Anesthesia in Pediatric Radiotherapy: A Systematic Literature Review by the SIOP Europe Working Group on Pediatric Anesthesia in Radiation Therapy.

Radiotherapy (RT) is essential in pediatric cancer treatment and often requires complete immobility. In younger or noncompliant children, this is typically achieved through sedation or general anesthesia (GA), which raises concerns about acute complications and potential long-term neurodevelopmental effects. Despite widespread use, standardized anesthesia protocols for pediatric RT are lacking. To support the development of practice recommendations, the SIOPE Working Group on Pediatric Anesthesia in Radiation Therapy conducted a systematic literature review. Studies from Medline and Embase wer

[Pediatric Anesthesia - Anesthesia for Congenital Heart Defects - Anesthesiological Approaches for Diagnostic and Surgical Procedures].

Congenital heart disease (CHD) is the most common congenital malformation, affecting approximately 1 in 100 newborns. Due to advances in clinical management of CHD, the population of patients with congenital heart disease is growing. Gaining knowledge and expertise in the perioperative management of patients with CHD is hence becoming increasingly important not only for specialized, but for all anesthetists. The perioperative management of patients with CHD varies depending on the anatomy and pathophysiology of the specific defect and the planned intervention. A pathophysiological classificatio

Spinal anesthesia for pediatric urological surgery: Reducing the theoretic neurotoxic effects of general anesthesia.

BACKGROUND: Spinal anesthesia (SA) is an effective technique that has been used in children for years. With growing concern with regard to the risks of general anesthesia (GA), we developed a SA program to provide an alternative option. We present our initial experience with this program. OBJECTIVE: To implement a SA program at a large tertiary care pediatric center and assess the safety and efficacy of the technique as an alternative to GA for urologic surgery. STUDY DESIGN/METHODS: We prospectively collected data on all children undergoing SA at our institution. We recorded demographics, p

Propofol: A Medication That Changed Pediatric Anesthesia Practice.

The introduction of propofol changed our understanding of pediatric anesthesia pharmacology, research approaches for intravenous drug study, and led to infusion pump development for the maintenance phase of anesthesia. The story of propofol development provides a blueprint for other intravenous drug development. The introduction of the target concentration strategy based on pharmacokinetic-pharmacodynamic and covariate understanding is central to total intravenous anesthesia techniques and is best exemplified by propofol. While use of the EEG to gauge depth of anesthesia preceded the propofol

Pediatric Anesthesia, Psychology, and Interventions: A Narrative Review.

The maintenance of pediatric perianesthetic mental health is more specific compared with that in adults, involving anesthesiology, pharmacology, psychology, nursing, pediatrics, and other disciplines. Although anesthesia can effectively prevent children from recalling the events of the surgical procedure, about 65-80% of children suffer from great anxiety and discomfort during surgical preparation, especially during anesthesia induction. The most obvious consequence of anxiety during anesthesia induction is that children are prone to extreme discomfort and mania during induction, and postopera

Pediatric Anesthesia Outside the Operating Room: Case Management.

Anesthesiology teams care for children in diverse locations, including diagnostic and interventional radiology, gastroenterology and pulmonary endoscopy suites, radiation oncology units, and cardiac catheterization laboratories. To provide safe, high-quality care, anesthesiologists working in these environments must understand the unique environmental and perioperative considerations and risks involved with each remote location and patient population. Once these variables are addressed, anesthesia and procedural teams can coordinate to ensure that patients and families receive the same high-qu

Technique for pediatric corneal crosslinking under general anesthesia.

Corneal cross-linking (CXL) is typically performed under topical anesthesia, which may be unsuitable in children and individuals with severe anxiety, poor cooperation, or neurodevelopmental issues. We describe a technique of CXL under general anesthesia that uses sevoflurane and propofol with laryngeal airway or nasal oxygen cannulas, and forceps or microsponges to stabilize the eye position intraoperatively. We also developed a simple and time-saving technique of bilateral simultaneous CXL, whereby the second eye can undergo CXL concurrently with the first in a staggered fashion. Using this t

Significant factors influencing inadvertent hypothermia in pediatric anesthesia.

