative diagnosis of a perilymphatic fistula is presented. Two cases are reported in which the test clearly indicated the presence of a fistula even though no visible leak of fluid was noted by the surgeon. In each case, there was a clear history of vertigo and hearing loss. Preoperatively, the presence of a perilymphatic fistula was suggested using raised intrathoracic pressure test in electrocochleography (ECochG). During the surgery, there was no visible leak of fluid into the middle ear. Electrocochleography was undertaken intraoperatively by placing an electrod

Window Coverage and Liquid Biopsy in the First-Line Therapy of Severe Sudden Sensorineural Hearing Loss.

INTRODUCTION: Based on clinical practice guidelines, the application of corticosteroids as a first-line therapy is common. Although sudden sensorineural hearing loss (SSHL) etiology is primarily idiopathic, hearing loss can result from a perilymphatic fistula (PLF). Recent findings show evidence of a specific rate of PLF based on a cochlin-tomoprotein (CTP) detection test. Based on this rate of PLF treatment, the concepts of SSNHL need to be re-evaluated. The present study aimed to evaluate CTP in SSNHL patients and compare pre-surgical and post-surgical pure tone hearing thresholds after roun

Functional outcomes of Vibrant Soundbridge applied on the middle ear windows in comparison with conventional hearing aids.

CONCLUSION: Vibrant Soundbridge (VSB) application to the middle ear windows yields better functional outcomes than conventional hearing aids. However, speech discrimination scores obtained with VSB and conventional hearing aids are similar. OBJECTIVE: To assess audiological outcomes of round and oval window applications of VSB in comparison with conventional hearing aids. METHODS: Nineteen adult patients were included in the study. The patients had mild to moderate, moderate or moderate to profound conductive or mixed hearing loss. During surgery the floating mass transducer (FMT) was placed

Imaging Criteria to Predict Surgical Difficulties During Stapes Surgery.

BACKGROUND AND PURPOSE: Stapes surgery for otosclerosis can be challenging if access to the oval window niche is restricted. The aim of this study was to determine the accuracy of the computed tomographic (CT) scan in the evaluation of anatomical distances, and to analyze its reliability in predicting surgical technical difficulties. MATERIAL AND METHODS: A total of 96 patients (101 ears) were enrolled in a prospective study between 2012 and May 2015. During surgery, we evaluated the distance D1 between the stapes and the facial nerve, distance D2 between the promontory and the facial nerve a

Association of spontaneous anterior fossa CSF rhinorrhea and congenital perilymphatic fistula in a patient with recurrent meningitis.

A case of recurrent meningitis associated with spontaneous cerebrospinal fluid (CSF) rhinorrhea and left sensorineural hearing loss in a 4-year-old boy was found to be due to simultaneous congenital defects. High resolution CT examination clearly showed an anterior fossa defect and an inner ear malformation, including demineralization in the region of the footplate of the stapes, and thus provided clear guidance for the surgeon.

The facial nerve in congenital middle ear malformations.

The two most common anomalies of the facial nerve encountered in patients with a congenital malformation of the middle ear are displacement of the nerve and lack of a bony cover, two conditions that place the nerve at risk of being injured by the unwary surgeon. Malformations of the stapes are often found in association with facial nerve anomalies and may range from underdevelopment to complete absence. A congenital absence of the oval window is not uncommon. The position of the facial nerve in relation to the location and maturation of the ossicles will determine the method of ossicular chain

[Fenestration of semicircular canal for congenital microtia].

The purpose of this paper is to present a surgical method for microtia(small auricle, small external audiotory meatus and small tympanic membrane). 7 cases received fenestration of semicircular canal. During the operation it was found that the patients had congenital absence of oval windows and stapes, significant low position of horizontal facial canal covering the oval windows. It was impossible to perform labyrinthotomy in these patients. The only choice of operation is fenestration of semicircular canal. 7 ears underwent the operation as described. 6 weeks-21 months follow-up showed that a
