Impact of daily cotrimoxazole on clinical malaria and asymptomatic parasitemias in HIV-exposed, uninfected infants.

BACKGROUND: Cotrimoxazole preventive therapy (CPT) is recommended for all human immunodeficiency virus (HIV)-exposed infants to avoid opportunistic infections. Cotrimoxazole has antimalarial effects and appears to reduce clinical malaria infections, but the impact on asymptomatic malaria infections is unknown. METHODS: We conducted an observational cohort study using data and dried blood spots (DBSs) from the Breastfeeding, Antiretrovirals and Nutrition study to evaluate the impact of CPT on malaria infection during peak malaria season in Lilongwe, Malawi. We compared malaria incidence 1 year

Trypanosoma rhodesiense: variable effects of cyclophosphamide on antibody production, survival, and parasitemia in infected mice.

Mice of the CBA/CaJ strain, infected with Trypanosoma rhodesiense, were injected with a single high dose (approximately 200 mg/kg) of the immunosuppressive drug cyclophosphamide to determine if an induced, transient inability to make antibody affected survival or parasitemia. When given on the day of infection, the drug had no significant effect on survival. It delayed, but did not prevent, the appearance of specific antibodies and the clearance of the infecting trypanosome variants. When cyclophosphamide was injected 1 week after infection, survival mass significantly decreased. Antibody prod

Cytokine-induced killer (CIK) cells inhibit Plasmodium falciparum parasitemia through cytolytic effector activity in vitro.

Cytokine-induced killer (CIK) cells, with dual traits resembling natural killer (NK) and T cells, have shown a promising clinical efficacy against cancer in clinical application leading to licensing of CIK cells in many countries. Here, we demonstrated that CIK cells can also inhibit the growth of Plasmodium falciparum in vitro, leading to a significant reduction in parasitemia levels after 24 h. We found that CIK cells cytotoxicity against infected RBCs is mostly dependent on their secretions rather than cell to cell communication, as they are a substantial repository of lytic agents, includi

The acid ceramidase/ceramide axis controls parasitemia in Plasmodium yoelii-infected mice by regulating erythropoiesis.

ABSTRACT: Gender-affirming hormone therapy (GAHT) facilitates the desired phenotypic changes consistent with an individual's gender identity. However, inter-individual variability in treatment effects has been reported at the endocrine and testicular tissue levels. To address the hypothesis that treatment effects are not homogeneous, we selected 24 persons from a cohort of 453 persons assigned male at birth, based on a similar dose of cyproterone acetate (Androcur®, 10 or 12.5 mg/day). Principal component analysis and hierarchical clustering on principal components were performed considering c

Steroid Pulse Therapy May Mitigate Prolonged Neurological Manifestations after Eradication of Severe Plasmodium falciparum Parasitemia.

A 58-year-old Japanese man with a high parasitemia of Plasmodium falciparum, returning from Uganda, was admitted to our hospital since his consciousness level rapidly deteriorated after the initial dose of mefloquine. Despite the parasitemia was cleared by quinine by day 7, the coma remained unchanged and diffuse leukoencephalopathy was detected on magnetic resonance image. Steroid pulse therapy was initiated on day 8. Subsequently, the neurological manifestations improved and he was discharged on day 73 without any sequelae. Pathogenesis of P. falciparum causing cerebral malaria is diverse an

Competition for red blood cells can enhance Plasmodium vivax parasitemia in mixed-species malaria infections.

We assess the consequences of competition for red blood cells (RBCs) in co-infections with the two major agents of human malaria, Plasmodium vivax and Plasmodium falciparum, using differential equations to model the population dynamics of RBCs and parasites. P. vivax parasitizes only the youngest RBCs, but this can reduce the broader RBC population susceptible to P. falciparum. We found that competition for RBCs typically causes one species to suppress the other, depending on their relative reproduction rates and timing of inoculation. However, if the species' reproduction rates are nearly equ

Trust the Process: Prolonged Babesia Parasitemia in an Elderly Man with Asplenia from the American Midwest.

