[Pericardial window by videothoracoscope in the treatment of pericardial effusion and tamponade].

INTRODUCTION: Malignancies are the most common causes of pericardial effusion (PE) or tamponade (PT). Lung and breast carcinoma are the most frequent. The treatment of PE consists in the performance of drainage and/or pericardial window (PW) usually subxiphoid. In the present study we describe our experience with the PW by videothoracoscopic (VATS) approach, a procedure scarcely referred to in the literature. OBJECTIVE: To evaluate the efficacy of the PW by videothoracoscopic (VATS) approach in the treatment of PE and/or PT. PATIENTS AND METHOD: Retrospective study of 12 patients with PE sub

Post-cardiac injury syndrome and pericardial effusion recurrence after pericardial effusion drainage in chronic idiopathic pericardial effusion.

INTRODUCTION: The management of even large pericardial effusions in asymptomatic patients is still a matter of debate. Aim of the present study is to explore, in a multicenter setting, the rate of post-cardiac injury syndromes (PCIS) and pericardial effusion recurrence after pericardial effusion drainage procedure. MATERIAL AND METHODS: This is a multicenter international retrospective study including a consecutive cohort of patients diagnosed with large, chronic and idiopathic pericardial effusions, prospectively evaluated from January 2003 to December 2021 who underwent a clinically indicat

[Two cases of cardiac tamponade, complicated by malignant, effusive pericarditis and treated by creating a pericardial peritoneal window].

Reported are two cases of a cardiac tamponade complicated by a malignant, effusive pericarditis were treated by a pericardial, peritoneal window. In both cases, a reaccumulation of pericardial fluid was noticed, within a month after percutaneous, continuous pericardial drainage, and a tumor invasion was seen to the anterior mediastinum creating a pericardial, peritoneal window was considered useful for the treatment of the cardiac tamponade, secondary to the malignant, effusive pericarditis.

Transcatheter management of cardiac and pericardial metastatic involvement.

We performed percutaneous balloon pericardiotomy and pulmonary valvuloplasty in a woman affected with cardiac and pericardial involvement from a primary pulmonary adenocarcinoma. Pericardial window was indicated for a recurrent, symptomatic, pericardial effusion. Valvular stenosis was severe and related to metastatic infiltration of cardiac tissue. Open surgery was avoided and the procedures were completed in two steps under local anesthesia in less than 60 min. The patient had no recurrence of pulmonary stenosis or pericardial effusion at 7 months post treatment. Transcatheter techniques are

Overview of Optimal Techniques for Pericardiocentesis in Contemporary Practice.

PURPOSE OF REVIEW: This review summarizes the optimal techniques for the performance of pericardiocentesis in contemporary practice, highlighting the indications, contraindications, and techniques used. Routine pericardial catheter management and the diagnostic role of pericardial fluid analysis are described. RECENT FINDINGS: Echocardiographic-guided pericardiocentesis should be considered the therapy of choice in current clinical practice and may be performed safely despite the presence of coagulopathy and thrombocytopenia in the hands of expert operators. Computed tomography (CT)-guided te

The pericardial window: is a video-assisted thoracoscopy approach better than a surgical approach?

OBJECTIVES: The approach to the pericardial window in patients with pericardial effusion (PE) remains undefined as to whether a surgical (transthoracic or subxiphoid) or a thoracoscopic pericardial window is the optimal operative approach to PE. We hypothesized that the window into the pleural space created by the thoracoscopy might improve the outcome. METHODS: We conducted a prospective study between September 2007 and October 2009. All patients with PE diagnosed by echocardiography who attended the Cardiothoracic Department in King Fahd Hospital are included in this study. They were 30 pat

Safety and feasibility of laparoscopic pericardioperitoneal window.

BACKGROUND: Recurrent pericardial effusion remains a diagnostic and therapeutic challenge due to high recurrence rates. Laparoscopic pericardioperitoneal window represents an understudied method in the management of pericardial effusion and is a minimally invasive technique that allows continuous drainage, achieving symptomatic improvement with minimal recurrence. METHODS: We conducted a single-centre prospective study between 2022 and 2024, including patients with recurrent pericardial effusion requiring surgical intervention. A pericardioperitoneal window was created via laparoscopy for all

Malignant pericardial effusion and pericardial tumor involvement secondary to cervical cancer.

BACKGROUND: Antemortem discovery of a malignant pericardial effusion secondary to cervical cancer is uncommon and management should focus on symptom control. CASE: A patient was diagnosed with widely metastatic cervical cancer following a simple hysterectomy for presumed benign etiology. Sixteen months later, she was diagnosed with pericardial tumor involvement and a malignant pericardial effusion resulting in severe dyspnea. The patient underwent a pericardial window procedure that temporarily alleviated her symptoms. She again developed symptoms 2 weeks following the procedure and ultimatel

The role of the 'pericardial window' in AIDS.

BACKGROUND: The leading cause of pericardial effusion in urban hospitals is now AIDS-related pathologies. Clinically, these effusions are a diagnostic and management dilemma. In our institution, pericardial biopsy and operative drainage have become part of the diagnostic and management plan. Surgical intervention, however, has appeared to have little clinical impact. METHODS: A retrospective review was conducted of all patients (n = 29) diagnosed as having AIDS who underwent "pericardial window" for pericardial effusions from 1986 to 1994. RESULTS: Fluid cultures and pericardial biopsy were

Results of subxiphoid pericardiostomy in pericardial effusion.

