AIM: Soccer places repeated demands on short-duration, high-intensity performance, including sprints, accelerations, jumps, and technical actions performed over the course of a match. Because these efforts depend heavily on rapid adenosine triphosphate (ATP) resynthesis, creatine is a plausible ergogenic aid for soccer players. However, findings in soccer-specific studies have been inconsistent, and previous reviews have addressed narrower questions using smaller evidence sets. This systematic review therefore examined the effects of creatine supplementation on performance, body composition, a

Unintentional doping, often caused by contaminated supplements or misinterpreted therapeutic prescriptions, poses significant health, ethical, and regulatory challenges in competitive sports. Understanding the cardiovascular risks associated with performance-enhancing substances (PESs) and the preventive role of nutrition requires integrated analysis. A systematic review was conducted in accordance with PRISMA guidelines. Searches of comprehensive bibliographic databases yielded studies published between 2015 and November 2025. Inclusion criteria encompassed peer-reviewed research on doping pr

Diuretics are commonly used in doping because they can conceal the presence of performance-enhancing substances in an athlete's urine through dilution and promote rapid weight loss. As a result, these substances are prohibited in sports by the World Anti-Doping Agency (WADA) under the S5 category ("Diuretics and Masking Agents"). Chlorthalidone, a thiazide-like diuretic, is medically used as an antihypertensive agent and is prescribed for conditions such as heart failure and liver cirrhosis. However, it is also misused in doping. The detection of chlorthalidone or its metabolite markers in an

INTRODUCTION AND AIMS: Historically, people who inject image and performance enhancing drugs (IPED) were not perceived as being at high risk of HIV or hepatitis C virus (HCV) infection. However, recent studies indicate HCV and HIV prevalences are elevated, with many HCV infections undiagnosed.
DESIGN AND METHODS: Men who inject IPEDs recruited from community settings and specialist services, including needle-syringe programs, across UK during 2016 self-completed a questionnaire. Multivariate analyses examined factors associated with HCV/HIV testing.
RESULTS: The participants' (n=562; 24% ser

PURPOSE OF REVIEW: This critical narrative review examines the nonmedical or aesthetic repurposing of pharmacological agents to accelerate weight loss, suppress appetite, manipulate fluid balance, and alter body composition in glamour-, fitness-, and appearance-oriented settings. The review uses the term "chemical diet" as an interpretive framework for this pattern of use and proposes "pharmacological malnutrition" as a conceptual description of drug-associated nutritional vulnerability, rather than as an established clinical diagnosis.
RECENT FINDINGS: Incretin-based therapies have substanti

BACKGROUND: Performance and image enhancing drugs (PIEDs) include a diverse range of substances used to improve physical performance, muscle mass or appearance. While historically associated with elite sport and bodybuilding, use has expanded to broader populations. Most PIEDs used outside clinical settings are non-prescribed and obtained through informal or online markets, creating additional health risks.
OBJECTIVE: This article aims to support general practitioners (GPs) in engaging with patients who use anabolic androgenic steroids and other PIEDs and to outline practical harm reduction s

INTRODUCTION: We investigated emerging trends in consuming behaviours for non-controlled substances in a cross sectional study on urban Italian adolescents and young adults, the reasons for consumption and risk perception as function of age, the relation with lifestyles and finally risk factors associated.
METHODS: The survey methodology involved the administration of an anonymous questionnaire. It consisted of 68 questions, divided into five sections: personal details, socioeconomic characteristics, family and peer group, free time and lifestyles, and substances use.
RESULTS: A total of 262

BACKGROUND: Capsaicin analogues have been proposed as potential ergogenic aids, but their acute effects on athletic performance remain inconsistent. This study aimed to evaluate the effects of acute capsaicin analogue intake on exercise performance using a three-level meta-analysis.
METHODS: PubMed, Web of Science, Scopus, SPORTDiscus, and the Cochrane Library were searched up to April 1, 2025. Randomized controlled trials examining acute oral capsaicin analogue intake in healthy adults were included. Outcomes were classified into strength, power, muscular endurance, and aerobic capacity. BACKGROUND: A growing body of evidence supports the use of supplements to enhance cycling performance through both direct and indirect mechanisms.
METHODS: This review was informed by a structured literature search conducted in PubMed, Scopus and Web of Science for peer-reviewed studies published up to May 2025. Studies were included if they involved human participants, were published in English and evaluated outcomes related to endurance performance, recovery or physiological function.
RESULTS: Direct enhancement with ergogenic supplements is primarily achieved via modulation of skeletal mu