BACKGROUND Babesia species are intraerythrocytic parasitic protozoa that are endemic to the Northeast and north Midwest of the United States. Babesia microti is the most common cause of babesiosis in North America and causes a malaria-like tick-borne parasitosis. Babesia is commonly transmitted through the bite of Ixodes species ticks, often concomitantly with other tick-borne organisms such as Borrelia burgdorferi, Ehrlichia, Rickettsia rickettsii, and Anaplasma phagocytophilum. In the Midwest, Lyme disease is the most common tick-borne illness, and other organisms can sometimes be overlooked

Novel Insights Into the Protective Role of Hemoglobin S and C Against Plasmodium falciparum Parasitemia.

Although hemoglobin S (HbS) and hemoglobin C (HbC) are well known to protect against severe Plasmodium falciparum malaria, conclusive evidence on their role against infection has not yet been obtained. Here we show, in 2 populations from Burkina Faso (2007-2008), that HbS is associated with a 70% reduction of harboring P. falciparum parasitemia at the heterozygous state (odds ratio [OR] for AS vs AA, 0.27; 95% confidence interval [CI], .11-.66; P = .004). There is no evidence of protection for HbC in the heterozygous state (OR for AC vs AA, 1.49; 95% CI, .69-3.21; P = .31), whereas protection

Comparison of PCR with blood smear and inoculation of small animals for diagnosis of Babesia microti parasitemia.

The specific diagnosis of babesiosis, which is caused by the piroplasm Babesia microti, is made by microscopic identification of the organism in Giemsa-stained thin blood smears, detection of babesial antibody in acute-and convalescent-phase sera, or identification of the organism following the injection of patient blood into laboratory animals. Although rapid diagnosis can be made with thin blood smears, parasites are often not visualized early in the course of infection. PCR is a new, rapid diagnostic technique for the detection of Babesia spp. that has not yet been systematically evaluated.

Chagas Disease: Parasitemia Identified via Serial Hemoculture in Blood Donor Candidates Infected with Trypanosoma cruzi.

The chronic phase of individuals infected with Trypanosoma cruzi is characterized by low parasitemia. Blood donors in endemic and some nonendemic countries are screened for antibodies against the parasite. Data on the parasitemia of individuals identified via serological tests, as well as the risk of transfusion transmission if the screening process fails to detect infected individuals, are scarce. The potential of culturing parasites during the chronic phase increases if blood collections are performed at least three times. Sequential hemoculture (three blood collections over 3 consecutive mo

A portable brightfield and fluorescence microscope toward automated malarial parasitemia quantification in thin blood smears.

Malaria is often most endemic in remote regions where diagnostic microscopy services are unavailable. In such regions, the use of rapid diagnostic tests fails to quantify parasitemia measurements which reflect the concentration of Plasmodium parasites in the bloodstream. Thus, novel diagnostic and monitoring technologies capable of providing such information could improve the quality of treatment, monitoring, and eradication efforts. A low-cost, portable microscope for gathering quantitative parasitemia data from fluorescently stained thin blood smears is presented. The system employs bimodal

Clinical relevance of low-density Plasmodium falciparum parasitemia in untreated febrile children: A cohort study.

BACKGROUND: Low-density (LD) Plasmodium infections are missed by standard malaria rapid diagnostic tests (standard mRDT) when the blood antigen concentration is below the detection threshold. The clinical impact of these LD infections is unknown. This study investigates the clinical presentation and outcome of untreated febrile children with LD infections attending primary care facilities in a moderately endemic area of Tanzania. METHODS/FINDINGS: This cohort study includes 2,801 febrile pediatric outpatients (median age 13.5 months [range 2-59], female:male ratio 0.8:1.0) recruited in Dar es

Cytomegalovirus vectors expressing Plasmodium knowlesi antigens induce immune responses that delay parasitemia upon sporozoite challenge.

