Preventing toothbrush abrasion and the efficacy of the Alert toothbrush: a review and patient study.

The literature recognizes two etiologies for periodontal attachment loss. On a statistical basis, bacterial periodontal disease causes attachment loss interproximally, while trauma, or self-inflicted periodontal disease, causes attachment loss on the buccal and lingual tooth surfaces. One study shows that the prevalence of attachment loss caused by abrasion is only slightly less than the attachment loss caused by bacterial periodontal disease. While the clinical community focuses on the prevention and treatment of bacterial disease, a large percentage of periodontal attachment loss (caused by

The effect of smoking on individuals with minimal periodontal destruction.

Recent studies have demonstrated that smoking is associated with periodontal destruction. The majority of these studies have focused on periodontal disease groups with moderate or severe periodontal destruction. Additionally, there have been few reports investigating the relationship between smoking and gingival recession. The goal of this report was to investigate the effect of smoking on periodontal destruction and recession in subjects with minimal or no interproximal attachment loss. This is a cross-sectional study of 142 non-smoking subjects and 51 smoking subjects. Subjects could have no

Epidemiology of periodontal status in dentate adults in France, 2002-2003.

BACKGROUND AND OBJECTIVE: Few recent nationwide studies of the periodontal landscape in European countries have been developed from the point of view of attachment loss and pocket depth. Decision makers are not always in a position to estimate the burden of periodontal disease. The purpose of this study was to investigate the prevalence and the oral distribution of periodontal status among dentate adults in the general population of France. MATERIAL AND METHODS: In 2002-2003, a National Periodontal and Systemic Examination Survey was organized to a stratified quota sample of 2144 adults, aged

Clinical and other risk indicators for early periodontitis in adults.

BACKGROUND: Periodontal diseases affect over half the adults in the U.S., disproportionately affecting minority populations. Periodontitis can be treated in early stages, but it is not clear what features indicate, or could be risk factors for, early stages of periodontal attachment loss. This study aimed to evaluate associations between clinical and other risk indicators of early periodontitis. METHODS: A cross-sectional evaluation of 225 healthy and early periodontitis adults aged 20 to 40 years was performed. Clinical measurements, demographic information, and smoking histories were record

Two critical confounding factors in periodontal epidemiology.

The prevalence and severity of periodontitis is considerably lower than was previously estimated. The available epidemiological methods are based on the premise that loss of periodontal attachment is a unique sign of periodontitis. However, additional factors resulting in loss of periodontal attachment confound data obtained from modern studies. The physiological process of continuous tooth eruption results in loss of periodontal attachment over a lifetime, at an approximate rate of 0.1 mm/year. Anthropological and clinical evidence for continuous tooth eruption is reviewed. The position of th

Dietary therapy and non-surgical periodontal treatment in obese patients with chronic periodontitis.

AIM: The effect of dietary weight loss intervention on periodontal therapy is unknown. Therefore, we aimed to evaluate whether weight loss improves the response of obese subjects to non-surgical periodontal treatment. MATERIALS AND METHODS: This interventional study in obese patients was conducted at the University Hospital Dr. Peset (Valencia, Spain). Patients were divided into two groups with and without dietary therapy. All participants received non-surgical periodontal treatment. Periodontal, anthropometric and biochemical parameters were assessed at baseline and 12 weeks. RESULTS: A tot

Reduced Periodontal Support for Lower Central Incisor - A 3D Finite Element Analysis of Compressive Stress in the Periodontium.

BACKGROUND: The aim of this study was to assess the stress concentration in simulated periodontal alveolar bone containing healthy teeth with and without attachment loss. METHODS: Six 3-D models of a lower central incisor were created simulating the teeth structure, cancellous and cortical bone and periodontal ligament. Each model presented a 1mm increasing distance between cement-enamel junction (CEJ) and alveolar bone crest (ABC) (1 to 6mm). A 100N, 45-degree load was applied to the buccal face of the lower central incisor. The effects of Minimum Principal Stress (MPS) on lamina dura (LD) a

Tooth rotational control with varying attachment configurations in clear aligners under compromised periodontal ligament support: a finite element study.

BACKGROUND: With the high prevalence of periodontitis, tooth rotation resulting from periodontal damage can be corrected through orthodontic treatment; however, the biomechanical complexity of tooth movement in compromised periodontal tissues, combined with the inherently low rotation accuracy of clear aligners, present significant challenges to tooth rotational control. This finite element study aimed to explore appropriate attachment configurations for tooth rotation under periodontally compromised conditions. METHODS: Based on cone beam computed tomography data of a patient, three-dimension

Changes in Osseous Morphology Following Non-Surgical Periodontal Therapy: A Possible Paradigm Shift for the Treatment of Intrabony Defects?

Sub-gingival non-surgical periodontal therapy (NSPT) constitutes step 2 of periodontal therapy, aiming to disrupt the subgingival microbial biofilm and resolve or reduce inflammation of the supracrestal periodontal tissues. A plethora of original studies and reviews have been published over several decades, highlighting its fundamental importance in periodontal therapy and in tooth survival. Evidence shows that step 2 therapy leads to probing pocket depth reduction, associated with increase in gingival recession, clinical attachment level gain and improvements in patient-reported outcome measu

The prevalence of periodontal disease measures in elderly adults, aged 79 and older.

From 1996-98, we examined 449 adults (mean age 85 years) from the 1982 Iowa 65+ Rural Health Study, in the field using headlight, mirror, and periodontal probe. Ninety-six of the 342 dentate subjects were excluded from the follow-up examination due to contraindications to probing, and 10 could not be assessed due to refusals, fatigue or other reasons. For all remaining subjects (n=236), attachment loss, defined as recession plus probing depth, was determined at four sites per tooth. Ninety-one percent of the subjects had at least one site with 4+ mm of attachment loss, 45% had one or more site

Mass spectrometric analysis of gingival crevicular fluid biomarkers can predict periodontal disease progression.

BACKGROUND AND OBJECTIVE: Gingival crevicular fluid has been suggested as a possible source of biomarkers for periodontal disease progression. This paper describes a technique for the analysis of gingival crevicular fluid from individual sites using mass spectrometry. It explores the novel use of mass spectrometry to examine the relationship between the relative amounts of proteins and peptides in gingival crevicular fluid and their relationship with clinical indices and periodontal attachment loss in periodontal maintenance patients. The aim of this paper was to assess whether the mass spectr
