Pectoralis Muscle Transposition in Association with the Ravitch Procedure in the Management of Severe Pectus Excavatum.

UNLABELLED: Pectus excavatum (PE) is the most common congenital chest wall deformity. PE is sometimes associated with cardiorespiratory impairment, but is often associated with psychological distress, especially for patients in their teenage years. Surgical repair of pectus deformities has been shown to improve both physical limitations and psychosocial well-being in children. The most common surgical approaches for PE treatment are the modified Ravitch technique and the minimally invasive Nuss technique. A technical modification of the Ravitch procedure, which includes bilateral mobilization

Bipolar transfer of the pectoralis major muscle for restoration of elbow flexion in 29 cases.

BACKGROUND: This study evaluated the functional outcomes of bipolar pedicled pectoralis major (PM) transfer to restore elbow flexion. METHODS: We retrospectively reviewed 29 transfers in 28 patients with a mean age of 31.2 years (range, 5-65 years). The loss of elbow flexion was due to brachial plexus palsy in 24 patients, elbow flexors necrosis in 4, and poliomyelitis in 1. The entire PM muscle was mobilized and fixed proximally to the coracoid process. Intraoperative positioning and postoperative immobilization of the shoulder and the elbow flexed at 60° and 120°, respectively, allowed dire

Unusual innervation pattern of pectoralis minor muscle-anatomical description and clinical implications.

The highlight of the current case study is unique branching pattern of the medial pectoral nerve in relation to the pectoralis minor muscle. In addition, the third thoracic spinal nerve also was observed to penetrate the muscle. The lateral pectoral nerve did not innervate the muscle. The pectoralis minor was observed to be pierced extensively by multiple branches from medial pectoral nerve. Therefore, this study was planned to report this unusual presentation. Such an extensive neural arrangement in relation to pectoralis minor is rather unusual and to the best of our knowledge has been not r

Pectoralis major-rectus abdominis bipedicled muscle flap in the treatment of poststernotomy mediastinitis.

OBJECTIVES: Although the incidence of infected sternotomy wounds after median sternotomy for cardiovascular surgery is relatively low (0.5% to 5%), it is associated with significant morbidity and a long period of treatment. Today, muscle flaps, such as the pectoralis major or the rectus abdominis, are widely accepted as a mainstay of reconstructive options. Each method carries unavoidable limitations and setbacks of its own. To overcome the disadvantages of the pectoralis muscle and rectus abdominis muscle flaps, we designed and performed a pectoralis major-rectus abdominis muscle bipedicled f

Results of transfer of the pectoralis major tendon to treat paralysis of the serratus anterior muscle.

BACKGROUND: Paralysis of the serratus anterior muscle can be functionally disabling. As a result of the scapular winging associated with such paralysis, the scapula does not remain apposed to the thorax when the upper extremity is elevated forward at the shoulder. This produces functional disability associated with pain and loss of a stable base for movement of the upper extremity. METHODS: We reviewed the results of transfer of the pectoralis major tendon with the addition of a fascial graft in sixteen patients who had paralysis of the serratus anterior. The average age of the patients at th

Anatomic basis for the construction of a musculotendinous flap derived from the pectoralis major muscle.

The vascularization, innervation and termination of the clavicular portion of the pectoralis major muscle were studied by dissection of 7 adult subjects and 2 fetuses. The vascular and nervous autonomy of the clavicular portion was confirmed. Blood-supply was effected by 2 arteries and innervation by an independent nerve. The exact distribution of the fibers composing the pectoralis major and their complex insertion are described. It is concluded that the clavicular portion can be employed as an active muscle transplant to the head of the humerus when the short rotator muscles of the shoulder

Pectoralis Major Muscle Flap With Fat Grafting in Oncoplastic Breast-Conserving Surgery.

Oncoplastic breast-conserving surgery integrates oncological resection with plastic surgery to optimize cosmetic outcomes. We present a novel approach that combines a pectoralis major muscle flap with fat grafting, referred to as the PMFG procedure, for the reconstruction of upper outer quadrant defects. A 52-year-old woman with breast cancer underwent partial mastectomy through a lateral incision, which resulted in a 70 mL defect. Using the same incision, the pectoralis major muscle flap was elevated over the thoracoacromial vessels while preserving the internal mammary perforators along the

Chronic Pectoralis Major Muscle Belly Repair and Reconstruction With Achilles Allograft Augmentation.

CASE: A 60-year-old man presented with a chronic right pectoralis major muscle belly tear of the lower sternal head that was missed and previously treated with tendon repair of the sternal and clavicular heads. The tear was repaired and reconstructed with an Achilles allograft. At the 1.5-year follow-up, the patient had excellent outcome scores on the Short Form Survey-36 and American Shoulder and Elbow Surgeons Scores, cosmesis, and strength. CONCLUSIONS: To our knowledge, this is the first report of a chronic pectoralis muscle belly tear reconstructed with an Achilles allograft, an uncommon

The Association Between Preoperative Pectoralis Muscle Quantity and Outcomes After Cardiac Transplantation.

