Implementing an Interprofessional Simulation Program for Paediatric Patient Safety.

BACKGROUND: Simulation-based medical education is a widely recognised tool to improve patient safety culture and outcomes. Many preventable clinic errors are specific to the unit where they occur. We planned and conducted an interprofessional in situ simulation program aligned with unit-specific errors and report its design, implementation and evaluation. APPROACH: This prospective, survey-based study was conducted at a paediatric tertiary care centre on a medical-surgical inpatient ward. Incident and accident reports were reviewed; the most frequent and/or severe events were identified. Eigh

Cybersecurity in Healthcare: Ensuring Patient Safety and Data Privacy.

OBJECTIVE: Healthcare data breaches have increased in both frequency and severity, yet limited empirical evidence exists on the factors associated with large-scale breach events. To address this gap, this study analyzed breaches reported to the US Department of Health and Human Services Office of Civil Rights between 2010 and 2025 to determine predictors of large breaches. METHODS: A total of 7327 breach reports from the Office for Civil Rights were analyzed. Logistic regression assessed predictors of high‑severity incidents (≥ 100,000 individuals affected). Differences in breach size between

Health equity is now a National Patient Safety Goal.

Governmental data generated during the COVID 19 pandemic from 2019 to 2021 demonstrated that the virus disproportionately impacted communities of color. Specifically, Black, Hispanic, and Asian people were found to have substantially higher rates of infection, hospitalization, and death compared with White people. The extent of these disparities drew attention to other disparate outcomes including maternal mortality and morbidity, cancer outcomes, and overall life expectancy. Various professional organizations, including the Institute for Healthcare Improvement, began to add health equity to t

Using perioperative patient safety guidelines to drive systems change.

A European consensus study in this issue addresses perioperative patient safety recommendations by synthesising multiple patient safety guidelines covering the entire patient journey to provide a single point of reference. In this editorial we address what is considered as evidence and seek inclusion of broader ways of knowing and understanding the complexity of safety in healthcare. We suggest prioritising feasibility could limit improvements to specific clinical practices rather than systems-wide change. Safety is more than a collection of isolated recommendations. Safety interventions shoul

Aspirin for Venous Thromboembolism Prophylaxis: Respecting the Patient Safety.

Aspirin is increasingly being used for venous thromboembolism (VTE) prophylaxis in patients undergoing total joint arthroplasty (TJA). It is considered a safer and more cost-effective alternative to other anticoagulants, with the added benefit of not requiring blood-level titration. Studies have shown that aspirin is effective in preventing VTE, with a lower risk of major bleeding and reduced mortality compared to other options. Additionally, aspirin provides indirect benefits such as a reduced risk of periprosthetic joint infections (PJIs), heterotopic ossification (HO), and joint stiffness,

Improved Patient Safety and Quality Outcomes With Amiodarone Infusions.

BACKGROUND: Amiodarone is a common intravenous medication and a known irritant to the vessel wall when administered peripherally. LOCAL PROBLEM: Nurses identified an increase in phlebitis associated with peripheral amiodarone leading to multiple catheter replacements and interruptions in drug therapy. Central venous access is recommended by the manufacturer but not practical for a short-term infusion based on the risk to the patient, time, and cost. METHODS: A 4-phased approach was used to identify a more suitable peripheral intravenous catheter. INTERVENTIONS: A collaborative effort betwee

A System-Wide Hospital Child Maltreatment Patient Safety Program.

BACKGROUND AND OBJECTIVES: Hospital-wide patient safety programs have been used to ensure appropriate provision of care. Similar approaches have not been widely applied to child maltreatment. In this study, we describe a hospital-system child maltreatment safety program by characterizing the frequency of patients needing further intervention, associations between the age of patient and location of care and need for further intervention, and patients who require immediate intervention. METHODS: For all staff concerns for child maltreatment, a social worker completed a patient at risk (PAR) for

Technical Issues and Patient Safety in Nonintubated Thoracic Anesthesia.

Nonintubated thoracic surgery (NITS) has a good safety record in experienced hands, but has pitfalls for beginners. The main aim of NITS is to keep the patient under spontaneous respiration, avoiding adverse effects, such as hypoxemia, hypercapnia, panic attacks, and finally conversion to general anesthesia. In this paper, the safety aspects of anesthesia for NITS is discussed based on data from the literature and personnel clinical experiences.

Pseudothrombocytopenia by ethylenediaminetetraacetic acid can jeopardize patient safety - report.

Pseudothrombocytopenia by ethylenediaminetetraacetic acid (EDTA) is an infrequent phenomenon of in vitro platelet agglutination due to the presence of antiplatelet autoantibodies. It has no clinical significance, but misdiagnosis may lead to clinical or therapeutic decision-making. In this study we report a case of an 8-year-old boy with no history of platelet disorder presenting a low platelet count and a peripheral blood smear showing clumping of platelets by EDTA. The initial diagnosis hypothesis was of an idiopathic thrombocytopenic purpura, and an unnecessary bone marrow aspirate was made
