The effect of the Shorter Stays in Emergency Departments health target on the quality of ED discharge summaries.

OBJECTIVE: Time targets for ED stays are used as a policy instrument to reduce ED crowding. There is debate whether such policies are helpful or harmful, as focus on a process target may divert attention from clinical care. The objective of this study is to investigate whether the Shorter Stays in Emergency Departments target in New Zealand was associated with a change in the quality of ED discharge information provided to primary care providers. METHODS: The quality of discharge summaries was assessed retrospectively over time using chart review. Logistic regression was used to account for s

Enhancing clinical documentation with voice processing and large language models: a study on the LAOS system.

The growing volume of Electronic Health Records (EHRs) has enhanced patient care quality but significantly increased the cognitive workload on clinicians, particularly in ophthalmology where specialists handle 1.6 times more patient consultations than other specialties. This study introduces the "LLM-based Auxiliary Ophthalmic System (LAOS)," an integrated framework leveraging Large Language Models (LLMs) and audio processing to improve clinical documentation accuracy and efficiency. LAOS combines voice recognition with Retrieval-Augmented Generation (RAG) and Low-Rank Adaptation (LoRA) to con

A prospective study of acute inpatient gout diagnoses and management in a tertiary hospital: the determinants and outcome of a rheumatology consultation.

BACKGROUND: Despite acute gout frequently complicating hospital admissions, diagnosis and management are variable. Rheumatology input may improve patient outcomes. AIM: To examine acute episodes of inpatient gout in a tertiary hospital to determine (i) factors that may lead to rheumatology input being sought and (ii) the differences in outcomes when rheumatology input occurs. METHODS: Data collection occurred between February and October 2012 for inpatients in a tertiary Australian hospital. Data were prospectively collected for all rheumatology consultations with a diagnosis of gout. Subjec

The delivery of interim discharge summaries to general practitioners by the elderly.

Two hundred and four patients discharged from an acute geriatric unit were given an interim discharge summary for delivery to their General Practitioner (GP). All but three summaries were received by the practices. Information regarding mode and timing of receipt was available in 191 patients. The summaries were received in a median of 1.0 day in both those living alone and those living with family; 75% of summaries had been received within two days of discharge and 93% within four days. General practitioners collected the summaries from patients' homes in a high proportion of both groups. Int

Sometimes less is more: experience with endoscopic myringoplasty.

A retrospective review of the outcomes of patients who underwent endoscopic myringoplasties in our institution was conducted. The aim was to highlight our results with this procedure. The database of patient records was manually checked, and the patients who had undergone Endoscopic Myringoplasties were identified, and their demographics, admitting notes, operating notes, and discharge summaries were reviewed. Graft failure was considered if the patient had a perforation in the graft during the outpatient follow-up. The information was compiled, and basic statistics were derived. A total of 31

A population-based study of infection-related hospital mortality in patients with dermatomyositis/polymyositis.

OBJECTIVE: Dermatomyositis (DM) and polymyositis (PM) are debilitating inflammatory myopathies associated with significant mortality. We evaluated the relative contribution of infection to hospital mortality in a large population-based study of individuals with PM/DM. METHODS: Data derive from the 2007 to 2011 Healthcare Cost and Utilization Project National Inpatient Samples and include all hospital discharges that met a validated administrative definition of PM/DM. The primary outcome was hospital mortality. Variables for infections and comorbidities were generated from discharge diagnoses

Checking coding completeness by mining discharge summaries.

Incomplete coding is a known problem in hospital information systems. In order to detect non-coded secondary diseases we developed a text classification system which scans discharge summaries for drug names. Using a drug knowledge base in which drug names are linked to sets of ICD-10 codes, the system selects those documents in which a drug name occurs that is not justified by any ICD-10 code within the corresponding record in the patient database. Treatment episodes with missing codes for diabetes mellitus, Parkinson's disease, and asthma/COPD were subject to investigation in a large German u

The impact of discharge letter content on unplanned hospital readmissions within 30 and 90 days in older adults with chronic illness - a mixed methods study.

BACKGROUND: Care transitions are high-risk processes, especially for people with complex or chronic illness. Discharge letters are an opportunity to provide written information to improve patients' self-management after discharge. The aim of this study is to determine the impact of discharge letter content on unplanned hospital readmissions and self-rated quality of care transitions among patients 60 years of age or older with chronic illness. METHODS: The study had a convergent mixed methods design. Patients with chronic obstructive pulmonary disease or congestive heart failure were recruite

Extraction of Substance Use Information From Clinical Notes: Generative Pretrained Transformer-Based Investigation.

BACKGROUND: Understanding the multifaceted nature of health outcomes requires a comprehensive examination of the social, economic, and environmental determinants that shape individual well-being. Among these determinants, behavioral factors play a crucial role, particularly the consumption patterns of psychoactive substances, which have important implications on public health. The Global Burden of Disease Study shows a growing impact in disability-adjusted life years due to substance use. The successful identification of patients' substance use information equips clinical care teams to address

Prescribing and adherence to bone protection medications following hip fracture in the United Kingdom: results from the World Hip Trauma Evaluation (WHiTE) cohort study.

