Interventional Management for Pelvic Pain.

Interventional procedures can be applied for diagnostic evaluation and treatment of the patient with pelvic pain, often once more conservative measures have failed to provide relief. This article reviews interventional management strategies for pelvic pain. We review superior and inferior hypogastric plexus blocks, ganglion impar blocks, transversus abdominis plane blocks, ilioinguinal, iliohypogastric and genitofemoral blocks, pudendal nerve blocks, and selective nerve root blocks. Additionally, we discuss trigger point injections, sacroiliac joint injections, and neuromodulation approaches.

Management of Chronic Pelvic Pain.

The management of chronic pelvic pain poses a significant challenge to both physicians and patients. Patients should be evaluated for both gynecologic and nongynecologic conditions, and >1 source may be identified. Treatment involves medical as well as surgical options, and it is important to set realistic goals with patients. Treatment may not be curative, but should improve the patient's quality of life.

Chronic Pelvic Pain in Women.

Chronic pelvic pain in women is defined as persistent, noncyclic pain perceived to be in structures related to the pelvis and lasting more than six months. Often no specific etiology can be identified, and it can be conceptualized as a chronic regional pain syndrome or functional somatic pain syndrome. It is typically associated with other functional somatic pain syndromes (e.g., irritable bowel syndrome, nonspecific chronic fatigue syndrome) and mental health disorders (e.g., posttraumatic stress disorder, depression). Diagnosis is based on findings from the history and physical examination.

[Endometriosis and chronic pelvic pain].

OBJECTIVES: To analyse pathophysiology and clinical symptoms of chronic pelvic pain and their therapeutic care. MATERIAL AND METHODS: Review of articles and consensus conferences published on this subject in the Medline(®) (PubMed(®)) database. RESULTS: Pain importance and endometriosis lesion size are somewhat linked. In the case of chronic pains, endometriosis lesions are not always the only cause of the pain. Imaging consists mostly of MRI and transvaginal ultrasonography. Treatment must be offered to patients with painful endometriosis. Hormonal treatment meant to cause amenorrhea is rec

[Chronic pelvic pain and endometriosis].

Chronic pelvic pain and endometriosis remain two of the most perplexing problems in gynaecology. In some women, the problem is to determine whether or not endometriosis causes the pain they are consulting for. Deep pelvic endometriosis presents essentially in the form of a painful syndrome dominated by deep dyspareunia and painful functional symptoms that recur according to the menstrual cycle. The semiology is directly correlated with the location of the lesions (bladder, rectum). Lesions of the utero-sacral ligaments are the most frequent deeply infiltrating endometriosis lesions. The follow

Chronic pelvic pain in men.

Chronic pelvic pain is a condition which receives less attention in men than in women. It is often difficult to diagnose and more difficult to treat. The new classification of prostatis and its variants has introduced the term 'chronic pelvic pain syndrome' which underlines the difficulties in dealing with this disorder which may represent a variety of chronically painful conditions with a large functional component.

Understanding chronic pelvic pain syndrome.

Patients with non-inflammatory chronic pelvic pain syndrome, the largest group of prostatitis patients according to the US National Institute of Diabetes and Digestive and Kidney Diseases classification, are characterized by the absence of objective findings. Nothing thus links the symptoms of this disease to the prostate or other male organs in particular. For this reason, observations on interstitial cystitis in women are of interest to understand the chronic pelvic pain syndrome. New information from studies on the inflammatory response in expressed prostatic secretion in patients with chro

Gastroenterological causes of pelvic pain.

Chronic pelvic pain is a common condition, which accounts for up to 10% of gynecological consultations and for over a third of diagnostic laparoscopies. In addition to gynecological etiologies for the pelvic pain, the physician must also consider gastroenterological, urological, and neurological disease as a possible basis for the pain. This article discusses the major gastroenterological causes of pelvic pain.

Pelvic pain. A SAFE approach.

The efficacy of a multidisciplinary approach for the treatment of chronic pain has been well documented. A recent randomized prospective trial by Peters and co-workers confirmed that multidisciplinary management of chronic pelvic pain resulted in greater improvement in pain severity, increased employment, and improved functional health scores when compared with a traditional medical approach. Unfortunately, many gynecologists do not have the luxury of working in these types of settings. The SAFE approach allows the busy practitioner to evaluate the nonorganic and organic nature of pelvic pain

Endosalpingiosis and chronic pelvic pain.

OBJECTIVE: To show that endosalpingiosis (ES) is a frequently underdiagnosed or misdiagnosed entity in women thought to have endometriosis (EM) on initial pathologic review and that ES alone may be an independent cause of pelvic pain. STUDY DESIGN: A retro-spective review of pathology records from gynecologic cases from June 1992 to November 1994 revealed 37 cases of EM and 5 cases of ES. These cases were reviewed by a single pathologist (J.M.), who assigned a final diagnosis. Preoperative diagnosis or symptoms that led to surgery were compared to the initial and final pathologic diagnoses.

Diagnosis of acute pelvic pain.

The diagnosis of acute pelvic pain in the woman of reproductive age represents a major clinical challenge. In approaching such a patient, the clinician must differentiate between pregnancy-related causes, gynecologic disorders, and nonreproductive tract causes. A careful history and physical examination, along with selective and knowledgeable use of diagnostic tests and procedures, are essential to the diagnostic process. Diagnostic laparoscopy represents the reference standard for diagnosis of many of its possible causes and can obviate the need for exploratory laparotomy. Once competing diag

The Pelvic Pain Bias Assessment: a new assessment of interpretation bias specific to pelvic pain.

Pelvic pain is a common and often debilitating experience with limited treatment options. Improving understanding of the psychosocial mechanisms involved in pelvic pain experiences will enhance the efficacy of pain management interventions. Interpretation bias is a promising psychosocial factor in leading models of chronic pain, yet its relevance for pelvic pain remains poorly understood. The current study sought to assess disorder specific interpretation bias in people with endometriosis and other pelvic pain-related conditions. To do so, we developed a new interpretation bias assessment tool

Incidence and predictors of persistent pelvic pain following hysterectomy in women with chronic pelvic pain.

BACKGROUND: Chronic pelvic pain is a debilitating problem that afflicts 15% to 20% of women in the United States. Although more than 200,000 hysterectomies are performed annually for the treatment of chronic pelvic pain, previous studies indicate that 1 in 4 women undergo the discomfort and morbidity of hysterectomy without the relief of pain. The factors that predict treatment failure remain poorly characterized. OBJECTIVE: To describe the incidence of persistent pelvic pain 6 months following hysterectomy in women with chronic pelvic pain and determine whether a simple, self-reported measur

Endometriosis and mechanisms of pelvic pain.

Endometriosis remains an enigmatic disorder in that the cause, the natural history, and the precise mechanisms by which it causes pain are not completely understood. The pain symptoms most commonly attributed to endometriosis are dysmenorrhea, dyspareunia, and chronic pelvic pain. Pain may be due to nociceptive, inflammatory, or neuropathic mechanisms, and there is evidence that all 3 of these mechanisms are relevant to endometriosis-associated pelvic pain. It is proposed that the clinically observed inconsistencies of the relationships of endometriosis severity and the presence or severity of