The use of creatine as a dietary supplement is widespread. However, its reported performance benefit has been largely demonstrated in male populations. The aim was to evaluate the effectiveness of creatine supplementation in improving exercise performance in active females. A secondary aim was to appraise the quality of research in this area. Five databases were searched from the earliest record to July 2024. Eligible studies used supplemental creatine as an intervention with physically active female participants and reported an exercise performance-related outcome. Study quality was appraised

Preexercise caffeine intake has proven to exert ergogenic effects on cycling performance. However, controversy exists on whether caffeine ingested during prolonged exercise could attenuate performance decrements. We aimed to assess the effect of caffeine ingested during prolonged cycling on subsequent time-trial (TT) performance in trained cyclists. The study followed a triple-blinded, randomized, placebo-controlled cross-over design. Thirteen well-trained Junior cyclists (16.3 ± 0.5 years) performed a field-based 8-min TT under "fresh" conditions and after two work-matched field-based bouts d

PURPOSE: Caffeine improves cycling time trial (TT) performance; however, it is unknown whether caffeine is ergogenic when competing against other riders. The aim of this study was to investigate whether caffeine improves performance during a 4-km cycling TT when riding against a virtual opponent, and whether it is associated with increased muscle activation and at the expense of greater end-exercise central and peripheral fatigue.
METHODS: Using a randomized, crossover, and double-blind design, eleven well-trained cyclists completed a 4-km cycling TT alone without supplementation (CON), or ag

Erythropoietin receptor agonists (ERAs), including continuous erythropoietin receptor activators (CERAs), are potent blood doping substances used to enhance endurance performance by stimulating erythropoiesis. While traditionally detected through direct analysis of urine or serum samples using sarcosyl-polyacrylamide gel electrophoresis (SAR-PAGE) and western blotting, the slow urinary elimination of third-generation ERAs like CERA has shifted anti-doping strategies toward serum-based detection. This study compared the detectability and stability of CERA in urine and serum matrices and evaluat

Meldonium has been developed in the 70s in Latvia and is currently used in a limited number of countries for heart-related diseases, such as heart attack, failure, or angina pectoris. Due to its metabolic properties (decrease of lactate production, increase of glycogen use, and protective action again oxidative stress), meldonium has been abused by numerous athletes to enhance their performance. The drug has been prohibited by the World Anti-Doping Agency since 2016 and is on the S4.4.3 list (metabolic modulators) of the prohibited substances at all times. As athletes can challenge their anti-

Injuries involving rupture of the pectoralis major are relatively rare. When they do occur, it is mostly frequently in a young, athletic man. The most common cause is weight lifting that results in eccentric muscle contraction (muscle contraction against an overbearing force, leading to muscle lengthening)-specifically, the bench press. Other mechanisms for this injury include forceful abduction and external rotation of the arm. Injury can occur anywhere along the pectoralis major from its medial origin on the sternum and clavicle to its lateral tendinous insertion on the humerus. At the time

In summer 2014, the world watched as Glasgow hosted the 2014 Commonwealth Games and athletes pushed the boundaries of human performance. Sport has developed into a multi-billion pound industry leading to the development of a 'win at any cost' mentality in some individuals. The abuse of performance-enhancing drugs has developed into a sophisticated arms race between those unfairly enhancing performance and those wishing to preserve the dignity of sport and the health of the competitors. The challenge for the Commonwealth games organising committee was to ensure that competition remained fair an

An athletic male in his 40 s presented to the emergency department following a syncopal episode. He was initially found to be hypoglycaemic at 0.9 mmol/L and improved clinically with resuscitation and normalisation of blood sugar. Cardiology was consulted, as his ECG showed sinus bradycardia, incomplete right bundle branch block and first-degree heart block. Medical history revealed that the patient took berberine supplements as a performance-enhancing herbal medication. A literature search showed that berberine-induced hypoglycaemia has previously only been reported in patients with diabetes

INTRODUCTION: Rhabdomyolysis-induced acute kidney injury (RIAKI) can interrupt physical training and increase mortality in injured warfighters. The legal performance-enhancing drugs caffeine and ibuprofen, which can cause renal injury, are widely used by service members. Whether caffeine or ibuprofen affects RIAKI is unknown. Cilastatin treatment was recently identified as an experimental treatment to prevent RIAKI at injury. To determine potential interacting factors in RIAKI treatment, we test the hypothesis that caffeine and ibuprofen worsen RIAKI and interfere with treatment.