BACKGROUND: Sarcopenia is underappreciated in advanced heart failure and is not routinely assessed. In patients receiving a left ventricular assist device, preoperative sarcopenia, defined by using computed-tomography (CT)-derived pectoralis muscle-area index (muscle area indexed to body-surface area), is an independent predictor of postoperative mortality. The association between preoperative sarcopenia and outcomes after heart transplant (HT) is unknown. OBJECTIVES: The primary aim of this study was to determine whether preoperative sarcopenia, diagnosed using the pectoralis muscle-area ind

Clinical significance of pectoralis muscle strength in elderly patients with idiopathic pulmonary fibrosis.

INTRODUCTION: Investigations of muscle dysfunction in patients with idiopathic pulmonary fibrosis (IPF) are limited to peripheral muscles. However, decreased thoracic muscle mass is known and deterioration of chest wall muscle strength is not clear. OBJECTIVE: The aims of the present study were to evaluate pectoralis muscle strength located on the chest wall and to investigate the relationship of spirometric measurements and respiratory muscle strength with pectoralis muscle strength. METHODS: Elderly patient with IPF (mean disease duration 7.47±7.04 years) and the age-and sex-matched health

Pectoralis Major Muscle Transfer With the Sternal Head and Hamstring Autograft for Scapular Winging.

Medial scapular winging is often due to dysfunction of the serratus anterior muscle as a result of injury to the long thoracic nerve. Impairment of the serratus anterior muscle may cause uncoordinated scapulohumeral rhythm during shoulder elevation and subsequent subscapular or shoulder pain, subacromial impingement, and glenohumeral joint instability. Although long thoracic nerve injury typically resolves in 12 to 18 months after a physical therapy regimen, surgical intervention is indicated in patients who fail conservative management. Both direct and indirect pectoralis major tendon transfe

Ultrasound of the pectoralis major muscle after reverse shoulder arthroplasty: a case report.

Only a few reports exist in the literature for sonographic assessment of the pectoralis major muscle. Presented is a case of pectoralis major muscle atrophy as a cause of persistent internal rotation weakness diagnosed via ultrasound in a patient with multiple previous surgeries and contraindication to magnetic resonance imaging because of a shoulder implant. This patient's physical examination suggested an abnormal contour of the pectoralis major muscle on contraction, so he was referred for diagnostic ultrasound. The ultrasound was key to guiding the management of this patient because surgic

[Treatment of recurrent anterior dislocation of the shoulder with pectoralis minor muscle-bone flap].

We treated 10 patients with recurrent anterior dislocation of the shoulder by transplantation of pectoralis minor muscle-bone flap transplanted to the upper part of the humerus. All patients were followed-up for an average 31 months and no recurrent was found. We consider that the muscle beily exerts a protective effect like a barrier on the weak anterio inferior region of the shoulder and increases the force for extending the shoulder and lifting the arm. The operation is based on biomechanics. Meanwhile, suturing the laxative joint capsule and repairing the weak anteroinfeior region of the s

Use of pectoralis major muscle flaps to repair defects of anterior chest wall.

We closed defects of the anterior chest wall in 6 patients, using either unilateral or bilateral pectoralis major muscle flap transpositions. In 4 of these patients the defect was stabilized with autogenous rib grafts, and none of these had a flail chest. All of the transposed muscle flaps remained viable and innervated. The deformities and the functional disturbances resulting from the transpositions were minimal.

Integrated assessment of computed tomography density in pectoralis and erector spinae muscles as a prognostic biomarker for coronavirus disease 2019.

UNLABELLED: BACKGROUND & AIMS: Muscle quantification using chest computed tomography (CT) is a useful prognostic biomarker for coronavirus disease 2019 (COVID-19). However, no studies have evaluated the clinical course through comprehensive assessment of the pectoralis and erector spinae muscles. Therefore, we compared the impact of the areas and densities of these muscles on COVID-19 infection outcome. METHODS: This multicenter retrospective cohort study was conducted by the COVID-19 Task Force. A total of 1410 patients with COVID-19 were included, and data on the area and density of the pec

Impact of pectoralis major muscle mass decrease after lobectomy on the prognosis of lung cancer.

BACKGROUND: Low preoperative skeletal muscle mass is a negative prognostic factor for non-small cell lung cancer. However, the clinical significance of postsurgical skeletal muscle loss remains unclear. We investigated the impact of a postoperative decrease in pectoralis major muscle mass on long-term outcomes. METHODS: A retrospective evaluation was conducted on 460 patients with pathological stage I-II non-small cell lung cancer who underwent lobectomy. Patients were categorized into two groups based on whether they did or did not show a decrease in pectoralis major muscle mass 12 months po
