Ultrasound in the assessment of pelvic organ prolapse.

Imaging is increasingly being used in urogynaecology. Because of low cost and universal availability, ultrasound (US) is the most commonly used diagnostic modality, which allows the observation of manoeuvres such as Valsalva and pelvic floor muscle contraction in real time. The ability to see beyond surface anatomy is particularly important in the posterior compartment and in obstructed defecation where this method may replace defecation proctography. Imaging is especially useful in the form of 3D/4D multiplanar and tomographic translabial US, as these modalities give access to the axial plane

Surgery for pelvic organ prolapse: a historical perspective.

Surgical treatment of pelvic organ prolapse has evolved from the use of pomegranates as pessary devices to contemporary robot-assisted laparoscopic sacral colpopexy. Symptomatic pelvic organ prolapse requires correction of all the defects to achieve optimal outcomes. Factors to consider in selecting the appropriate repair include patient's age; stage of prolapse; vaginal length; hormonal status; desire for uterine preservation and coitus; symptoms of sexual, urinary, or bowel dysfunction; and any comorbidities that influence her eligibility for anesthesia or chronically increase intra-abdomina

Is cervical elongation associated with pelvic organ prolapse?

INTRODUCTION AND HYPOTHESIS: It is commonly believed that pelvic organ prolapse (POP) is associated with cervical elongation. However, cervical lengths have not been formally compared between women with prolapse and those with normal support. METHODS: Cervix and uterine corpus lengths were measured on magnetic resonance images in a case-control study of 51 women with prolapse and 46 women with normal support determined by the Pelvic Organ Prolapse Quantification (POP-Q) examination. Group matching ensured similar demographics in both groups. Ranges for normal cervical lengths were determined

Fast magnetic resonance imaging of pelvic organ prolapse.

Pelvic organ prolapse is abnormal displacement of the pelvic organs from their normal anatomical position. Patients may present with a variety of symptoms, including pain, incontinence, constipation, urinary retention, and defecatory dysfunction. Any combination of cystocele, rectocele, enterocele, sigmoidocele, peritoneocele, and prolapse of the vagina and uterus may occur. Therefore, accurate preoperative evaluation of each organ is important for proper surgical planning. Compared with physical examination and other imaging modalities, advantages of magnetic resonance imaging (MRI) include a

Conservative and Nonsurgical Management of Pelvic Organ Prolapse.

Pelvic organ prolapse is a common condition for which treatment should be guided by symptom burden, functional impairment, and patient goals. Conservative management represents first-line therapy and includes expectant management, lifestyle and behavioral modification, pelvic floor physiotherapy, and vaginal pessary treatment. This review summarizes contemporary evidence supporting these approaches, including pelvic floor muscle training, constipation and activity management, and pessary fitting, maintenance, and outcomes. We additionally highlight recent advances in pelvic floor rehabilitatio

Pelvic Organ Prolapse: Current Challenges and Future Perspectives.

Pelvic organ prolapse (POP) affects millions of women around the world, with age-standardized prevalence rates of 2769 per 100,000 women in 2021. Although it greatly affects quality of life (QoL), only 18-50% of women experiencing this issue seek medical attention, largely due to a lack of knowledge, misunderstandings about the condition, and obstacles to accessing healthcare. This narrative review explores the progression of POP management towards a focus on patient-centered care, highlighting the importance of personalized treatment strategies that prioritize patient-reported outcomes (PROs)

Sexual function in women with pelvic organ prolapse.

A woman's sexuality is a complex phenomenon involving several factors, among which age and health are the most important. However, other aspects are not negligible. The impact of pelvic organ prolapse on sexual function cannot be ignored, as the reported prevalence of pelvic organ prolapse in female population exceeds 50%. This article presents a systematic review of articles dealing with the impact of pelvic organ prolapse on female sexual function. Sexual dysfunction is common in women with pelvic organ prolapse, regardless of prolapse stage or the compartment affected. Estrogen therapy has

Incidental tubal pathology from pelvic organ prolapse surgery.

Incidental gynaecologic malignancies are detected in up to 1.3% of pelvic organ prolapse (POP) surgeries. There is limited data on the incidence of fallopian tube pre-malignancies, such as serous tubal intraepithelial lesion (STIL) and carcinoma (STIC), which progress to ovarian cancer in 10.5% of patients over 5 years. We report two new cases of incidental tubal pathology after POP surgery. Patient one is a woman in her 70s who underwent a laparoscopically assisted vaginal hysterectomy and bilateral salpingectomy with sacrocolpopexy, revealing STIC and STIL in the left adnexa. Follow-up lapar