AIMS: Bone health assessment and the prescription of medication for secondary fracture prevention have become an integral part of the acute management of patients with hip fracture. However, there is little evidence regarding compliance with prescription guidelines and subsequent adherence to medication in this patient group. PATIENTS AND METHODS: The World Hip Trauma Evaluation (WHiTE) is a multicentre, prospective cohort of hip fracture patients in NHS hospitals in England and Wales. Patients aged 60 years and older who received operative treatment for a hip fracture were eligible for inclu

Indicators of missing Electronic Medical Record (EMR) discharge summaries: A retrospective study on Canadian data.

INTRODUCTION AND OBJECTIVES: Electronic medical records (EMRs), specifically the discharge summary (DS), can improve secondary use data availability and interprofessional communication. We aimed to describe the completeness of our EMRs by assessing the presence of a DS in the EMR. Additionally, we assessed for indicators of a missing DS. METHODS: A chart review was conducted on 3,011 non-obstetric adult inpatient charts in Calgary, Alberta. 893 charts were missing an electronic DS. A 10% sample was drawn to evaluate the presence of a paper DS. A Chi-square test, Fisher's test and logistic reg

Building a patient record system. New information technologies will support development of patient-focused care.

As part of its plan to build a regional integrated healthcare delivery network, Genesys Health System, Flint, MI, has been developing a computerized patient record (CPR). The CPR will give users throughout the system immediate access to diagnostic studies, treatment information, discharge summaries, operative reports, x-rays, and other patient information. Before considering technical aspects of the system, healthcare executives considering implementation of a CPR should examine business and clinical issues to determine what they want to accomplish with the CPR. The Genesys information system

Predicting postoperative acute respiratory failure in critical care using nursing notes and physiological signals.

Postoperative Acute Respiratory Failure (ARF) is a serious complication in critical care affecting patient morbidity and mortality. In this paper we investigate a novel approach to predicting ARF in critically ill patients. We study the use of two disparate sources of information – semi-structured text contained in nursing notes and investigative reports that are regularly recorded and the respiration rate, a physiological signal that is continuously monitored during a patient's ICU stay. Unlike previous works that retrospectively analyze complications, we exclude discharge summaries from our

Survey of medication history of patients with stroke after discharge from an acute hospital ward: a case series study.

BACKGROUND: Stroke is a leading cause of death, reducing disability-free life expectancy. After acute treatment, patients require rehabilitation to prevent recurrence. Continued use of medication is crucial for recurrence prevention and risk management, even after the transition from acute-care institutions to other medical institutions. Although "discharge summaries on medications" are shared between hospitals and community pharmacists, no reports have addressed medication continuity for patients with stroke transferred to other institutions after discharge. This study aimed to clarify medica

Artificial intelligence and deskilling in medicine.

Artificial intelligence is increasingly being used in medical practice to complete tasks that were previously completed by the physician, such as visit documentation, treatment plans and discharge summaries. As artificial intelligence becomes a routine part of medical care, physicians increasingly trust and rely on its clinical recommendations. However, there is concern that some physicians, especially those younger and less experienced, will become over-reliant on artificial intelligence. Over-reliance on it may reduce the quality of clinical reasoning and decision-making, negatively impact p

Treatment and outcome in 30 patients with posterior cerebral artery aneurysms.

OBJECT: The records of 30 patients with posterior cerebral artery (PCA) aneurysms treated during a 12-year period were reviewed to determine outcome and the risk of visual field deficit associated with PCA sacrifice. METHODS: Clinical data and treatment summaries for all patients were maintained in an electronic database. The Glasgow Outcome Scale (GOS) and modified Rankin Scale (mRS) scores were determined by an independent registrar. Visual field changes were determined by review of medical records. Twenty-eight patients were treated with open surgery, one of them after an attempt at detach

Delirium in hospital: an underreported event at discharge.

OBJECTIVE: Delirium, an important event in hospital, is associated with significant mortality and morbidity. Most patients with delirium recover fully; however, when left untreated, delirium may progress to stupor, coma, or death. Delirium is less likely to resolve completely in elderly patients in whom persistent cognitive deficits commonly occur. The extent to which this information is available to family doctors after discharge was investigated. METHOD: A total of 31 patients with delirium who were referred to consultation-liaison psychiatry were assessed using standardized measures. Medic

Quality of acute heart failure treatment in France: Data from REseau Nord-Alpin des Urgences (RENAU).

BACKGROUND: Although mortality due to acute heart failure has decreased, its prevalence in France is still high. The aim of this study was to examine the quality of acute heart failure treatment in French emergency departments (EDs) with reference to subsequently published European Society of Cardiology (ESC) recommendations. METHODS: The medical records of patients with acute pulmonary oedema (as a marker for acute heart failure) admitted to the EDs of 11 French hospitals in 2013 were reviewed retrospectively. RESULTS: A total of 834 patients were included (median [interquartile range] age
