UNLABELLED: Anorectal injuries, with the exception of iatrogenic damage, are rare. BACKGROUND: Perineal gangrene was first described in healthy, young males more than two and one-half centuries ago. This disease, referred to commonly as Fournier gangrene, was marked by rapid progression, high mortality, and unknown etiology. In the last century the pathologic processes were described and accounts of perineal gangrene were reported in females. We have studied 20 patients with the descending perineum syndrome, half of whom were incontinent of faeces. Objective criteria were established in 103 control subjects for determining the relationship of the perineum to the bony pelvis. Maldescendus testis is a common entity in male genitourinary abnormalities. However, its perineal location is less prevalent. Despite its rarity, vigilant clinical examination helps in proper diagnosis and timely treatment to evade its eventual detrimental effects. Malignant mesothelioma is a rare aggressive tumour of the mesothelium with a propensity to spread locally and, rarely, to distant organs. The latest advances in its diagnosis and treatment have led to an increase in unusual disease presentations. Although a direct invasion of the perineum has been previously described in a men, a malignant mesothelioma spreading to the perianal region was never reported in a women. We presented a rare case of malignant mesothelioma recurrence spreading from the peritoneal cavity to the perineum through the rectovaginal space.

BACKGROUND: Immediate reconstruction of perineal defects secondary to abdominoperineal resection (APR) or pelvic exenteration with pedicled flaps decreases postoperative wound complications when compared with direct closure in high-risk patients.
