Generalized joint laxity and contralateral patellar hypermobility in unilateral recurrent patellar dislocators.

PURPOSE: The purpose of this study was to assess the relation between generalized joint laxity and patellar hypermobility in unilateral recurrent patellar dislocators. TYPE OF STUDY: Case series. METHODS: A total of 82 patients (23 male and 59 female patients) with unilateral recurrent patellar dislocation were studied compared with an age- and sex-matched control group. The modified Carter and Wilkinson criteria for generalized joint laxity and lateral patellar hypermobility test were examined. RESULTS: Generalized joint laxity (score of 4 or 5) was present in 20 patients (24%) with recurr

Intra-articular Chondral Fragment After Acute Patellar Dislocation.

An 18-year-old military cadet presented to a direct-access physical therapy clinic 3 days after sustaining a lateral patellar dislocation that self-reduced with extension of the knee. Suspecting a loose body, the physical therapist ordered radiographs, which did not demonstrate any associated fractures. Rapid disposition was required to determine her potential to participate in mandatory Army field training, so the patient was referred to an orthopaedic surgeon, who ordered magnetic resonance imaging to assess for intra-articular pathology. Magnetic resonance imaging demonstrated a full-thickn

Computational analysis of factors contributing to patellar dislocation.

Treatment for an initial incidence of patellar dislocation is usually conservative management; however, almost half of patients experience a subsequent, or multiple, dislocation(s). Patients often undergo multiple procedures which fail to treat the underlying anatomic abnormalities. The objective of this study was to evaluate interactions between key predisposing anatomic factors to patellar dislocation and identify combinations of abnormal factors which increase the risk of recurrent lateral dislocation. Four factors associated with lateral patellar dislocation were identified (sulcus angle,

Patellar dislocation with genu valgum treated by DFO.

Congenital habitual patellar dislocation is a rare condition of the knee where the patella dislocates during flexion and relocates during extension. The congenital form is permanent, irreducible, and presents at birth. It is characterized by a short quadriceps and a major patellofemoral dysplasia and short height. This article presents a rare case of a 27-year-old woman with recurring bilateral habitual dislocation of the patella after a failed previous proximal and distal realignment procedure. Clinical examinations of both knees revealed genu valgus knees and lateral joint pain that recurred

Traumatic patellar dislocation: loose bodies and the MPFL.

Traumatic patellar dislocation is a common occurrence in adolescents. Treatment in the acute situation is controversial but should always address replacement or removal of osteochondral loose bodies. Reconstruction of the medial patellofemoral ligament in the acute and recurrent situation has provided excellent results in this select patient group.

Extra-articular patellar dislocation with vertical axis rotation.

Extra-articular vertical dislocation is very rare. It occurs when the patella is forced crossing the lateral femoral trochlear surface, and rotated simultaneously along its longitudinal axis at about 90°, resulting in a wedged dislocation against lateral condyle. Reduction has been reported to require general anesthesia and an open procedure. We report a 19-year-old boy with extra-articular patellar dislocation, and a successful reduction was done by manipulation under local anesthesia. We believe that the mechanism of lateral vertical dislocation is the fulcrum mechanism.

Ligament reconstruction versus distal realignment for patellar dislocation.

UNLABELLED: Recently, medial patellofemoral ligament reconstruction has been emphasized for the treatment of patellar dislocation. This study compared the results of medial patellofemoral ligament reconstruction by adductor magnus tenodesis with distal patellar realignment in patients with recurrent patellar dislocation. Additionally, the development of patellofemoral osteoarthrosis was compared for these two procedures at a median 10-year followup. Between 1994 and 2000, 47 consecutive patients were treated for recurrent patellar dislocation by adductor magnus tenodesis (18 knees) or Roux-Gol

First-time traumatic patellar dislocation: a systematic review.

Acute patellar dislocations can result in patellar instability, pain, recurrent dislocations, decreased level of sporting activity, and patellofemoral arthritis. The initial management of a first-time traumatic patellar dislocation is controversial with no evidence-based consensus to guide decision making. Most first-time traumatic patellar dislocations have been traditionally treated nonoperatively; however, there has been a recent trend in initial surgical management. We performed a systematic review of Level I-IV studies to make evidence-based medicine recommendations on how a clinician sho

Recurrent patellar dislocation after medial patellofemoral ligament reconstruction.

We report on three cases of recurrent lateral patellar dislocation following a medial patellofemoral ligament (MPFL) reconstruction for patellar instability. In all three cases, an isolated MPFL reconstruction was performed with a double autogenous gracilis graft. The patellar fixation was done through bone tunnels. All three patients presented with a definite moderate to severe traumatic episode resulting in a recurrent patella dislocation and a transverse avulsion fracture at the medial rim of the patella. All three were treated by an open reduction and internal fixation with good results. N

Bilateral patellar dislocation associated with alpha-mannosidase deficiency.

