PCSK9 inhibitors usage in South China: A Multi-Center Real-World Study.

BACKGROUND: Proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) are effective for lipid management in high-risk patients with atherosclerotic cardiovascular disease. Their inclusion in China's National Reimbursement Drug List and the Hong Kong Hospital Authority Drug Formulary in 2021 improved accessibility. The Guangdong-Hong Kong-Macau Greater Bay Area is a unique integrated economic zone in Southern China where divergent healthcare systems coexist. How these disparate policies create inequities in drug access remains unexplored for regional health integration. This study aimed

Signaling Demand with Cost-Effectiveness Analysis: A Case Study of PCSK9 Inhibitors.

OBJECTIVES: Healthcare decision-makers increasingly account for heterogeneity in cost-effectiveness across patient populations. We aimed to characterize the demand curve implied by this heterogeneity. This framework was applied in an analysis of proprotein convertase subtilisin/kexin type 9 inhibitor (PCSK9-I) therapy for Scottish adults with familial hypercholesterolemia (FH). METHODS: We showed how individual and subgroup level treatment value can be estimated by combining incremental health benefits and non-drug costs with a cost-effectiveness threshold, then annualized to obtain value-bas

Discovery of Novel PCSK9-LDLR PPI Inhibitors by a Structural Modification Approach.

Proprotein convertase subtilisin/kexin 9 (PCSK9) mediating the degradation of the hepatic low-density lipoprotein receptor (LDLR), has emerged as a novel therapeutic target for the treatment of hyperlipidemia. Herein, to disrupt the protein-protein interactions (PPIs) between PCSK9 and LDLR, a total of 43 compounds were designed and synthesized by structural modification of previous derived PCSK9 inhibitors. In the PCSK9-LDLR PPI interfering test, compounds B7, B23, and B27 showed inhibitory potency with IC50 values of 8.86 μM, 3.95 μM, and 5.02 μM, respectively, compared with the positive con

Therapeutic Potential of PCSK9 Inhibitors in Regulating Neuroinflammation in Acute Ischemic Stroke.

Acute ischemic stroke (AIS) poses a substantial risk of permanent disability and death globally, with neuroinflammation being a key driver of secondary brain damage post-stroke. Proprotein convertase subtilisin/kexin type 9 (PCSK9), beyond its well-accepted role in cholesterol metabolism through low-density lipoprotein receptor (LDLR) degradation, has emerged as an important mediator of neuroinflammation, making it an attractive new therapeutic target. This has sparked broader discussions about the potential pleiotropic effects of PCSK9 inhibitors on brain function. Proprotein convertase subti

Statins, PCSK9 inhibitors and cholesterol homeostasis: a view from within the hepatocyte.

Statins and PCSK9 inhibitors dramatically lower plasma LDL levels and dramatically increase LDL receptor number within hepatocyte cell membranes. It seems self-evident that total clearance of LDL particles from plasma and total delivery of cholesterol to the liver must increase in consequence. However, based on the results of stable isotope tracer studies, this analysis demonstrates the contrary to be the case. Statins do not change the production rate of LDL particles. Accordingly, at steady state, the clearance rate cannot change. Because LDL particles contain less cholesterol on statin ther

Identification and evaluation of a lipid-lowering small compound as a PCSK9 inhibitor.

INTRODUCTION: Hyperlipidemia is a key contributor to cardiovascular diseases, underscoring the necessity for alternative lipid-lowering treatments beyond statins. OBJECTIVES: This study aimed to synthesize and identify small, low-toxicity lipid-lowering compounds and investigate their mechanisms of action. METHODS: A series of tetrahydroisoquinoline compounds were synthesized, with HepG2 cells used to screen and identify B11 as a potent, low-toxicity lipid-lowering candidate. B11's efficacy was tested in various hyperlipidemic animal models, including C57BL/6 mice, hamsters, and humanized PC

Statins and PCSK9 inhibitors: What is their role in coronavirus disease 2019?

