Patient-Reported Experiences With Long-Term Lifestyle Self-Monitoring in Heart Disease: Mixed Methods Study.

BACKGROUND: Lifestyle behaviors strongly predict cardiovascular morbidity and mortality, emphasizing the need for strategies that support sustained lifestyle changes in patients with cardiac disease. Digital health solutions, including wearables, mobile apps, and chatbots, enable self-monitoring of lifestyle behaviors but often face challenges with engagement and usability. While self-monitoring systems can increase awareness and accountability, maintaining user engagement remains crucial for their effectiveness in promoting behavior change and long-term improvements. OBJECTIVE: This study ev

Multivariate models for compliance with phase 3 cardiac rehabilitation services in Johannesburg.

The efficacy of an exercise cardiac rehabilitation programme depends on an adequate compliance of its participants. A 50% dropout rate after 12 months has been reported by most rehabilitation programmes. Compliance with attendance and the exercise prescription are monitored daily at the Johannesburg Cardiac Rehabilitation Centre. This study evaluated the attendance of 711 patients admitted to the programme between June 1986 and March 1990 and looked at possible differences in attendance and intensity rates of compliers and dropouts prior to dropout. Multiple regression analyses were performed

Factors associated with early dropout from adolescent psychiatric outpatient treatment.

OBJECTIVE: To examine background factors, psychopathology, and psychosocial impairment among adolescents complying with or dropping out early from outpatient psychiatric treatment. METHOD: Family background, psychiatric history, and other data were collected prospectively on 143 male and 154 female outpatients aged 12 to 22 years. DSM-II-R psychiatric diagnoses were assessed at the end of treatment. RESULTS: Fifty-three adolescents (17.8%) attended 1 or 2 treatment appointments, and 33 of them (11.1% of 297) then dropped out; 50.5% of the total attended 3 to 13, and 31.6% attended 14 or more

Reasons for dropout from cardiac rehabilitation programs in women: A qualitative study.

BACKGROUND: Empirical evidence has shown that cardiac rehabilitation programs are effective in reducing morbidity and mortality, improving quality of life in patients with cardiovascular disease. Despite the benefits, women have a high cardiac rehabilitation dropout rate. Our aim was to explore women's perceptions about the reasons they faced for dropout from these programs. METHODS: Semi-structured interviews were conducted with women (n = 10) after dropping out from three different cardiac rehabilitation centers in Spain. In addition, a focus group and a semi-structured interview with cardi

Nocebo is the enemy, not placebo. A meta-analysis of reported side effects after placebo treatment in headaches.

The aim was to determine the magnitude of the nocebo (adverse effects following placebo administration) in clinical trials for primary headache disorders. We reviewed randomized, placebo-controlled studies for migraine, tension-type headache (TTH), and cluster headache treatments published between 1998 and 2009. The frequency of nocebo was estimated by the percentage of placebo-treated patients reporting at least one adverse side effect. The dropout frequency was estimated by the percentage of placebo-treated patients who discontinued the treatment due to intolerance. In studies of symptomatic

[Drop-out from a mental health centre after the first contact].

AIM: Many patients drop-out from treatment after a single contact; however, characteristics have only rarely been investigated. We aimed at estimating the frequency of early drop-out and identifying possible risk factors. METHODS: The study was performed at the Palestrina Mental Health Centre, located near Rome, Italy. The clinical charts of all first-time attenders who did not come back during a 7-year index period (2000-2006) were reviewed and information was collected on sociodemographic factors, psychiatric history, clinical presentation, and process of care. RESULTS: During the index pe

Client Perspectives of Premature Discontinuation in Couple Therapy.

Premature discontinuation, defined as unilateral termination before goal completion against therapist advice, affects approximately 36% of couples who begin therapy. While research often examines factors predicting premature discontinuation, few studies directly ask clients their reasons for stopping. This study explored these reasons by surveying 160 individuals (61% female, 38% male, 1% other; primarily aged 25-44; 92% heterosexual relationships; median income $50-60k) who prematurely discontinued couple therapy without discussing the decision with their therapist. The sample was predominant

Dropout from cognitive-behavioral therapy for eating disorders: A meta-analysis of randomized, controlled trials.

OBJECTIVE: Cognitive-behavioral therapy (CBT) is efficacious for a range of eating disorder presentations, yet premature dropout is one factor that might limit CBTs effectiveness. Improved understanding of dropout from CBT for eating disorders is important. This meta-analysis aimed to study dropout from CBT for eating disorders in randomized controlled trials (RCTs), by (a) identifying the types of dropout definitions applied, (b) providing estimates of dropout, (c) comparing dropout rates from CBT to non-CBT interventions for eating disorders, and (d) testing moderators of dropout. METHOD: R

Early enteral feeding, compared with parenteral, reduces postoperative septic complications. The results of a meta-analysis.

This two-part meta-analysis combined data from eight prospective randomized trials designed to compare the nutritional efficacy of early enteral (TEN) and parenteral (TPN) nutrition in high-risk surgical patients. The combined data gave sufficient patient numbers (TEN, n = 118; TPN, n = 112) to adequately address whether route of substrate delivery affected septic complication incidence. Phase I (dropouts excluded) meta-analysis confirmed data homogeneity across study sites, that TEN and TPN groups were comparable, and that significantly fewer TEN patients experienced septic complications (TEN

Learning the Lessons of Premature Discontinuation of Eating Disorder Therapy: Informing Guidelines to Improve Retention.

