The effect of patient age on discharge destination and complications after lumbar spinal fusion.

Age is an important patient characteristic that has been correlated with specific outcomes after lumbar spine surgery. We performed a retrospective cohort study to model the effect of age on discharge destination and complications after a 1-level or multi-level lumbar spine fusion surgery. The American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database was used to identify patients who underwent lumbar spinal fusion surgery from 2013 through 2017. Perioperative outcomes were compared across ages 18 to 90 using multivariable nonlinear logistic regressioncontr

Patient perceptions of same-day discharge after minimally invasive gynecologic and pelvic reconstructive surgery.

BACKGROUND: Studies have demonstrated that same-day discharge after minimally invasive gynecological and pelvic reconstructive surgery is safe and cost effective and does not result in increased readmissions when compared with inpatient surgery. However, few studies have assessed patient satisfaction with same-day discharge after minimally invasive gynecological and pelvic reconstructive surgery. Increased knowledge of patients' values allows for evidence-based, patient-centered perioperative care and guides perioperative counseling. OBJECTIVES: The objectives of the study were to evaluate pa

Analysis of Risk Factors for Patient Readmission 30 Days Following Discharge From General Surgery.

IMPORTANCE: Previous studies investigating patients at risk for hospital readmissions focus on medical services and have found chronic conditions as contributors. Little is known, however, of the characteristics of patients readmitted from surgical services. OBJECTIVE: Surgical patients readmitted within 30 days following discharge were analyzed to identify opportunities for intervention in a cohort that may differ from the medical population. DESIGN, SETTING, AND PARTICIPANTS: Medical record review of patients readmitted to any service within 30 days of discharge from the general surgery se

Suicide risk after discharge from in-patient psychiatric care: A 15-year retrospective cohort study of individual patient data.

BACKGROUND: Suicide rates are known to be increased in patients after discharge from in-patient psychiatric treatment. However, evidence on risk factors for suicide within this patient group are contradictory. Thus, this study aims to investigate suicide after discharge from a sizeable psychiatric care facility to determine associated risk factors. METHODS: Data on individual patient level from a 15-year single-centre cohort were linked to data from the national death registry and cumulative incidence rates were calculated applying competing risk models. Independent variables included the pat

Development and testing of a patient-reported discharge readiness measure for daytime knee arthroscopy.

BACKGROUND: The lack of a specific assessment tool for knee arthroscopy patient leads to a lack of validity and accuracy of the assessment results, which affects the overall success of discharge preparation. This study aims to develop and psychometrically test the Patient-Reported Scale for assessing Discharge Readiness in patients undergoing daytime Knee Arthroscopy. METHODS: This was a quantitative empirical research design. Development of the patient-reported instrument entailed three stages: item pool generation, pretesting evaluation, and psychometric testing. Item pools were generated t

Discharge Readiness and Patient Experience in the Hospital-to-Home Transition After Digestive Surgery.

Background/Objectives: Complete postoperative recovery increasingly occurs after hospital discharge, particularly in the context of enhanced recovery after surgery (ERAS) programmes. However, patient-reported experience during the transition from hospital to home after digestive surgery remains poorly characterized. This study aimed to evaluate patient-reported experience and to explore its associations with perceived quality of recovery at discharge, surgical complexity, postoperative complications, and unplanned healthcare utilization. Methods: This study analyzed a prospective observational

Assessing artificial intelligence-generated patient discharge information for the emergency department: a pilot study.

BACKGROUND: Effective patient discharge information (PDI) in emergency departments (EDs) is vital and often more crucial than the diagnosis itself. Patients who are well informed at discharge tend to be more satisfied and experience better health outcomes. The combination of written and verbal instructions tends to improve patient recall. However, creating written discharge materials is both time-consuming and costly. With the emergence of generative artificial intelligence (AI) and large language models (LMMs), there is potential for the efficient production of patient discharge documents. Th

Can I Discharge This Adult Patient with Abnormal Vital Signs From the Emergency Department?

BACKGROUND: Vital signs are an essential component of the emergency department (ED) assessment. Vital sign abnormalities are associated with adverse events in the ED setting and may indicate a risk of poor outcomes after ED discharge. CLINICAL QUESTION: What is the risk of adverse events among adult patients with abnormal vital signs at the time of ED discharge? EVIDENCE REVIEW: Studies retrieved included 6 retrospective studies with adult patients discharged from the ED. These studies evaluated adverse outcomes in adult patients discharged from the ED with abnormal vital signs. Hypotension

Implantable Cardioverter-Defibrillator Discharge in a Patient with Dilated Cardiomyopathy: What Is the Mechanism?

The response to antitachycardia pacing can sometimes reveal clues about tachycardia mechanisms. This article discusses a case in which the diagnosis of typical atrioventricular nodal reciprocating tachycardia could be firmly established from the implantable cardioverter-defibrillator interrogation alone.

Diagnosis of gastrointestinal anastomotic dehiscence after hospital discharge: Impact on patient management and outcome.

BACKGROUND: Anastomotic leaks are inevitable complications of gastrointestinal surgery. Early hospital discharge protocols have increased concern regarding outpatient presentation with anastomotic leaks. METHODS: One hundred anastomotic leaks in 5,387 intestinal operations performed at a single institution from 2002 to 2007 were identified from a prospectively maintained database. Statistical analysis was conducted by the unpaired t test, Chi-square test, and analysis of variance. RESULTS: Overall anastomotic leak with a rate of 2.6% for colonic and 0.53% for small bowel anastomoses. Mean ti
