Medical and surgical management of pelvic floor disorders affecting defecation.

Pelvic floor disorders that affect stool evacuation include structural (for example, rectocele) and functional disorders (for example, dyssynergic defecation (DD)). Meticulous history, digital rectal examination (DRE), and physiological tests such as anorectal manometry, colonic transit study, balloon expulsion, and imaging studies such as anal ultrasound, defecography, and static and dynamic magnetic resonance imaging (MRI) can facilitate an objective diagnosis and optimal treatment. Management consists of education and counseling regarding bowel function, diet, laxatives, most importantly be

Organising a clinical service for patients with pelvic floor disorders.

The evolution of the multidisciplinary approach to the management of chronic conditions is a reflection of how medicine has evolved from a singular to a plural effort recognising the complex causations and consequences of such disorders. This thinking should not be confined to tertiary centres alone and should be adapted where local expertise is available. Such an approach is especially important in pelvic floor disorders, where the correlation between structure and function is not always straightforward. There is a need to avoid over-investigation by accurate clinical assessment allied to tai

[The role of medical imaging in the exploration of pelvic floor disorders].

Colpocystodefecography (CCD) and dynamic MRI with defecography (MRId) permit an alternation between filling and emptying the hollow organs and the maximum abdominal strain offered by the defecation. The application in imaging of these two principles reveal the masked or underestimated prolapses at the time of the physical examination. Rigorous application of the technique guarantees almost equivalent results from the two examinations. The CCD provides voiding views and improved analysis of the anorectal pathology (intussusceptions, anismus) but involves radiation and a more invasive examinatio

The standardization of terminology for researchers in female pelvic floor disorders.

The lack of standardized terminology in pelvic floor disorders (pelvic organ prolapse, urinary incontinence, and fecal incontinence) is a major obstacle to performing and interpreting research. The National Institutes of Health convened the Terminology Workshop for Researchers in Female Pelvic Floor Disorders to: (1) agree on standard terms for defining conditions and outcomes; (2) make recommendations for minimum data collection for research; and (3) identify high priority issues for future research. Pelvic organ prolapse was defined by physical examination staging using the International Con

[Laparoscopic therapy of functional disorders of the rectum and pelvic floor].

Within 4.5 years, 72 laparoscopic rectopexies were performed. The indications included rectal prolapse, morphologic outlet-constipation and a combination of both. Using a modified suture rectopexy (according to Sudeck), without taking any foreign material we resected the sigmoid in 39 patients. In four cases, a resection of the sigmoid colon was carried out. Conversion rate was 2% and complications that needed reoperation occurred in 9%. No patient died; laparotomy rate was only 4%. In the mean follow up period of 24.1 months (max. 50 months), no recurrent prolapse occurred. Incontinence was a

Adherence and continuance intention of biofeedback-assisted pelvic floor muscle training: a theory-driven study among women with pelvic floor disorders.

BACKGROUND: Adherence to pelvic floor muscle training (PFMT) remains a major challenge in the conservative management of pelvic floor disorders in women. Although biofeedback-assisted PFMT facilitates motor learning, sustained engagement may be influenced by a range of behavioral and experiential factors. This study investigated clinical and behavioral determinants of engagement with PFMT technologies and examined changes in manometry-derived pelvic floor muscle performance during a supervised biofeedback-assisted PFMT. METHODS: This prospective observational study examined outpatient care fo

Pelvic Pain and Pelvic Floor Disorders in Women: A Physiatrist's Approach to Epidemiology and Examination.

Chronic pelvic pain is a complex diagnosis that has a significant impact on quality of life and function in women of all ages. Symptoms often span across numerous organ systems and involve several types of pain including visceral, neuropathic, musculoskeletal, and psychological, making management and treatment difficult. To adequately assess and recognize etiologies of pelvic pain, it is critical to first understand the specialized skills required for history taking and physical examination. This article aims to serve as a guide to understanding the physiatrist's approach to history taking and

Factors Associated With Self-reported Family History of Pelvic Floor Disorders.

IMPORTANCE: Although useful to drive prevention efforts, awareness of family history (FH) of pelvic organ prolapse (POP) or urinary incontinence (UI) may be influenced by factors other than the actual presence or absence of POP/UI in a family member. This analysis identifies demographic and clinical factors associated with positive self-reported family history (FH POP/UI +) during the first pregnancy and through 1-year postpartum (1-yr PP). OBJECTIVES: Based on self-reported FH during the third trimester, the objectives were to evaluate differences between nulliparous women with and without F

Managing female pelvic floor disorders: a medical device review and appraisal.

Pelvic floor disorders (PFDs) will affect most women during their lifetime. Sequelae such as pelvic organ prolapse, stress urinary incontinence, chronic pain and dyspareunia significantly impact overall quality of life. Interventions to manage or eliminate symptoms from PFDs aim to restore support of the pelvic floor. Pessaries have been used to mechanically counteract PFDs for thousands of years, but do not offer a cure. By contrast, surgically implanted grafts or mesh offer patients a more permanent resolution but have been in wide use within the pelvis for less than 30 years. In this perspe