OBJECTIVE: Around one to two in five patients prematurely disengage from eating disorder therapy. This Forum paper suggests innovative strategies to increase retention and strategies for evaluating these with patients who have eating disorders. METHOD: We start by clarifying the definition of premature discontinuation and note one obstacle in the field is a lack of routine and uniform reporting across treatment studies and clinical practice. We then examine common reasons for discontinuation, suggest evidence-informed strategies to improve retention, and ways to evaluate the success of these

The Unsolved Problem of Attrition Rates on Randomized Clinical Trials for Cocaine Use Disorders: A Scoping Review.

BACKGROUND: Cocaine use disorder (CUD) is a significant and insufficiently studied public health issue, especially considering that the global prevalence of CUD is estimated to be higher than ever. There is still no consensus on effective treatments for CUD. Important barriers for research in the field include the high attrition levels observed in randomized controlled trials (RCTs) for CUD treatment and the lack of emphasis on methods to reduce attrition in CUD RCTs. METHODS: The goal of this study was to systematically review over 2 decades of CUD RCTs, with the objective of evaluating the

Attrition and discontinuation in amyotrophic lateral sclerosis clinical trials: a meta-analysis.

OBJECTIVES: Attrition due to adverse events and disease progression impacts the integrity and generalizability of clinical trials. The aim of this study is to provide evidence-based estimates of attrition for clinical trials in amyotrophic lateral sclerosis (ALS), and identify study-related predictors, through a comprehensive systematic review and meta-analysis. METHODS: We systematically reviewed the literature to identify all randomized, placebo-controlled clinical trials in ALS and determined the number of patients who discontinued the study per randomized arm. Subsequently, we meta-analyz

Patient-related factors associated with patient retention and non-completion in psychosocial treatment of borderline personality disorder: A systematic review.

The potential efficacy of psychosocial interventions in the treatment of borderline personality disorder (BPD) is impacted by significant treatment non-completion (TNC), with meta-analytic studies reporting rates of attrition of between 25% and 28%. Increasing patient retention could facilitate outcomes and improve resource utilization, given limited healthcare services. A systematic search of PsycINFO, CINAHL, EMBASE, CENTRAL, and Web of Science Core Collection identified 33 articles that met the criteria for inclusion. Although substantial heterogeneity in terms of methodology and quality of

Intervention Program Dropout Among Perpetrators of Intimate Partner Violence: A Meta-Analysis of Correlated Variables.

Dropout in perpetrator intervention programs (PIPs) is extensively documented in the literature, yet findings across various studies exhibit global inconsistency. This meta-analysis aimed to identify dropout rates among individuals attending PIPs and examine sociodemographic, violence-related, intrapersonal, interpersonal, and external variables related to dropout. A search was conducted across six databases, encompassing studies published between 2010 and 2022 in English, Spanish, or Portuguese. Thirty manuscripts, comprising data from 31 independent samples, were included. Dropout rates vari

Psychological Assessment in Anorexia Nervosa: A Single Case Study on Predictive Factors of Dropout.

Anorexia nervosa (AN) has the highest mortality rate among psychiatric disorders. Adult AN patients have a chronic history of treatment dropout due to denial of their psychological and physical disease states, which may be connected to defense mechanisms. We developed an assessment protocol to evaluate the psychological functioning of patients undergoing a psychodynamic approach for eating disorders (PAED), aimed at identifying the psychological factors associated with intervention success or dropout. We analyzed the case of an adult patient who quit treatment at the start and discussed her ps

A systematic review of dropout from treatment in outpatients with anorexia nervosa.

OBJECTIVE: Dropout from treatment has serious implications for patients, clinicians, and researchers. The aim of this study was to examine rates of dropout from outpatient treatment for anorexia nervosa (AN) and critically examine the various definitions of dropout used. METHOD: A systematic review was conducted, including pharmacological and psychological interventions. All articles in PubMed, Web of Science, and the Cochrane Library were considered, and screened against a priori inclusion/exclusion criteria. Relationships between treatment outcome and dropout rate were examined across studi

Searching for the reasons for drop-out from peritoneal dialysis: a nationwide survey in Japan.

PURPOSE: We wanted to clarify, in a large population, the reasons that peritoneal dialysis (PD) was interrupted and to propose appropriate strategies that might prolong the use of PD as a renal replacement therapy. PATIENTS AND METHODS: We enrolled 5 391 patients from 439 centers into the study. Each center used a worksheet to report details about patients who either transferred to hemodialysis (HD) or died on continuous ambulatory peritoneal dialysis (CAPD) over a 6-month period from 1 April 2000, to 30 September 2000. RESULTS: We collected 252 drop-out cases from 141 centers. Of the 252 ca

Stage of change as a predictor of response to psychotherapy for bulimia nervosa.

OBJECTIVE: This study was designed to test the hypotheses that stage of change is a useful predictor of dropout and that it is related to treatment outcome in patients receiving brief psychotherapy for bulimia nervosa. METHOD: One hundred ten patients with bulimia nervosa were randomly assigned treatment with cognitive-behavioral therapy (CBT) or interpersonal therapy (IPT). On initial screening, patients were classified as being in the stage of precontemplation, contemplation, or preparation. RESULTS: Initial stage of change was not related to dropout either before or following randomizatio