37 patients with pericardial effusions were managed by early subxiphoid pericardiostomy under local anaesthesia in most cases (31/37). The aetiology was pyogenic infection in 21, tuberculosis in 7, trauma in 3, cardiomyopathy in one. It was unclear in five patients. The effusion recurred in two patients (one with pyogenic, the other with traumatic pericarditis). An infant with pyogenic pericarditis developed pericardial constriction. The mortality, mainly due to associated infections or diseases, was related to aetiology: 0% for tuberculosis and trauma, 19% (4/21) for pyogenic infections, 80%

Malignant pericardial effusion secondary to ovarian adenocarcinoma: a case report.

BACKGROUND: The treatment for ovarian cancer has continued to improve, resulting in disease recurrence associated with previously unusual locations. However, there are still very few documented cases involving treatment for ovarian cancer metastatic to the pericardium. CASE: A 46-year-old woman was diagnosed with and treated for primary ovarian cancer in 1999. She underwent optimal cytoreductive surgery and received adjuvant chemotherapy. In 2003 she developed ovarian cancer metastatic to the breast and was treated with additional chemotherapy. In 2005 she presented with a malignant pericardi

Thoracoscopic pericardial window creation and thoracic duct ligation in neonates.

OBJECTIVE: We describe 2 newborn infants with persistent pericardial effusion treated with thoracoscopic pericardial window and thoracic duct ligation. METHODS: Patient 1 was a premature female newborn who presented with severe cardiac anomalies, including dextrocardia. She was treated with pulmonary artery handing and pacemaker placement for complete cardiac block. Postoperatively, she developed pericarditis with persistent symptomatic pericardial effusion. She did not improve despite pericardial drain placement. She was treated with a thoracoscopic pericardial window. Patient 2 was a newbor

Mid-Term Efficacy of Subxiphoid Versus Transpleural Pericardial Window for Pericardial Effusion.

BACKGROUND: The optimal surgical technique for drainage of pericardial effusions is frequently debated. Transpleural drainage via thoracotomy or thoracoscopy is hypothesized to provide more durable freedom from recurrent pericardial effusion than a subxiphoid pericardial window. We sought to compare operative outcomes and mid-term freedom from recurrent effusion between both approaches in patients with nontraumatic pericardial effusions. METHODS: All patients at our institution who underwent a pericardial window from 2001 to 2018 were identified. After excluding those who underwent recent car

Pericardioperitoneal and pericardiopleural windows: A drainage technique for the treatment of recurrent cardiac tamponade. A case report.

INTRODUCTION: Given that pericardial effusion may sometimes lead to cardiac tamponade and chronic heart failure, its management seems absolutely essential. In case of a poor response to medical therapy, surgical drainage of the effusion is required. Although some drainage procedures for pericardial effusion (e.g., temporary puncture, pericardiopleural drainage, and pericardioperitoneal drainage) are currently used in clinical practice, their long-term efficacy remains unclear. PRESENTATION OF CASE: We present a case of a 58-year old female with recurrent pericardial effusion secondary to syst

Pericardial effusion cytology: malignancy rates, patterns of metastasis, comparison with pericardial window, and genomic correlates.

INTRODUCTION: Cytologic evaluation of pericardial fluid is essential for diagnosing malignant pericardial effusions secondary to metastatic disease and for guiding appropriate clinical management; however, large cohort and up-to-date studies on malignancy rates and distribution of primary tumor sites is lacking. MATERIALS AND METHODS: A retrospective analysis of pericardial fluid specimens from 2 large academic medical centers over a 10-year period was conducted. Clinical and specimen characteristics were correlated with cytologic diagnoses, and compared with surgical pathology pericardial sp

Surgical management of aortopulmonary window.

BACKGROUND: Aortopulmonary window is a rare anomaly, and a variety of surgical techniques have been described for its closure. METHODS: We treated 6 infants with aortopulmonary window between 1993 and 1995. Three had associated type A interrupted aortic arch, and another had a muscular ventricular septal defect. The diagnosis was made by echocardiography, confirmed by cardiac catheterization in 4 infants. In 1 very sick neonate with interrupted arch, diagnosis of the window was considerably delayed. In 4 patients, we closed the window by using a flap of pulmonary artery, which was reconstruct

Long-Term Outcome of Pericardial Drainage in Cases of Chronic, Large, Hemodynamically Insignificant, C-Reactive Protein Negative, Idiopathic Pericardial Effusions.

Pericardial effusion (PE) prognosis depends on the underlying etiology. We sought to assess the outcome of patients with chronic (>3 months), large (diastolic echo-free space >2 cm), idiopathic (without apparent etiology), C-reactive protein (CRP) negative PE, a topic where data are lacking. A total of n = 74 patients were enrolled in this study. Patients were treated by pericardiocentesis (n = 39) or surgical pericardial "window" (PW) (n = 13) or conservatively (n = 22). The median follow-up was 24 months (interquartile range: 15 to 38). Among those patients who had PE drained (n = 52), PE re