Statins and proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors interfere with several pathophysiological pathways of coronavirus disease 2019 (COVID-19). Statins may have a direct antiviral effect on severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) by inhibiting its main protease. Statin-induced up-regulation of angiotensin-converting enzyme 2 (ACE2) may also be beneficial, whereas cholesterol reduction might significantly suppress SARS-CoV-2 by either blocking its host-cell entry through the disruption of lipid rafts or by inhibiting its replication. Available human

[New perspectives in the treatment of hypercholesterolaemia since the availability of PCSK9 inhibitors].

The large clinical trials on cardiovascular prevention have demonstrated that the more atherogenic cholesterol is reduced the greater the preventive benefit, and neither a threshold value below which that effect disappears nor negative effects on health have been observed. Therefore, the objectives of hypercholesterolaemia control in patients at high cardiovascular risk are becoming ever stricter. The fact that most high-risk patients do not achieve these objectives requires, among other measures, rational use of available lipid-lowering drugs, including monoclonal antibodies that inhibit the

The Pharmacologic Role and Clinical Utility of PCSK9 Inhibitors for the Treatment of Hypercholesterolemia.

In addition to monoclonal antibodies against proprotein convertase subtilisin-kexin type 9 (PCSK9), vaccines against PCSK9 and smaller molecule inhibitors as well as RNA inhibitors of PCSK9 production have been created. The monoclonal antibodies against PCSK9 and the PCSK9 RNA inhibitors can reduce low-density lipoproteins (LDLs) by over 50%, non-high-density lipoprotein (HDL) cholesterol and triglycerides, and increasing HDL. Although effective in several homozygous familial hypercholesterolemia patient types, PCSK9 inhibitors does not work in all patient types. Outcome trials show no effects

Theoretical Cost-Effectiveness of PCSK9 Inhibitors in Stroke Due to Intracranial Atherosclerosis.

IMPORTANCE: Given the high costs of treatment with proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) and the limited US Food and Drug Administration label for these treatments in patients with stroke due to atherosclerotic disease, it is unknown whether these agents are cost-effective for reducing recurrent stroke events. OBJECTIVE: To estimate the theoretical cost-effectiveness of currently available PCSK9i for the prevention of recurrent stroke in patients with high-risk intracranial arterial stenosis. DESIGN, SETTING, AND PARTICIPANTS: This economic evaluation was a post h

Comparative Efficacy of Statins Versus PCSK9 Inhibitors in Coronary Heart Disease Treatment.

BACKGROUND: Combining PCSK9 (proprotein convertase subtilisin/kexin type 9) inhibitors with statins significantly lowers low-density lipoprotein cholesterol and reduces cardiovascular events in patients with coronary heart disease versus statins alone. However, it remains unclear which monotherapy offers greater cardiovascular benefit. METHODS: This prospective non-randomized real-world observational cohort study enrolled coronary heart disease inpatients from July 2020 to March 2024. Patients received either alirocumab (75 mg/2 weeks) or statins (atorvastatin 20 mg/day or rosuvastatin 10 mg/

Association of PCSK9 inhibitors versus statins with cancer incidence: a target trial emulation.

BACKGROUND: The relationship between PCSK9 inhibitors (PCSK9i) and cancer incidence remains poorly understood. This study is to investigate the association between PCSK9i use and cancer risk compared to statin therapy. METHODS: We conducted a retrospective cohort study using target trial emulation principles on de-identified electronic health records from the TriNetX network (July 2015 – June 2023). Adults with indications for lipid-lowering therapy who initiated PCSK9i (alirocumab, evolocumab, or inclisiran) or statins (no prior statin use) were included. New PCSK9i users were propensity-scor

Role of PCSK9 inhibitors in venous thromboembolism: current evidence and unmet clinical needs.

Proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) have recently emerged as promising therapeutic agents for lowering low-density lipoprotein cholesterol and reducing the risk of cardiovascular events. Moreover, preliminary evidence from randomized controlled trials (RCTs) suggests that PCSK9i may also offer beneficial effects for patients following venous thromboembolism (VTE), with the most significant reductions in risk appearing over time, particularly beyond the first year of treatment. However, there is a lack of randomized controlled data supporting their efficacy and saf