The development of a sterilizing vaccine against malaria remains one of the highest priorities for global health research. While sporozoite vaccines targeting the pre-erythrocytic stage show great promise, it has not been possible to maintain efficacy long-term, likely due to an inability of these vaccines to maintain effector memory T cell responses in the liver. Vaccines based on human cytomegalovirus (HCMV) might overcome this limitation since vectors based on rhesus CMV (RhCMV), the homologous virus in rhesus macaques (RM), elicit and indefinitely maintain high frequency, non-exhausted eff

Preconditioning with hemin decreases Plasmodium chabaudi adami parasitemia and inhibits erythropoiesis in BALB/c mice.

Increased susceptibility to bacterial and viral infections and dysfunctional erythropoiesis are characteristic of malaria and other hemolytic hemoglobinopathies. High concentrations of free heme are common in these conditions but little is known about the effect of heme on adaptive immunity and erythropoiesis. Herein, we investigated the impact of heme (hemin) administration on immune parameters and steady state erythropoiesis in BALB/c mice, and on parasitemia and anemia during Plasmodium chabaudi adami infection. Intra-peritoneal injection of hemin (5 mg/Kg body weight) over three consecutiv

Impaired cell-mediated immunity in Plasmodium falciparum-infected patients with high-parasitemia and cerebral malaria.

Several cell-mediated functions were studied in vivo and in vitro in 63 Thai patients with acute falciparum malaria, including 21 cases with cerebral manifestations and 10 cases with initial parasitemia over 10%. Initial delayed cutaneous reactions to phytohemagglutinin and soluble protein antigens were negative in most cerebral malaria cases. In other patients, skin reactions were impaired or abolished as a direct function of parasitemia. No major alteration in the numbers of blood T and B lymphocytes was found. In lymphocyte cultures, proliferative responses to lectins were generally found w

Donor management implications for high-sensitivity detection of low-level malaria parasitemia in blood donor screening.

INTRODUCTION: Transfusion-transmitted malaria can occur when donors have unrecognized asymptomatic infection with submicroscopic parasitemia, highlighting the need to detect low-level parasitemia in blood donor screening. However, these highly sensitive blood donor testing algorithms may refer donors to clinicians who use diagnostic algorithms with less analytical sensitivity. Here we describe the analytical sensitivity of a Health Canada (HC)/FDA approved blood donor screening test compared to commonly used diagnostic test algorithms. Spontaneous Clearance of Polymerase Chain Reaction-Positive Plasmodium vivax Parasitemia in Northwestern Thailand: An Epidemiological Perspective.

BACKGROUND: Asymptomatic, low-density Plasmodium vivax infections pose a significant challenge to malaria elimination because of their subclinical presentation and potential for ongoing transmission. However, the natural history of such untreated infections, including the extent to which they resolve spontaneously, remains poorly understood and has not been systematically studied for P. vivax. METHODS: We conducted a longitudinal cohort study from 2018 to 2023 in northwestern Thailand, incorporating 14 rounds of mass blood surveys (MBS) covering approximately 5000 individuals. Metformin improves CD8+ T cell responses and parasitemia control via macrophage modulation during Trypanosoma cruzi infection.

The immune response to Trypanosoma cruzi infection relies on robust inflammatory activation of macrophages and proper CD8+ T cells function to control parasite replication. However, an exacerbated respiratory burst from macrophages can damage tissues where parasites reside, such as the heart and liver. Subsequent fibrotic repair in the heart contributes to cardiomyopathy in patients with chronic Chagas disease and in mouse models. Additionally, respiratory burst metabolites are implicated in the impairment of CD8+ T cell responses. While excessive reactive oxygen species production is associat

Evaluating Plasmodium falciparum automatic detection and parasitemia estimation: A comparative study on thin blood smear images.

Malaria is a deadly disease that is transmitted through mosquito bites. Microscopists use a microscope to examine thin blood smears at high magnification (1000x) to identify parasites in red blood cells (RBCs). Estimating parasitemia is essential in determining the severity of the Plasmodium falciparum infection and guiding treatment. However, this process is time-consuming, labor-intensive, and subject to variation, which can directly affect patient outcomes. In this retrospective study, we compared three methods for measuring parasitemia from a collection of anonymized thin blood smears of p
