[Paraneoplastic syndromes in ENT surgery].

Paraneoplastic syndromes are extremely rare in otolaryngology. However, they can significantly influence both the therapy and outcome of patients with oncologic disease. This report deals with two cases of paraneoplastic syndromes associated with malignant tumors of the head and neck region. A 45-year-old male presented with a polymyalgia rheumatica, which was identified as a synchronous feature of a hypopharyngeal carcinoma. The symptoms disappeared after successful surgical treatment of the carcinoma. No analogous report has been published as yet. As demonstrated in a second case of a 54-year

[Paraneoplastic syndromes in otoneuro-ophthalmology].

INTRODUCTION AND OBJECTIVE: The so-called neurological paraneoplastic syndromes (NPNS) are a group of diseases of the central nervous system of unknown etiology which are seen almost exclusively in patients with cancer. We review the main NPNS paying particular attention to those with ophthalmological and otological features. DEVELOPMENT: Certain neuro-ophthalmological findings may constitute, at least partly, some paraneoplastic syndromes. There are alterations of vision in paraneoplastic retinopathy and in optic neuritis of paraneoplastic origin. The latter, unlike the retinopathy, usually

The neuropathology of paraneoplastic syndromes.

The term "paraneoplastic neurological syndromes" encompasses a number of uncommon disorders associated with systemic malignancies. In order to be classified a paraneoplastic neurological syndrome, the malignancies must not invade, compress, or metastasize to the nervous system. They can either focally or diffusely involve the central and peripheral nervous system or the neuromuscular junction. This paper reviews the neuropathology of the syndrome. It will first describe the clinical presentation and give an account of the systemic tumors most commonly associated with the various types of disor

Oculomucocutaneous changes as paraneoplastic syndromes.

Bullous dermatoses such as erythema exsudativum multiform major (EEMM) and bullous pemphigoid can lead to severe ocular involvement. In rare cases, both diseases develop as paraneoplastic syndromes. The immunopathologic mechanisms are discussed. A 69-year-old woman with non-Hodgkin's lymphoma (NHL) of grade IIIb developed EEMM while under systemic treatment with Fluconazole, Ofloxacin, and/or a combination of sulfamethoxazole and trimethoprim after polychemotherapy. In the eye, conjunctival necrosis with sicca syndrome led to Staphylococcus aureus-induced corneal superinfection, perforation, a

[Paraneoplastic syndromes in internal medicine].

Paraneoplastic syndromes supplement the complex clinical picture of malignomas. Their knowledge improves early diagnosis. The development, course and regression of paraneoplastic syndromes indicates that they have a multifactorial origin and their manifestation depends on the biochemical, metabolic and immunological situation of the organism. The pathogenesis involves many obscure points and the clinical picture is often a surprise, an unexpected variant with regard to theoretical expectations. Sometimes paraneoplastic syndromes prove fatal in patients with a tretable tumour. There does not ex

[Paraneoplastic syndromes in bronchogenic carcinomas].

During 5 years the authors followed up 35 patients with bronchogenic carcinomas. Of these, 34.3% had diverse paraneoplastic manifestations. The paraneoplastic reactions determined by ectopic production of different hormones and other substances as well as so-called "masks" of the systemic diseases are reviewed. An attempt is made to specify the basic mechanisms of the pathogenesis of each paraneoplastic sign.

Paraneoplastic syndromes and the retina.

PURPOSE OF REVIEW: Paraneoplastic diseases of the retina are rare. Diagnosis can be challenging, and studies on management strategies are limited. The purpose of this review is to summarize our current understanding of the presentation, diagnosis, and management for cancer associated retinopathy, melanoma associated retinopathy, paraneoplastic vitelliform maculopathy, and bilateral diffuse uveal melanocytic proliferation. RECENT FINDINGS: Multimodal imaging can aid in the diagnosis of paraneoplastic retinopathies since early fundus findings and symptoms can be nonspecific. No standardized tre

[Autoimmune reactions and paraneoplastic syndromes].

CLINICAL ISSUE: Autoimmune disorders of the central nervous system (CNS) are common but are also a heterogeneous group of diseases. The most common form is multiple sclerosis (MS), others are clinically isolated syndrome (CIS), acute demyelinating encephalomyelitis (ADEM) and neuromyelitis optica spectrum disorders (NMOSD). Paraneoplastic syndromes are rare and tumor-associated, they are not induced by direct invasion of tumor tissue but by tumor-associated autoantibodies mostly against specific CNS proteins, e. g. limbic encephalitis and paraneoplastic cerebellar ataxia or degeneration. Diagnostics of paraneoplastic neurological syndromes.

This document presents the guidelines for onconeural antibody testing that have been developed following a consensus process built on questionnaire-based surveys, internet contacts, and discussions at workshops of the sponsoring Italian Association of Neuroimmunology (AINI) congresses. Essential clinical information on paraneoplastic neurological syndromes, indications and limits of onconeural antibody testing, instructions for result interpretation, and an agreed laboratory protocol (Appendix) are reported for the communicative community of neurologists and clinical pathologists.

Paraneoplastic Syndromes in Neuro-Ophthalmology.

