Supraclavicular block evaluation in oncoorthopedic patients under general anesthesia using perfusion index: A prospective cohort study.

BACKGROUND: Supraclavicular brachial plexus blocks (SCBPB) are routinely placed prior to anaesthetic induction for post-operative pain relief after prolonged orthopaedic oncosurgery, since patients are required to remain awake for sensorimotor evaluation of block. If the window period after surgery but before anesthesia-reversal is employed for administering SCBPB, it bestows the quadruple advantage of being painless, not augmenting surgical bleed, longer post-operative analgesia and reduced opioid-related side effects. The problem spot is assessing SCBPB-efficacy under general anesthesia. ME

MR perfusion index as a quantitative method of evaluating epiphyseal perfusion in Legg-Calve-Perthes disease and correlation with short-term radiographic outcome: a preliminary study.

BACKGROUND: Current radiographic prognosticators of the outcome of Perthes disease can only be applied after femoral head deformity has occurred. Quantification of femoral head perfusion using the gadolinium-enhanced subtraction magnetic resonance imaging (MRI) technique may serve as an early prognosticator of outcome. The purposes of this study were 2-fold: (1) to develop a reliable method to quantify femoral head perfusion using this MRI technique; and (2) to determine whether the perfusion at early stages of Perthes disease correlates with radiographic deformity after a 2-year follow-up. The effect of patent ductus arteriosus on pre-ductal and post-ductal perfusion index in preterm neonates.

OBJECTIVE: The ductus arteriosus is a blood vessel that connects the pulmonary artery to the descending aorta during fetal life and generally undergoes spontaneous closure shortly after birth. In premature neonates it often fails to close (patent ductus arteriosus-PDA), which can result in diversion of a significant part of the left-ventricular cardiac output to the pulmonary circulation. This left-to-right shunt may result in significant increase of pulmonary blood flow and decrease of systemic perfusion (hemodynamically significant PDA-hsPDA), which may lead to severe neonatal morbidity. The

Use of the mean transit time of an intravascular contrast agent as an exchange-insensitive index of myocardial perfusion.

A simple two-compartment model was used to study the effects of water exchange on the signal produced by an inversion recovery prepared rapid gradient-echo sequence during the first passage of a low dose of an intravascular contrast agent. Water exchange at intermediate rates of exchange (1-10 Hz) between the vascular and extravascular spaces caused the form of the signal changes during the first pass to be dependent on both the fractional sizes of the vascular and extravascular compartments and on the exchange rate. Unless the effects of exchange are minimized by using a very short inversion

Peripheral perfusion index for differentiating cardiac and pulmonary dyspnea in the emergency department: A prospective observational study.

BACKGROUND: Dyspnea is a common reason for emergency department (ED) visits, often caused by cardiac or pulmonary conditions with overlapping clinical presentations. Rapid differentiation between these etiologies is crucial for timely management and prognosis. The peripheral perfusion index (PPI), derived from pulse oximetry, has emerged as a non-invasive biomarker of tissue perfusion. This study aimed to evaluate the diagnostic performance of PPI in distinguishing cardiac from pulmonary causes of dyspnea and to assess its prognostic value for clinical outcomes. METHODS: We conducted a prospe

Evaluation of perfusion index as a predictor of successful caudal block in pediatric patients: A prospective randomized study.

BACKGROUND AND AIMS: Caudal block is among the most widely administered regional anesthesia in pediatric patients. The clinical signs and objective assessments are not fast and reliable enough to provide a good feedback. Perfusion index (PI) is considered as a sensitive marker to assess the efficacy of caudal block. We aim to assess PI as an indicator for success of caudal block in pediatric patients. MATERIAL AND METHODS: Sixty pediatric patients scheduled for elective surgery of lower abdomen and below were included. Patients were randomly allocated into two groups (n = 30): Group 1 receive

Meta-Analysis of the Prognostic Value of Skin Perfusion Parameters in Sepsis Patients: Evidence Integration Based on the Mottling Score, Capillary Refill Time, and Peripheral Perfusion Index.

BACKGROUND: Despite advances in intensive care, sepsis and septic shock still have high morbidity and mortality. Microcirculatory disturbance is a key issue in sepsis, leading to hypoperfusion, hypoxia, and organ failure. The skin is a sensitive indicator of microcirculatory changes and often shows early systemic changes. Recently, bedside skin perfusion indicators such as the mottling score (MS), capillary refill time (CRT), and peripheral perfusion index (PPI) have become common in practice. However, their value in predicting outcomes lacks strong evidence. METHODS: A systematic search was

Multi-angle fluoroscopic lung perfusion imaging using x-ray pulsatility index: A pilot study in healthy subjects.

BACKGROUND: X-ray pulsatility index (XPI) quantifies cardiac-synchronous attenuation changes as a surrogate for regional lung perfusion, but current implementations are limited by projectional overlap and the lack of reference values in healthy individuals. PURPOSE: To characterize regional XPI patterns and establish preliminary reference values in healthy participants using a novel multi-angle fluoroscopic imaging system. METHODS: Nineteen healthy participants underwent an 8 s breath-hold during fluoroscopic imaging using 15 fps at four simultaneous projections (LPO, AP caudal, RPO, AP cran

Prediction of preload dependency using phenylephrine-induced peripheral perfusion index during general anaesthesia: a prospective observational study.

BACKGROUND: Tracking preload dependency non-invasively to maintain adequate tissue perfusion in the perioperative period can be challenging. The effect of phenylephrine on stroke volume is dependent upon preload. Changes in stroke volume induced by phenylephrine administration can be used to predict preload dependency. The change in the peripheral perfusion index derived from photoplethysmography signals reportedly corresponds with changes in stroke volume in situations such as body position changes in the operating room. Thus, the peripheral perfusion index can be used as a non-invasive potent
