Xanthogranulomatous pancreatitis mimicking potentially malignant pancreatic neoplasm: report of a case.

Xanthogranulomatous pancreatitis (XGP) is a rare benign disease that may mimic or accompany other pancreatic diseases. Here we report a case of XGP initially suspected as malignant cystic neoplasm of the pancreas. A 64-year-old man had been incidentally found to have hypodense lesion at the body of pancreas during a lung cancer workup. All laboratory tests were within normal limits except that carcinoembryonic antigen was elevated to 31.3 ng/ml. Imaging study showed 1.8 cm sized, well demarcated, and low-attenuated mass on computed tomography (CT) with heterogeneously high intensity on T2-weig

Pyogenic pancreatic abscess mimicking pancreatic neoplasm: a four-case series.

A pyogenic pancreatic abscess mimicking pancreatic neoplasm in the absence of acute pancreatitis is rare. We report four patients who each presented with a pancreatic mass at the pancreas head or body without acute pancreatitis. The presenting symptoms were abdominal pain, fever, or weight loss. Abdominal CT scans showed low-density round masses at the pancreas head or body with/without lymphadenopathy. In each case, a PET-CT scan showed a mass with a high SUV, indicating possible malignancy. Comorbid diseases were identified in all patients: chronic pancreatitis and thrombus at the portal vei

Accuracy of cytologic interpretation of pancreatic neoplasms by fine needle aspiration and pancreatic duct brushings.

OBJECTIVE: To investigate the accuracy of fine needle aspiration (FNA) specimens and pancreatic duct brushings in the detection of pancreatic lesions and to compare the results with follow-up biopsy and/or surgical interpretation. STUDY DESIGN: We reviewed a total of 57 specimens (37/20), 37 FNA specimens and 20 pancreatic duct brushings, from 45 patients treated at Froedtert Memorial Lutheran Hospital, affiliated with the Medical College of Wisconsin, Milwaukee, over a 4-year period. The FNA and brushing samples were categorized as follows: positive for malignancy (21/3 = 24), suspicious for

Gruber-Frantz tumor: a rare pancreatic neoplasm.

We present the case of a 37-year-old female with abdominal pain in the epigastrium radiating to the right flank of a month of evolution. On physical examination, the epigastric mass was palpated, firm and painless. Computed tomography (CT) showed a cystic tumor in the body and tail of the pancreas with solid areas and defined borders (12 x 10 cm), which displaced structures. Endoscopic ultrasound (EUS)-guided fine needle biopsy was performed, with a cytology consistent with solid pseudopapillary tumor (SPT). Subsequently, a distal pancreatectomy with tumor resection and nodal dissection were p

Coincidence of GIST and pancreatic endocrine neoplasm in neurofibromatosis.

Carcinoids of the ampulla of Vater are infrequent tumors of which a quarter of cases have been detected in patients with type I neurofibromatosis. This hereditary disease is also associated with gastrointestinal stromal tumors (GIST). However, the coincidence of these three entities together have only been formerly detected in five cases. A 53 year-old female patient, diagnosed with type I neurofibromatosis, with a malignant carcinoid of ampulla of Vater and multiple gastrointestinal stromal tumors in the duodenum and jejunum, was treated with total pancreatectomy and the excision of her intes

Abdominal lymphangioma masquerading as a pancreatic cystic neoplasm.

Lymphangiomas are congenital abnormalities of the lymphatics that occur predominantly in the head and neck, most often in children. We present an unusual case of a middle-aged man who had a large septated cystic lesion adherent to the pancreas that could not be differentiated from a pancreatic cystic neoplasm despite analysis by ultrasound, computed tomography, magnetic resonance imaging, or percutaneous needle aspiration. The correct diagnosis was established only via laparotomy and subsequent pathologic interpretation. Although lymphangiomas are rare, they should be included in the different

Hepatobiliary and Pancreatic neoplasms in patients with McCune-Albright syndrome.

BACKGROUND: McCune-Albright syndrome (MAS), which includes polycystic fibrous dysplasia, precocious puberty, and café au lait spots, is a rare disorder caused by somatic activating mutations of the GNAS gene. GNAS mutations have also been implicated in various sporadic tumors, including hepatobiliary and pancreatic neoplasms. AIM: The aim of this study was to assess the prevalence of hepatobiliary and pancreatic neoplasms in patients with McCune-Albright syndrome. PATIENTS AND METHODS: Nineteen patients diagnosed between 1995 and 2012 with MAS in a tertiary referral center for rare growth di

Cystic pancreatic neoplasms evaluation by CT and magnetic resonance cholangiopancreatography.

CT provides limited assistance in the differentiation between serous and mucinous neoplasms. Because of the variability in the radiographic appearance of serous cystadenomas and overlap in CT characteristics with mucinous neoplasms, most serous neoplasms still require ancillary testing such as biopsy to reach a definitive diagnosis. MRCP is useful in differentiating benign and malignant mucinous tumors including IPMT of the pancreas. The presence of mural nodules is suggestive of malignancy; however, the absence of mural nodules does not indicate that the tumor is benign. A maximum main pancre

Inflammatory Pseudotumor of the Pancreas Mimicking a Pancreatic Neoplasm.

