Care of the trauma patient beyond the emergency department: a patient care standard to guide bedside nurses.

Injuries sustained from illicit drug use or alcohol intoxication are common in emergency departments. Ongoing assessments of psychosocial issues in trauma patients are imperative, even after the patient leaves the specialized area of the emergency department. Oftentimes, bedside nurses are ill prepared to identify the subtle clues of deeper psychosocial issues in complex patients such as trauma patients. This article focuses on the rationale for the development of a patient care standard to guide the bedside staff nurse in the care of the trauma patient. An example of a multiple trauma diagnos

Care of the Dying Patient: Maximizing Compassionate Care on the Battlefield.

Palliative care is a philosophy of treatment that focuses on relieving suffering while aligning care with a patient's goals. This treatment modality has proven benefits for civilian patients of all ages with both traumatic injuries and non-traumatic disease states. It results in the reduction of distressing symptoms and improved patient satisfaction, as well as reducing painful, ultimately futile interventions. Palliative care has also been employed throughout history on the battlefield and is a vital component of compassionate care for dying patients. Unfortunately, its use on the battlefield

[Complementary Therapy: Application of Photobiomodulation in Self-Care and Patient Care].

Photobiomodulation (PBM), also known as low-level laser therapy, is a non-invasive light therapy that applies near-infrared light sources near target tissues. PBM allows photons to penetrate tissues and interact with cells, promoting photophysical and chemical changes that result in desired changes at the molecular, cellular, and tissue levels (Oliveira et al., 2022; Shetty et al., 2023). This complementary therapy has garnered significant research attention both domestically and internationally. The results of recent research indicate non-invasive transcranial light stimulation can enhance hi

General Care of the Pregnant Patient in the Intensive Care Unit.

Pregnant women represent a small subset of all intensive care unit (ICU) admissions and may require intensive care for "obstetric" or "nonobstetric" reasons. Women may be admitted to the ICU at any stage of pregnancy or in the postpartum period. Pregnancy may be discovered at the time of admission to the ICU. Pregnancy impacts on ICU care in a variety of ways and requires a multidisciplinary approach to management. Pregnancy is associated with considerable physiological changes that affect most organ systems, including an expansion in blood volume, an increase in minute ventilation, and an inc

A national quality survey of in-patient diabetic foot care calls for centralizing care and improving patient communication.

AIMS: To evaluate the quality of inpatient care for patients with diabetic foot ulcers (DFU) and examine the relationship between managerial awareness of quality indicators and the actual care provided, as documented in the electronic medical records (EMRs). METHODS: A peer-reviewed audit of inpatient DFU care based on interviews with hospital and department management, yielding a comprehensive management score for each hospital; review of electronic medical records (EMR); and patient interviews. Data regarding facilities and personnel were collected as well. RESULTS: Data was collected from

Goals of Care Discussions in Medical Training: Integrating Palliative Care for Holistic, Patient-Centered Care.

Background: Goals of care discussions are essential communication skills in medical training that bridge patient values with clinical decision-making. Integrating palliative care principles into these conversations enables holistic, patient-centered care, yet medical trainees often lack structured preparation for these critical interactions. Objective: This narrative review examines how medical training can effectively integrate palliative care approaches into goals of care discussions through structured communication frameworks, interdisciplinary collaboration, and emerging innovations to pro

The patient participation puzzle: Organizing patient participation in health care networks.

INTRODUCTION: Health care is increasingly organized in interorganizational networks, with patient participation gaining prominence to better tailor care to patient needs. However, these often occur independently, limiting understanding of how to organize interorganizational patient participation. Poorly implemented participation risks draining time and energy from vulnerable groups without creating meaningful change. METHODS: We conducted an in-depth case study of a multihospital network, using 23 semistructured interviews with patients, health care staff, and network administrators, 77 hours

AI and Patient Trust in Health Care.

Health care providers are among the most trusted professionals, and they are rapidly adapting to AI integration in medicine. Authors Hou et al reviewed and synthesized qualitative studies of patient concerns regarding AI in health care. Themes included privacy, data security, and the "black box" complexity of AI decision-making; decreased trust in the physician-patient relationship and in the accountability of health systems; and equitable access, ethical regulation, and the displacement of human workers. Global and national health leaders can support health systems by establishing specific gu

Lifetime congenital urologic care: centering patient voices.

BACKGROUND: Adults with congenital urologic conditions, such as bladder exstrophy and vesicoureteral reflux, often require lifelong care that begins with childhood surgical reconstructions and extends through adolescence and adulthood with ongoing surveillance, follow-up care, and revision procedures when needed. Measurement of "success" can be difficult to quantify, as the definition of success may differ between parents and patients later in adulthood, and the definition of success should extend beyond f technical success to the broader care interventions and long-term outcomes, which often

Comprehensive Gynecologic Care of the Transfeminine Patient.

As visibility of transfeminine individuals increases, so does the need for inclusive and competent gender-affirming health care. Regardless of a patient's stage during their transition, gynecologists are essential health care providers for transfeminine patients. With the proper education and training, gynecologists can provide gender-affirming hormone therapy, assess and manage postoperative complications, and conduct routine health screening. The goal of this article is to provide an overview of available feminizing hormone therapies and effects, review feminizing surgical procedures, discus

[Perioperative care about a patient with multimorbidity].

The assumption of accelerated postoperative recovery according to the ERAS (Enhanced Recovery After Surgery) method is good condition of the patient prepared for surgery and gently operated on. Application of ERAS in postoperative care in the situation of patient with multimorbidity requires an individual approach and greater pre-operative preparation. During the healing process, the operation causes a systemic inflammatory reaction in the body, which is proportional to the size of the surgery trauma. There is fluid movement between the intravascular and interstitial spaces, increased protein

[Temporomandibular joint dysfunctions: evolution in patient care].

Temporomandibular joint dysfunctions are a frequently occurring condition that can have a considerable impact on the quality of life. The treatment modalities vary according to the anatomical involvement and symptomatology of the patient. Conservative and surgical management is still controversial and has evolved significantly in recent decades. The temporomandibular joint prosthesis is one of them. Through improvements in material and case management, the joint prosthesis has gained in popularity and has moved from being a treatment of last resort to being part of standard management.

Improving patient care pathways in otitis externa.

INTRODUCTION: Referrals for otitis externa (OE) have dramatically increased but the reasons for this remain unclear. We aim to characterize management of patients both pre- and post-referral to identify areas of improvement at the primary-secondary care interface. METHODS: Questionnaire study from consultant-led research clinic specifically set up to prospectively analyse OE referrals at a tertiary referral centre for Otolaryngology. RESULTS: Sixty-two patients were included; 63% female, median age 57 years. One was excluded (clinically not OE). Most patients had multiple primary care visits

Perioperative care of the adult diabetic patient.

The prevalence of diabetes mellitus is rapidly increasing in the UK, presenting greater complexity in its management when in the presence of other comorbidities. Palliative Care in the Hip Fracture Patient.

INTRODUCTION: Older patients with hip fracture have a 20% to 30% mortality rate in the year after surgery. Nonoperative care has higher 1-year mortality rates and is generally only pursued in those with an extraordinarily high surgical risk. As the population ages, more patients with hip fracture may fall into this category. The orthopedic surgeon is typically the main consultant responsible for deciding between surgery and conservative management, and the reasoning behind one decision over the other is often poorly understood. We undertook a review to determine decision-making tools for surge
