[Opsoclonus-myoclonus syndrome: a case report].

Opsoclonus-myoclonus syndrome is a rare disorder. We report a 44-year-old patient with opsoclonus associated with a cerebellar syndrome revealing a small cell lung carcinoma. The treatment with chemotherapy initially improved the clinical symptoms but these eventually recurred. Opsoclonus is a complex disorder of the ocular motility, characterized by irregular, continuous and chaotic eye saccades. When it is associated with other manifestations of the central nervous system (head myoclonus and ataxia), it constitutes a clinical picture known as opsoclonus-myoclonus syndrome. In adults, the mos

Recent advance in immunological tests in paraneoplastic neurological syndrome.

Paraneoplastic neurological syndromes are uncommon, however; their diagnosis is of major practical importance. Any portion of the nervous system may be involved in paraneoplastic syndromes. There is increasing evidence that the pathogenesis of many paraneoplastic neurological syndromes appears to be an immune reaction against antigen shared by the cancer and the nervous system. The identification of antibodies in the serum or cerebrospinal fluid in the central nervous system of paraneoplastic syndrome patient confirms the clinical diagnosis of paraneoplastic syndrome, and allows early identifi

A case of ocular flutter associated with sole anti-sulphatide antibody positivity: successful treatment with intravenous immunoglobulin in a pediatric patient.

BACKGROUND: Ocular flutter is a neurological disorder characterized by irregular, rapid horizontal eye movements and is often associated with autoimmune diseases, infections, drug intoxication, or paraneoplastic syndromes. The brain regions involved in ocular flutter have not been definitively determined. Sulfatide is an acidic glycolipid crucial for maintaining myelin sheath integrity and neuronal transmission. Antibodies against sulfatide can disrupt neuronal signals, and their formation is linked to autoimmune conditions such as Guillain-Barré syndrome and GALOP syndrome. To our knowledge,

Case Report: Paraneoplastic melanoma-associated retinopathy mimicking posterior uveitis.

INTRODUCTION: Melanoma-associated retinopathy (MAR) is a rare paraneoplastic ocular manifestation of cutaneous or mucocutaneous melanoma. We describe a novel case of MAR that mimicked an inflammatory or infiltrative retinopathy. PATIENT PRESENTATION: A 68-year-old female developed bilateral, painless, floaters over six months with accompanying photopsia. Bilateral posterior uveitis was noted, and bilateral vitrectomies were non-diagnostic for malignancy. A systemic evaluation revealed a subcutaneous lesion on the medial aspect of her right thigh. Biopsy of the lesion was consistent with melan

The potential role of plasma exchange as a treatment for bilateral diffuse uveal melanocytic proliferation: a report of two cases.

Bilateral diffuse uveal melanocytic proliferation (BDUMP) is a rare paraneoplastic syndrome associated with gynecologic malignancies in women and pancreatic or lung carcinomas in men. The clinical presentation consists of the rapid onset of decreased visual acuity due to bilateral serous retinal detachment and cataracts. Pathologically, there is diffuse uveal thickening and proliferation of uveal melanocytes. The onset of blindness is often rapid, with some patients presenting with blindness. We describe the cases of two women with gynecologic malignancies who were treated with plasma exchange

Paraneoplastic ophthalmological syndromes.

The different pathogenic and clinical aspects of the paraneoplastic ophthalmological syndromes are discussed by the author. Opsoclonus-myoclonus, retinal degeneration are largely reviewed. It is possible that new modern immunological methods to elucidate the role of antibodies in the genesis of paraneoplastic syndrome and the role of these antigens in the functioning of nervous system. Recognising a ophthalmological syndrome as paraneoplastic will prompt a search for an early detection of a cancer and a more prompt treatment.

Cancer-induced, immune-mediated ocular degenerations.

The means whereby vision can be lost from a disease located distant from the eye include autoimmunity, with sensitization resulting from extraocular stimuli, a process illustrated here by the immunologic confusion caused by cancers. The uncontrolled proliferation of malignancies commonly involves the expression of components of the central nervous system, but a damaging loss of tolerance is rare. When autoimmunity does develop, organ-specific antigens are more often involved than the more generalized and widely disseminated common neuronal components. A focus upon a single antigen is typical o

Optic neuritis as the initial clinical presentation of limbic encephalitis: a case report.

