The surgical pathologist and laparoscopic gynecologic surgeries.

BACKGROUND: Laparoscopic surgery is a recent advance in the field of gynecological surgery. There are innumerable reports in literature on its advantages and disadvantages. However, problems faced by the surgical pathologist during grossing and histopathological reporting of these morcellated specimens have never been discussed before. We present our experience and the difficulties faced by a gynecologic pathologist (first author) and try to provide some clues for their solution. MATERIALS AND METHODS: Sample size was 153 consecutive laparoscopic specimens, which varied from in toto uterus wi

The pathologist's approach to small airways disease.

The small airways constitute a significant compartment of the lung, and diseases therein are not uncommon, occurring in a wide variety of diseases, either as a primary bronchiolar disorder or as a component of local or systemic disease affecting multiple anatomical compartments. Unlike the idiopathic interstitial pneumonias, there is as yet no consensus classification system for bronchiolar disorders, and a multidisciplinary approach to their classification is discussed. Whereas some conditions show relatively characteristic histological features, others show non-specific or subtle changes tha

The spectrum of laryngeal neoplasia: the pathologist's view.

Several types of neoplastic change with different prognostic implications typically involve the laryngeal squamous epithelium. The purpose of this review is to examine the spectrum of these changes, as well as their relationship to benign squamous epithelial proliferative states. Since these pathological changes are apt to occur in regions where the epithelial lining is typically squamous, it is important to recognize that the epithelium of the larynx varies from stratified squamous to respiratory-type, depending on the location. The lingual (anterior) surface of the epiglottis is lined by a s

Mucosal biopsy techniques and interaction with the pathologist.

The endoscopy era made it possible to see many of the diseases that were being treated by clinicians. The use of endoscopic biopsy further enhanced that ability. This article illustrates how gastrointestinal biopsy and other practices can be improved so that patients benefit more than they might otherwise. This article focuses on pinch biopsy forceps technique and on dialogue with the pathologist.

[Consequences of cholestasis from the pathologist's viewpoint].

The present review summarizes extrahepatic obstructive cholestasis and its hepatic morphological sequelae. The first part focuses on early and late changes visualized in liver biopsies. In the second part, pathogenic mechanisms involved in liver fibrosis are discussed. A third section deals with the morphological differentiation of liver atrophy occurring in obstruction and secondary biliary cirrhosis. Biliary obstruction induces a wide array of typical early and late changes of liver morphology. They comprise the visible accumulation of bile in hepatocytes and macrophages, liver cell damage (

The pathologist and the hydropic placenta, fetus, or infant.

Many anatomic and histologic abnormalities, in large variety, are associated with fetal and placental edema. It is the pathologist's responsibility first, to identify these abnormalities, and second, to propose or to establish a mechanism by which a candidate abnormality might have led to development of edema. The first process, of documentation, is often assisted by antenatal studies that have identified a candidate lesion. However, the pathologist must bear in mind that the presence of one disorder does not exclude others. The second process, of elucidating the abnormal physiologic mechanism

Wegener's granulomatosis. Thoughts and observations of a pathologist.

The clinical entity now known as Wegener's granulomatosis was first reported in 1936. Since then the disease's histopathology has been clarified, although its etiology remains unknown. Treatment requires immuno-suppressant therapy and careful following of affected patients. The present report reviews the historical background for the discovering of Wegener's granulomatosis and current clinical considerations.

The future of bioprosthetic valves. A pathologist's perspective.

The processes responsible for primary tissue failure of bioprostheses were illustrated. Calcification is the major cause of failure of porcine bioprosthetic valves, usually with superimposed tearing. Calcific deposits may be found grossly or microscopically in most bioprostheses removed from adult humans after four years. The predominate loci are the cuspal commissures and basal attachments, in the porcine spongiosa layer, in cells and in collagen. Pericardial valves also tear frequently, most often near the top of the stent post. The status of understanding of bioprosthetic valve calcificatio

The role of the pathologist in the surgical treatment of hyperparathyroidism.

