OBJECTIVE: Sonography has become a popular technique for the assessment of musculoskeletal disorders. Patient positioning is crucial to a thorough and accurate assessment of rotator cuff tendons. Two positions, the Crass and modified Crass, have been routinely used in the research and clinical settings to examine the supraspinatus tendon. Our study was a prospective trial to determine whether the Crass or the modified Crass position affords the most accurate measure of supraspinatus tears when compared with surgical findings.
SUBJECTS AND METHODS: Twenty-one patients with full-thickness supra

INTRODUCTION: Integrating magnetic resonance imaging (MRI) and stereotactic radiosurgery (SRS) has revolutionised cancer treatment, enhancing precision and improving patient outcomes. MRI/CT image fusion improves the visualisation of targets and surrounding tissues, supporting accurate radiotherapy planning. This study evaluates the impact of multidisciplinary cooperation between radiology and radiotherapy departments on MRI/CT image fusion, incorporating a thermomouldable cervical cushion during CT and MRI planning to optimise patient positioning.
METHODS: This retrospective study analysed 2

BACKGROUND: Precise patient positioning is crucial for successful radiotherapy, ensuring accurate delivery of radiation to tumors while minimizing exposure to healthy tissues. Positional errors can significantly impact treatment efficacy and increase side effects. This study evaluates the effectiveness of daily cone beam computed tomography (CBCT) imaging in the Halcyon system for detecting and correcting patient misalignments across various cancer types and treatment sites.
METHODS: A retrospective analysis was conducted on 411 patients treated with the Varian Halcyon linear accelerator from

Shoulder arthroscopy is one of the most common orthopaedic procedures. Since its introduction in the 1970s, significant advances have been made in surgical techniques and instrumentation. Shoulder arthroscopy is often performed in either the beach-chair or lateral decubitus position, and each positioning technique has its own benefits. In this work, we will describe the history of lateral decubitus positioning for shoulder arthroscopy. We outline a detailed guide for safe and consistent lateral decubitus positioning, and key steps of lateral decubitus positioning will be emphasized. Pearls, pi

PURPOSE: To evaluate the risk of injuring the axillary nerve during an inferior glenohumeral ligament (IGHL) plication and finding out whether shoulder position (either beach chair position or lateral decubitus position) has any effect in this risk.
METHODS: The axillary nerve (AN) was identified through a 3-cm posterior incision in 12 cadaveric shoulders. Under arthroscopic visualization, a curved indirect suture-passing device was placed through the posterior and anterior bands of the IGHL. The distances between the device and the AN were measured with the shoulder specimen placed at simula

PURPOSE: Daily set up of patients with prostate cancer using orthogonal kV/kV imaging and weekly set up control require 1h to 1h30 of off line revision by a radio-oncologist per day and per accelerator. The aim of this study was to evaluate the possibility to delegate this control to radiation therapists.
MATERIAL AND METHODS: The files of 33 patients (including 13 with prostate cancer) treated from November 2010 to February 2011 on a Varian™ Clinac IX accelerator with an OBI™ system were evaluated. Radiation therapists made the daily kV/kV imaging. Radiation therapists made the online contro

BACKGROUND AND PURPOSE: Intra-fraction patient motion is incompletely understood and the optimum amount of support or immobilization during stereotactic body radiotherapy (SBRT) is unclear. Rigid immobilization is often advocated, but motion still occurs. In contrast, we deliver the vast majority of SBRT using simple supporting devices, simultaneously emphasizing comfort, frequent position checks and progressive reduction in treatment times. We report spine stability during lung SBRT.
MATERIALS AND METHODS: Patients lie on a thin mattress with arms supported above their head and below-knee su

PURPOSE: In this work, a novel stochastic framework for patient positioning based on linac-mounted CB projections is introduced. Based on this formulation, the most probable shifts and rotations of the patient are estimated, incorporating interfractional deformations of patient anatomy and other uncertainties associated with patient setup.
METHODS: The target position is assumed to be defined by and is stochastically determined from positions of various features such as anatomical landmarks or markers in CB projections, i.e., radiographs acquired with a CB-CT system. The patient positioning p