A 78-year-old woman was admitted to our hospital with a pancreatic tumor, incidentally discovered in an abdominal ultrasound exam. She was asymptomatic and without any previous personal pathological condition. The computed tomography (CT) and the magnetic resonance imaging (MRI) scan showed a mass lesion of 4 cm in diameter, located in the pancreatic body, conditioning the invasion of the splenic vein. The patient was admitted to surgery. During the laparotomy, we found a tumoral lesion highly suspicious of pancreatic neoplasia located in the transition of the head/body of the pancreas, with a

Unicentric Castleman's Disease Masquerading Pancreatic Neoplasm.

Castleman's disease is a rare nonclonal proliferative disorder of the lymph nodes with an unknown etiology. Common locations of Castleman's disease are mediastinum, neck, axilla, and abdomen. Castleman's disease of a peripancreatic location masquerading as pancreatic neoplasm is an even rarer entity. On search of published data, we came across about 17 cases published on peripancreatic Castleman's disease until now. Here we are reporting a case of retropancreatic Castleman's disease masquerading as retroperitoneal neoplasm in a 46-year-old male patient.

Pancreatic mucinous cystic neoplasm in a male patient.

CONTEXT: Mucinous cystic neoplasm (MCN) of the pancreas usually affects female patients and is characterized by an ovarian-type stroma. From literature review, only 9 cases of MCNs have been reported in male patients. CASE REPORT: We describe the 10th case of a MCN in a 65-year-old male patient who underwent a distal pancreatectomy with spleen resection and standard lymphadenectomy. CONCLUSIONS: MCN may rarely regard male patients, probably for embryological abnormalities.

Intraductal papillary mucinous neoplasm and acute pancreatitis.

Intraductal papillary mucinous neoplasms (IPMNs) are cystic pancreatic tumors that arise from the pancreatic ducts and are increasingly reported worldwide. Both benign and malignant tumors of the pancreas are thought to contribute to recurrent pancreatitis possibly by pancreatic duct obstruction, and IPMNs contribute to a major share of this burden. The rate of acute pancreatitis (AP) in IPMN patients in the largest published surgical series has varied from 12% to 67%. IPMN may be categorized into 3 forms on the basis of the areas of involvement: main pancreatic duct (MD-IPMN), side branch (SB

Intrapancreatic accessory spleen imitating a pancreatic neoplasm.

Accessory spleens are present in 10% of population and are formed during embryonic development. Besides the splenic hilum, the next most frequent localization of accessory spleens is the pancreatic tail. Intrapancreatic accessory spleens are usually diagnosed occasionally and make diagnostic difficulty because they imitate a pancreatic neoplasm. We present the case of a 61-year old woman with a mass in the pancreatic tail, diagnosed by computed tomography. The patient was operated with suspicion of neuroendocrine tumor. Postoperative histopathological examination revealed the intrapancreatic a

[Duodenal ulcer clinically simulating pancreatic neoplasm].

A small fraction of solid mass in the head of the pancreas are inflammatory rather neoplastic in origin. The failure to recognize the true nature of a solid tumor in the head of pancreas may lead to an incorrect treatment strategy. This is a report of a patient with clinical history of pancreatic cancer which computed tomography showed a mass in the head of the pancreas with radiologic characteristics of malignancy. The patient underwent exploratory laparotomy with the finding of a mass in the head of the pancreas. The inflammatory palpable characteristic of the mass led to a intraoperative bi

Predicting the Unpredictable: AI-Driven Prognosis in Pancreatic Neuroendocrine Neoplasms.

The clinical management of Pancreatic Neuroendocrine Neoplasms (Pan-NENs) is complicated by the disease's intrinsic variability, which creates significant hurdles for accurate risk profiling and the standardization of treatment protocols. Recently, Artificial Intelligence (AI) has offered a promising avenue to address these challenges. By integrating and processing high-dimensional multimodal datasets (encompassing clinical history, radiomics, and pathology), these computational tools can refine survival forecasts and support the development of personalized medicine. However, the transition fr

Extra-pancreatic malignancies in patients with intraductal papillary mucinous neoplasms.

INTRODUCTION: Intraductal papillary mucinous neoplasms (IPMNs) are precursor lesions of pancreatic ductal adenocarcinoma, and clinical markers of increased risk of extra-pancreatic malignancies (EPMs). However, the true burden, organ-specific risks, and optimal screening strategies for EPMs in this population remain unclear. AREAS COVERED: This review summarizes current evidence on the association between IPMNs and EPMs, focusing on colorectal, gastric, breast, lung, and renal cancers. Geographic and ethnic differences in cancer distribution are examined, with gastric cancer predominating in

Pancreatic Neoplasms Are Frighteningly Common: A Role for Immune Surveillance?

Carpenter and colleagues analyze organ donors to find that pancreatic intraepithelial neoplasia (PanIN), the precursor lesions of pancreatic ductal adenocarcinoma, are highly prevalent in the average healthy adult starting from a young age. Why these precursor lesions do not progress to cancer in most people is a mystery. See related article by Carpenter et al., p. 1324 (1).

Immunohistochemical Biomarkers of Gastrointestinal, Pancreatic, Pulmonary, and Thymic Neuroendocrine Neoplasms.

Neuroendocrine neoplasms (NENs) are a heterogeneous group of epithelial neoplastic proliferations that irrespective of their primary site share features of neural and endocrine differentiation including the presence of secretory granules, synaptic-like vesicles, and the ability to produce amine and/or peptide hormones. NENs encompass a wide spectrum of neoplasms ranging from well-differentiated indolent tumors to highly aggressive poorly differentiated neuroendocrine carcinomas. Most cases arise in the digestive system and in thoracic organs, i.e., the lung and thymus. A correct diagnostic app