Mannosidosis is an extremely rare genetic disease characterized by a deficiency of the lysosomal enzyme, alpha-mannosidase. This enzyme is necessary for cleavage of mannose from many glycoproteins. In the absence of this enzyme, mannose accumulates in cells throughout the body, including the joints and the synovium. This disease causes many skeletal changes including dysostosis multiplex, synovial hypertrophy, and Charcot-type joints. We report the case of a girl, aged 9 years and 6 months, who developed bilateral patellar dislocation and severe synovial hypertrophy secondary to alpha-mannosid

Periprosthetic patellar fracture after an open knee dislocation.

We present a patient who sustained an open knee dislocation with a periprosthetic fracture of the patella and who was treated satisfactorily with tension band wiring and a conservative rehabilitation program. In addition to describing this unique combination of injuries, we review the literature on the treatment of periprosthetic patellar fractures. Although internal fixation of these fractures historically has provided unpredictable results, we think that it is warranted if the patella has adequate bone stock.

Epidemiology and natural history of acute patellar dislocation.

BACKGROUND: The goals of this study were to (1) define the epidemiology of acute patellar dislocation, (2) determine the risk of subsequent patellar instability episodes (subluxation and/or redislocation) during the study period, and (3) identify risk factors for subsequent instability episodes. STUDY DESIGN: Prospective cohort study. METHODS: The authors prospectively followed 189 patients for a period of 2 to 5 years. Historical data, injury mechanisms, and physical and radiographic measurements were recorded to identify potential risk factors for poor outcomes. RESULTS: Risk was highest

[The value of arthroscopy following traumatic patellar dislocation].

Traumatic dislocations of the patella and associated osteochondral fractures can be easily diagnosed by arthroscopy. During 1984-1986 acute arthroscopy was performed in 280 patients with traumatic haemarthrosis of the knee joint. In 26 cases the intraarticular bleeding was caused by patellar dislocation. Most of these injuries occurred during sports activities. Arthroscopic surgery gave excellent results in most of the cases. Arthrotomy had to be performed in 3 patients only.

Acute patellar dislocation: results of immediate surgical repair.

Seventeen cases of acute, first time, lateral patellar dislocations without fracture treated by immediate surgical repair were reviewed and evaluated. Nine patellar tendon realignments and 8 medial side reefing procedures were performed. There were no recurrences of patellar dislocation. Twelve knees were painful, of which 8 were rarely painful. All but one patient were satisfied with their results and participated in any desired sport. Surgical repair of acute patellar dislocation is advocated in the young patient who anticipates future participation in athletics.

The Importance of Medial Patellar Shape as a Risk Factor for Recurrent Patellar Dislocation in Adults.

BACKGROUND: Research on the cause of lateral patellar dislocation (LPD) has focused on trochlear morphologic parameters, joint alignment, and patellofemoral soft tissue forces. A paucity of information is available regarding how patellar morphologic parameters influence the risk for LPD. PURPOSE/HYPOTHESIS: The purpose was to assess whether patellar morphology is a risk factor for recurrent LPD. It was hypothesized that (1) patients with recurrent LPD would have decreased patellar width and volume and (2) patellar morphologic parameters would accurately discriminate patients with recurrent LP

Patellar instability: will my patella dislocate again?

PURPOSE OF REVIEW: The incidence of patellar instability in pediatric patients ranges is 50-100 in 100,000 patients per year. Risk of recurrent dislocations however has been cited from 8.6% to 88% depending on individual patient factors. This manuscript highlights the demographical, historical, and anatomic factors associated with recurrent patellar instability following a first-time patella dislocation in the pediatric population. RECENT FINDINGS: In recent years, various studies have focused on identifying risk factors for recurrent patellar instability following a primary patellar dislocat

[Analysis of the sonographic patterns in Traumatic Patellar Dislocation].

UNLABELLED: The diagnosis of osteochondral fractures and medial patellar retinaculum injuries that originate as a result of Traumatic Patellar Dislocation (TPD) appears to be crucial for establishing the treatment strategy. Those injuries may be efficiently recognized by means of ultrasonography (USG). MATERIAL AND METHODS: The analysis of the USG patterns in 22 knees after TPD in 21 patients treated between 2004 and 2007 was proceeded. In all children knee USG examination followed its clinical examination and X ray imaging. The standard USG knee assessment was supplemented by patellar articu

Functional treatment of patellar dislocation in an athletic population.

Fifty-eight athletically active study participants with 69 knees that had experienced patellar dislocations were available for followup after being selected for a functional rehabilitation program without antecedent immobilization. Follow-up evaluation was at a minimum of 24 months after onset of treatment and averaged 46.2 months. Good or excellent results occurred in 39 (66%) knees treated after an initial patellar dislocation and in 15 (50%) knees with a chronic history of patellar instability. Twenty-six percent of the 69 knees had experienced recurrent patellar instability at followup. Ov

Management of the First Patellar Dislocation: A Narrative Review.

First patellar dislocation is a common injury of the knee, involving often adolescents and the active population. The consequences of the first episode can be various and potentially disabling. Among these, acute patellar dislocation can often result in recurrent patellar instability. Recurrent patellar instability is certainly multifactorial but depends primarily on the injury of the medial patellofemoral ligament (MPFL), the major soft-tissue stabilizer of the patella. Some classifications are extremely useful in establishing the diagnosis and therapy in patellofemoral disease, in particular