Thermal imbalances are very common during surgery. Hypothermia exposes the patient to post-operative shivering, cardiac dysfunction, coagulopathy, bleeding, wound infection, delayed anesthesia recovery, prolonged hospital stay and increased hospitalization cost. There are many factors contributing to intraoperative hypothermia. This is a prospective cohort study conducted through observation and measurement of pediatric surgical patients' temperature. Convenience sampling methods were used in this study. Initial skin temperature and core temperature at 10 min, 30 min,1 h, 2 h, 3 h, 4 h, 5 h, 6

[Dose requirement for caudal anesthesia in pediatric patient].

A series of 305 infants and children between 1 day to 14 years of age were operated upon using caudal anesthesia. We used 1% mepivacaine 1.2 ml.kg-1 to obtain a level of anesthesia above T10, 1.0 ml.kg-1 above L1, and 0.8 ml.kg-1 above S. Complete failure occurred in 4.3% of patients. We investigated the blood concentration of mepivacaine in 17 patients, and it ranged from 0.5 microgram.ml-1 to 5.0 micrograms.ml-1. In a two month old infant, weighing 6.7 kg, apnea and bradycardia occurred. This was managed by tracheal intubation and controlled ventilation. But there were no other severe compli

Regional anesthesia for pediatric cardiac surgery: a review.

BACKGROUND: Effective pain management in pediatric cardiac surgery is essential for optimizing postoperative outcomes and promoting faster recovery. While intravenous analgesia remains a standard approach, regional anesthesia (RA) techniques have gained attention in this population due to their analgesic efficacy, reduced dependence on systemic opioids, and enhanced hemodynamic stability. MAIN BODY: This article provides an overview of current evidence of RA techniques for pediatric cardiac surgery. We discuss the role of RA in pediatric pain management, outlining various techniques, such as

Pediatric Dental Surgery Under General Anesthesia: Uncooperative Children.

Dental treatment of young pediatric patients can be confounded by lack of cooperation for dental rehabilitation procedures and even examination and/or radiographs. With the recent US Food and Drug Administration warning applied to many anesthetic/sedative agents for children less than 3 years old, a retrospective review of general anesthesia (GA) cases from 1 private pediatric dental practice was studied for age, gender, body mass index, anesthetic duration, airway management used, extent of dental surgical treatment, recovery time, and cardiac/pulmonary complications. For the 2016 calendar ye

[20 years' experience using anesthesia in pediatric bronchology].

The authors inform about their experience with anesthesia gained in the last 20 years with 50,000 bronchological examinations in children. Different anesthesia methods are discussed. They conclude that local anesthesia in adults and in adolescents is a possible procedure using fiberscope. In infants and children the most preferred anesthesia is halothane-nitrous oxide-oxygen-anesthesia with relaxation by succinylcholine.

Control of intracranial pressure during pediatric neurosurgery anesthesia.

Some of the important points of view of anesthesia in pediatric neurosurgery are: adequate alveolar exchange of oxygen and carbondioxide by means of artificial ventilation; stable cardiovascular function; fluid balance; monitoring, and attempts to keep intracranial pressure low. This last point is achieved by various anesthesiological procedures, e.g., hyperventilation, positive-negative pressure ventilation, rapid induction of anesthesia and choice of suitable anesthetics. Enflurane (Ethrane) increases intracranial pressure less than halothane. The new intravenous anesthetic etomidate lowers

[Anesthesia for a pediatric patient with Coffin-Siris syndrome].

A 3-year-old boy with Coffin-Siris syndrome was scheduled to undergo diagnostic laparoscopy, inguinal herniorrhaphy and orchiopexy at an ambulatory setting and same-day admission. Following anesthesia induction with inhalational sevoflurane, upper airway obstruction and hypoxemia developed. Hypoxemia was resolved immediately by manual positive pressure ventilation, although the stomach became bulged. Operation was finished uneventfully. However, he had massive bronchial secretion during anesthesia. He was admitted as planned and discharged on postoperative day 2. Since patients with Coffin-Sir

The use of the laryngeal mask airway in pediatric anesthesia.

Laryngeal mask airway (LMA) insertion was tried in 120 pediatric cases, from 2 months to 12 years of age. Initial indications for LMA were the same as for a face mask, except for two additional conditions; anticipation of difficulty with intubation and difficulty in management by a face mask. Size 2 LMA was used in the vast majority of cases. The insertion was successful on the first trial in 108 cases. More than one trial was necessary in 9 cases but only 3 cases required more than 3 trials. Insertion could not be completed in 3 cases. The relationship between the depth of LMA at the front te
