Peritoneal transport properties and dialysis dose affect growth and nutritional status in children on chronic peritoneal dialysis. Mid-European Pediatric Peritoneal Dialysis Study Group.

To evaluate a possible effect of peritoneal transport properties and dialysis dose on the physical development of children on chronic peritoneal dialysis, a cohort of 51 children was prospectively followed for 18 mo. Peritoneal transport characteristics were assessed by serial peritoneal equilibration tests (PET), dialysis efficacy by dialysate and residual renal clearance measurements, and growth and nutritional status by the longitudinal changes (delta) of height SD score (SDS), body mass index (BMI) SDS, and serum albumin. delta height SDS was negatively correlated with the creatinine equil

Continuous Ambulatory Peritoneal Dialysis Peritonitis Guidelines - Consensus Statement of Peritoneal Dialysis Society of India - 2020.

Continuous ambulatory peritoneal dialysis (CAPD) related peritonitis is a major cause of technique failure, morbidity, and mortality in patients on CAPD. Its prevention and management is key to success of CAPD program. Due to variability in practice, microbiological trends and sensitivity towards antibiotics, there is a need for customized guidelines for management of CAPD related peritonitis (CAPDRP) in India. With this need, Peritoneal Dialysis Society of India (PDSI) organized a structured meeting to discuss various aspects of management of CAPDRP and formulated a consensus agreement which

The culture from peritoneal dialysis catheter enhances yield of microorganism identification in peritoneal dialysis-related peritonitis.

An additional yield of culture from the removed peritoneal dialysis (PD) catheter in diagnosis of pathogen causing refractory peritonitis was assessed in 118 eligible patients from 7 PD centers. Peritoneal dialysis fluid (PDF) culture identified organisms in 86 (72.9%) patients, while the catheter culture identified organisms in 55 (46.6%) patients. PD catheter culture could additionally identify organisms in 19 patients whose PDF culture were negative, increasing the positive culture rate to 89%, in other word 16.1% reducing the culture-negative rate. PD catheter culture provided additional y

A Case of Peritoneal Dialysis-related Peritonitis Caused by Aeromonas Hydrophila in the Patient Receiving Automated Peritoneal Dialysis.

Peritoneal dialysis (PD)-related peritonitis is a major cause of injury and technique failure in patients undergoing PD. Aeromonas hydrophila is ubiquitous in the environment, and is a Gram-negative rod associated with infections in fish and amphibians in most cases; however, it can also cause opportunistic infections in immunocompromised patients. We report a case of A. hydrophila peritonitis in a 56-year-old male on automated PD. Peritonitis may have been caused by contamination of the Set Plus, a component of the automated peritoneal dialysis device. Although Set Plus is disposable, the pat

Paraneoplastic leukocytosis in peritoneal dialysis.

Liposarcoma (LPS) typically occurs in the extremities and retroperitoneum, but rarely occurs in visceral organs such as pancreas. Only few cases of primary pancreatic LPS have been reported in literature. Here we describe the first case of primary pancreatic LPS in a patient receiving continuous ambulatory peritoneal dialysis (CAPD). This patient was a 60-year-old female who had been on CAPD for 3 years due to kidney failure from type 2 diabetes mellitus. She presented with a 2-week history of intermittent fever measuring up to 38 degrees Celsius. The septic workup including peripheral blood,

Ergothioneine Depletion in Peritoneal Dialysis.

KEY POINTS: Hemodialysis has been shown to greatly deplete ergothioneine, a diet-derived antioxidant. Peritoneal dialysis, which provides lower clearance of solutes, also depletes ergothioneine but to a lesser extent than hemodialysis. Whether ergothioneine repletion benefits people receiving dialysis remains to be tested. BACKGROUND: Dialysis may deplete the body of valuable solutes. We previously found that the diet-derived antioxidant ergothioneine was markedly depleted in patients on hemodialysis. Standard peritoneal dialysis (PD) prescriptions provide lower clearance of small molecules t

Peritoneal dialysis and heart failure.

PURPOSE OF REVIEW: Heart failure and chronic kidney disease are frequently comorbid, with both characterized by salt and water retention that lead to clinical deterioration. Peritoneal dialysis can effectively treat fluid overload and maintain euvolemia but remains underutilized. This review addresses the use of peritoneal dialysis in heart failure, including its potential advantages and also its challenges. RECENT FINDINGS: The 2019 Advancing American Kidney Health initiative incentivized increased use of peritoneal dialysis in the United States. There is, therefore, growing recognition of p

Novel Uses for Peritoneal Dialysis.