BACKGROUND: Paraneoplastic syndromes that affect the visual pathways and present with neuro-ophthalmologic signs or symptoms may involve the afferent or efferent systems. Afferent syndromes may involve the optic nerve or retina and, in some cases, these may be associated with systemic neurologic disease. Efferent symptoms typically affect eye movements and may involve the neuromuscular junction or involuntary eye movements. EVIDENCE ACQUISITION: Literature review and personal clinical and research experience. RESULTS: Diagnosis of paraneoplastic syndromes relies on clinical and laboratory ev

Oral cancer-associated paraneoplastic syndromes.

Paraneoplastic syndromes are cancer-associated endocrinological, haematological, dermatological or neurological disorders, which are directly related neither to the physical effects of the tumour mass, nor to invasion by the primary tumour, nor to metastasis of the tumour; nor are they associated either with the side-effects of anticancer treatment or with any of the complications of cancer. These syndromes are brought about by the ectopic production of biological mediators by the malignant tumour cells, or by immunological responses to the malignancy. Certain cancers are typically associated

Paraneoplastic syndromes and the kidney.

A paraneoplastic syndrome is defined as a group of symptoms that develop when substances released by some cancer cells disrupt the normal function of the surrounding cells and tissue. Paraneoplastic renal syndromes are diseases that indirectly compromise tubular and glomerular function by electrolyte imbalance, hormone-producing tumors or deposition of antigen-antibody complexes in the glomeruli. In order to describe the most common paraneoplastic syndromes, which may compromise the renal function, an extensive review was performed of papers, including case reports, guidelines, meta-analysis a

Paraneoplastic neurological syndromes: a review.

BACKGROUND: Paraneoplastic neurologic syndromes (PNS) constitute a rare group of disorders resulting from damage to the nervous system in the setting of cancer physically unrelated to the tumor site. PNS are believed to result from an autoimmune attack of normal neuronal tissue, spurred by similar neuronal antigens ectopically expressed by tumor cells. REVIEW SUMMARY: The most common PNS are reviewed and also their association with specific onconeural antibodies, some directly pathogenic, others whose role in the disease process is less clear-cut. This diversity in pathogenesis is likely due

Paraneoplastic syndromes of the CNS.

Major advances in the management of paraneoplastic neurologic disorders (PND) include the detection of new antineuronal antibodies, the improved characterisation of known syndromes, the discovery of new syndromes, and the use of CT and PET to reveal the associated tumours at an early stage. In addition, the definition of useful clinical criteria has facilitated the early recognition and treatment of these disorders. In this article, we review some classic concepts about PND and recent clinical and immunological developments, focusing on paraneoplastic cerebellar degeneration, opsoclonus-myoclo

[Mental disorders in paraneoplastic syndromes].

Usually paraneoplastic symptoms precede actual diagnosis of a neoplasm, however, may also be concurrent with it. They do not result from the localization of the primary lesion nor are associated with metastases or with other effects of presence and treatment of the neoplasm. The immunological etiology is assumed. In diagnostics estimation of level of the antionconeuronal antibodies may be helpful. Mental disorders in the course of paraneoplastic syndrome may coexist with neoplasm, but also may precede it (combined with neurological signs or without them). Such mental disorders have diverse cli

Rheumatic disorders as paraneoplastic syndromes.

The long-established observation that some rheumatologic disorders (RDs) are associated with--or precede--the clinical manifestations of a variety of solid and hematological tumors represents an important clue for the early diagnosis and effective treatment of the cancers. Inflammatory myopathies, seronegative rheumatoid arthritis and some atypical vasculitides are the most frequently reported paraneoplastic RDs, although paraneoplastic scleroderma- and lupus-like syndromes, erythema nodosum, and Raynaud's syndrome have also been observed. Generally, the clinical course of a paraneoplastic RD

Life-threatening paraneoplastic cutaneous syndromes.

Paraneoplastic syndromes are diseases or symptom complexes associated with malignancy, usually internal. In dermatology, we modify the definition to refer to dermatoses associated with internal malignancy. In this article, we discuss the link between malignancy and such dermatologic disorders as acanthosis nigricans, acrokeratosis paraneoplastica of Bazex, dermatomyositis, erythema gyratum repens, necrolytic migratory erythema (glucagonoma syndrome), and paraneoplastic pemphigus and discuss, where such information is known, the mechanism by which these paraneoplastic diseases occur.

[Diagnosis of paraneoplastic neurological syndromes].

INTRODUCTION: Paraneoplastic neurological syndromes are rare non-metastatic manifestations of malignancy. They are differentiated from side effects of tumor therapy, tumor-associated coagulopathy, infections, metabolic, and nutritional disorders. In the majority of cases neurological symptoms precede diagnosis of associated malignancy. Detection of anti-neuronal antibodies suggests a paraneoplastic mechanism. OBJECTIVES: To summarize syndromes, diagnostic steps, and currently available diagnostic possibilities. METHODS AND RESULTS: Serum and/or cerebrospinal fluid is analysed using indirect

[Lung cancer and paraneoplastic syndromes].

Paraneoplastic syndromes (PNS) are a relatively common manifestation of cancer, and in some cases they may be the first symptom. Lung cancer has the highest incidence of paraneoplastic syndrome. This fact is important considering a non explained endocrinological and neurological syndrome, it may facilitate a prompt diagnosis, and in some cases an adequate treatment. PNS evolution seems to be parallel to the subjacent cancer. PNS management requires specific measures, because in some cases, it may compromise the patient life. Neurological and endocrinological PNS associated to lung cancer are r