Appropriately extensive surgical treatment of hyperparathyroidism depends upon accurate assessment of the extent of disease. We have believed that such assessement is the responsibility of the surgeon because at random biopsy with light microscopy the pathologist may not be able to differentiate adenoma from hyperplasia or even normal from abnormal glands. To test this hypothesis, three pathologists reviewed 50 unlabelled slides of parathyroid tissue and attempted to correlate them with clinical diagnoses which were based upon widely accepted criteria. They were asked to identify each slide as

The Pathologist's Role in the Diagnosis of Hirschsprung's Disease.

INTRODUCTION: The possibilities, recent advances, and pitfalls in the histopathologic workup of specimens submitted for the diagnosis of Hirschsprung's disease (HSCR) are presented. MATERIALS AND METHODS: A literature research was performed in the database PubMed including the years 2000 to 2024. RESULTS: The pathologist is involved in the diagnosis of HSCR in three scenarios: (1) the primary diagnosis in a child with the typical clinical symptoms, (2) the intra-operative guidance of surgery in a case where the diagnosis is already established, and (3) the confirmation of the diagnosis and t

Hirschsprung Disease for the Practicing Surgical Pathologist.

OBJECTIVES: Hirschsprung disease (HD) is a congenital condition defined by the absence of ganglion cells in the distal-most portion of the gastrointestinal tract. Biopsies and resections for HD can be adrenaline inducing for the general surgical pathologist because specimens are infrequent; HD is 1 of only a few neuroanatomic diseases that general surgical pathologists diagnose; numerous preanalytic factors (eg, biopsy adequacy, surgeon sampling protocol, processing artifacts) can affect histologic interpretation; and most importantly, the diagnosis has high stakes. METHODS: We provide a comp

[Hereditary breast carcinomas pathologist's perspective].

Breast cancers occurring in the context of a hereditary mutation of a predisposition gene represent 5 to 10% of all breast cancers, 20 to 25% of which being due to a mutation in the BRCA1 or BRCA2 genes. Authorization to market PARP inhibitors for breast cancer patients with hereditary BRCA1 and 2 mutations has recently been obtained. Given the annual frequency of breast cancer, morphological identification could facilitate the patient care process to limit the search for BRCA1 and 2 mutations to patients whose tumors have very specific characteristics. However, only a few morphological featur

Congenital coronary artery anomalies for the pathologist.

Congenital coronary artery anomalies are rare. Pathologists are exposed to those in mainly two settings; in association with sudden death and usually extreme exercise in young adults, and in association with complex congenital heart disease in the pediatric and perinatal population. Pediatric pathologists, other pathologists and pathologists' assistants performing pediatric or forensic autopsies therefore need to be familiar with coronary artery anomalies.

The Role of the Pathologist in Diagnosing Periprosthetic Infection.

Most patients who undergo total joint arthroplasty experience dramatic relief of pain and improved ambulation for many years, but some eventually develop pain, often accompanied by radiographic evidence of bone resorption around their implants. The most frequent cause of device failure is osteolysis, but infection is another important cause of pain and arthroplasty failure. The distinction between infection and aseptic loosening is important because the 2 conditions are treated very differently. The purpose of this article is to summarize the role of the anatomic and clinical pathologist in he

Primary pulmonary hypertension: pathologist as patient.

We describe the case of Julia Polak, an internationally known pathologist with an interest in primary pulmonary hypertension, a condition which she was subsequently diagnosed to have. This was successfully treated by heart/lung transplantation and she was then in the unique position to study and present her own pulmonary pathology. Professor Polak's case illustrates the difficulty in diagnosis, the frequent failure of medical treatment and an excellent outcome from heart/lung transplantation. Based on a presentation to a clinico-pathological conference at Hammersmith Hospital.

The pathologist's approach to the diagnosis of metabolic disease.

Many inborn errors of metabolism have tissue changes that suggest or indicate a diagnosis on morphologic bases. This emphasizes the involvement of the pathologist in the study of tissues obtained by biopsy as part of the diagnostic workup of cases with clinical evidence suggesting a metabolic disorder. Some of these diseases involve proteins with enzymatic activity with a consequent backlog of precursor metabolites. If these metabolites are insoluble or compartmentalized in cells, they accumulate in tissues. The diagnosis in those cases rests on establishing the nature of the accumulated mater