BACKGROUND: Limbic encephalitis is characterized by rapid onset of working memory deficit, mood changes, and often seizures. The condition has a strong paraneoplastic association, but not all cases are invariably due to tumors. CASE PRESENTATION: We present a case of limbic encephalitis in a Chinese patient who initially presented to our hospital with optic neuritis and no other neurological symptoms. The diagnosis was made radiologically, and cognitive and neurological symptoms did not occur until 5 months later. Extensive investigations for autoimmune, infective, and neoplastic causes were

Ocular manifestations of common pulmonary diseases: a narrative review.

Several pulmonary disorders can cause ocular involvement. Understanding these manifestations is critical for early diagnosis and treatment. Hence, we set out to examine the most common ocular manifestations of asthma, chronic obstructive pulmonary disease (COPD), sarcoidosis, obstructive sleep apnea (OSA), and lung cancer. Allergic keratoconjunctivitis and dry eye are two ocular manifestations of bronchial asthma. The inhaled corticosteroids used to treat asthma can cause cataract formation. COPD is associated with ocular microvascular changes as a result of chronic hypoxia and systemic inflam

Neuro-ophthalmologic manifestations of paraneoplastic syndromes.

Paraneoplastic syndromes with neuro-ophthalmologic manifestations may involve the central nervous system, cranial nerves, neuromuscular junction, optic nerve, uvea, or retina. Most of these disorders are related to immunologic mechanisms presumably triggered by the neoplastic expression of neuronal proteins. Accurate recognition is essential to appropriate management.

Paraneoplastic Papillitis as a Precursor to Colon Carcinoma: A Case of Unexpected Vision Loss Leading to Early Cancer Detection.

INTRODUCTION: Paraneoplastic neurological syndromes are rare manifestations of cancer, characterized by autoantibodies targeting neuronal antigens. These syndromes often precede or accompany cancer diagnosis, complicating their interpretation. In this case report, we present the first documented patient with paraneoplastic papillitis causing visual disturbances in association with colorectal adenocarcinoma. CASE PRESENTATION: A 76-year-old female presented with acute vision loss and difficulty walking. A multidisciplinary team, including an ophthalmologist, neurologist, gastroenterologist, ra

Management of Bilateral Diffuse Uveal Melanocytic Proliferation (BDUMP)-A Case Report.

Background and Objectives: This study reports a case of a 62-year-old patient experiencing a significant decline in vision over the past three months. The initial best-corrected visual acuity (BCVA) of 20/20 in both eyes diminished to 20/200 in the right eye (RE) and counting fingers (CF) in the left eye (LE) within this timeframe. The patient was diagnosed with stage 4 ovarian cancer just one month before the significant vision deterioration. Materials and Methods: A thorough ophthalmologic examination revealed a notable progression of cataracts and the presence of subretinal fluid on the pos

Bilateral diffuse uveal melanocytic proliferation with multifocal diffuse integumentary melanocytic proliferation paraneoplastic syndrome: A case report.

Bilateral diffuse uveal melanocytic proliferation (B-DUMP) is a rare paraneoplastic syndrome typically presenting with bilateral visual loss. B-DUMP is associated with extraocular systemic malignancies with the most common being lung cancer in males and uro-gynaecological cancer in females (mainly ovarian cancer). Cutaneous and/or mucosal involvement in patients with B-DUMP has been reported but it is not well characterised. Herein, we present a female in her 70s with diagnosis of stage IV vaginal clear-cell carcinoma and metastatic melanoma of unknown primary that developed progressive bilate

Paraneoplastic neuromyelitis optica spectrum disorders: three new cases and a review of the literature.

Neuromyelitis optica spectrum disorders (NMOSD) occasionally develop in patients with tumor in relation to aquaporin-4 IgG (AQP4-IgG), representing a new paraneoplastic phenomenon. We reported three patients with paraneoplastic NMOSD and provided a comprehensive review of the literature. A total of 34 cases with paraneoplastic NMOSD were identified from our own case database (n = 3) and the previous literature (n = 31). The median age at NMOSD-related symptom onset was 50.5 years, and 91% of the cases were female. 11 (32%) cases had breast carcinoma. In 15 (44%) cases, NMOSD-related symptoms p