BACKGROUND: Neuromuscular scoliosis (NMS) can displace the diaphragm into the thorax, reducing lung volume and impairing respiration. As an alternative to posterior spinal fusion, growth-friendly surgery (GFS) allows thoracic growth while correcting deformity. We evaluated associations of GFS with thoracic dimensions and diaphragm positioning, hypothesizing that GFS would alleviate respiratory constraints in pediatric NMS.
METHODS: We reviewed data from 45 patients who underwent GFS for NMS at a mean age of 8 years, from 2003 to 2022. Radiographic measures were lung volume, space available fo

BACKGROUND: The novel upright walk-through PET (WT-PET) scanner enhances patient throughput compared to the conventional cylindrical PET systems but introduces unique challenges related to patient motion. This study evaluates the rigid body motion of the head, shoulders, chest, and abdomen of patients standing in a WT-PET mock-up scanner, focusing on ergonomic features, including a headrest and hand supports, designed to minimize motion during upright imaging. To contextualize these findings, a patient study using a conventional PET scanner was conducted, along with a control healthy volunteer

The authors report a case of nontraumatic orbital subperiosteal hematoma after general anesthesia with patient in the prone position. The patient, who was on aspirin, clopidogrel, and subcutaneous heparin, presented immediately after sacral ulcer debridement with acute bilateral vision loss and periorbital edema. While the OD improved with conservative management, the OS continued to have 20/200 vision, decreased color vision, afferent pupillary defect, and extraocular movement limitation after lateral canthotomy and cantholysis. Imaging revealed bilateral subperiosteal hematomas in the superi

Cervical spine surgery in patients with myelopathy poses a substantial anesthetic challenge, primarily due to the risk of secondary spinal cord injury (SCI). Traditionally, concerns have centered around cervical movements during intubation. However, limited evidence supports a direct link between intubation and SCI, so anesthesiologists must consider other factors, including patient positioning, spinal perfusion pressure, and direct surgical complications. In this context, multimodal intraoperative neurophysiological monitoring, including somatosensory (SSEPs) and motor evoked potentials (MEPs

During spinal surgery using a horseshoe headrest with the patient in the prone position, the possibility of central retinal artery occlusion (CRAO) increases, and its cause can be attributed primarily to excessive extraocular pressure, a very rare complication. This report describes a case of CRAO, occurring in an adult, after cervical syringomyelia surgery in which a horseshoe headrest was used.

Digital subtraction angiography (DSA) remains an important reference modality for evaluating chronic limb-threatening ischemia (CLTI), with left transradial access (TRA) increasingly favored for its lower complication rates and patient comfort. Radiation safety for operators is paramount, yet the impact of patient positioning on scatter radiation during lower limb diagnostic catheter angiography (CA) is understudied. This single-center randomized controlled trial evaluated whether head-first (HF) vs. feet-first (FF) supine patient orientation affects operator radiation exposure during lower ex

Background: Duplex ultrasound assessment with the patient standing is standard for diagnosing chronic venous insufficiency (CVI). However, assessments are often performed with patients in tilted or supine positions due to age or physical limitations. This study aimed to evaluate the influence of the patient's positioning on venous diameters and venous valve function. Patients and Methods: Monocentric, prospective cohort study in 30 patients with CVI of the great saphenous vein (GSV). 18 contralateral limbs without GSV insufficiency were defined as the control group. With duplex ultrasound, we

The purpose of this study was to determine the residual patient positioning error of ocular proton treatment based on daily setup X-ray images and tantalum clips. The residual day-to-day patient positioning accuracy in a cohort of 60 patients and 240 fractions was determined in two steps. The 3D tantalum clips' positions in the orthogonal X-ray images just prior to aperture placement were registered with 3D clips' positions derived from the reference data from treatment planning. The aperture, placed before irradiation, reduced the field of view in axial images blocking clips partially or comp