Apart from its use in patients with end-stage kidney disease or acute kidney injury, there has recently been interest in the potential use of peritoneal dialysis for other, nonrenal indications. Herein, we review two nonrenal areas that are currently being evaluated: use of liposomal-supported peritoneal dialysis for the removal of endogenous and exogenous toxins and use of peritoneal dialysis to reduce risk of secondary brain injury following ischemic stroke.

Constipation in Peritoneal Dialysis Patients.

Constipation in peritoneal dialysis (PD) is an infrequent but potentially serious condition affecting the mechanical properties of dialysis techniques and predisposing to bacterial intestinal translocation and eventual enteric peritonitis. Despite the importance of the problem, published literature is scarce, consisting mostly of uncontrolled single-center trials. This inconsistency may be attributed to the large number of clinical, radiological, and endoscopic tools that have been used in the studies with a lack of generally accepted core primary outcomes. The current narrative review discusse

Peritoneal Dialysis Access Associated Infections.

Infection is a significant driver of morbidity and mortality in patients with end-stage renal disease undergoing maintenance dialysis. In the United States, septicemia and other infections account for 8% deaths in patients undergoing dialysis. In patients undergoing peritoneal dialysis (PD), PD-related peritonitis remains the most frequent treatment-related infection and is the greatest contributor to infection-related morbidity, including risk for hospitalization, and temporary or permanent transfer to hemodialysis. In the 4 decades since the introduction of ambulatory PD in clinical practice

Peritoneal dialysis beyond kidney failure?

Compared to extracorporeal modalities, peritoneal dialysis (PD) is less invasive and more cost-effective, wherein blood is dialyzed intra-corporeally against a solution instilled in the peritoneal cavity. Although PD is mainly indicated for patients with end-stage renal failure, it has also been used for several non-renal indications. The aim of this review is to provide an overview of the role of PD beyond kidney failure. The alternative indications of PD include hypothermia, congestive heart failure, hyperammonemia and poisoning with xenobiotics. The use of PD as a treatment for acute pancre

[Focusing on peritoneal dialysis adequacy].

The optimal method to assess the adequacy of peritoneal dialysis therapies is controversial. Today, the adequacy must not be considered as a number or a concept assessed only by two parameters (total KT/V urea and total solute clearance) but defined by many more items. In the absence of data, based on theoretical considerations, the reanalysis of the CANUSA study showed that renal kidney function, rather than peritoneal clearance, was associated with improved survival. Residual renal function is considered as a major predictor factor of cardiovascular mortality. Results of this reanalysis were

Current techniques in peritoneal dialysis.

OBJECTIVE: To provide a current overview of the technique of peritoneal dialysis in dogs and cats. CLINICAL IMPLICATION: Peritoneal dialysis is the process by which water and solutes move between blood in the peritoneal capillaries and fluid (dialysate) instilled into the peritoneal cavity, across the semipermeable membrane of the peritoneum. The primary indication for peritoneal dialysis (PD) in animals is for treatment of renal failure to correct water, solute, and acid-base abnormalities and to remove uremic toxins. SUMMARY: Peritoneal dialysis is a modality of renal replacement therapy c

Calcimimetics in peritoneal dialysis patients.

Though the prevalence of secondary hyperparathyroidism (SHP) and the related mineral metabolism (MM) changes have been reported at almost the same rate in peritoneal dialysis (PD) as in hemodialysis (HD) patients, PD patients have a higher prevalence of adynamic bone disease (ABD), suggesting that their bone is less sensitive for a given level of PTH. Furthermore, the phosphorus control seems to be better and vitamin D deficiency is more common in PD patients than in HD patients. So, the therapeutic approach to SHP and MM changes in PD patients might be different from the one applied to HD pat

Phosphate control in peritoneal dialysis.

Chronic kidney disease (CKD) is characterized by phosphorus retention and, in more advanced stages, by high serum phosphorus (P) levels. During the last decade, it has been elucidated the central role of P in the pathogenesis of CKD mineral bone disorder (CKD-MBD), determining both renal osteodystrophy and cardiovascular disease. Unfortunately, at least one third of patients on chronic dialysis have high serum P levels, with a consequent higher serum PTH levels, commonly associated with vitamin D deficiency, increased vascular calcification and the highest ratios of morbidity and mortality. Volume status in peritoneal dialysis.

Fluid homeostasis is one of the fundamental roles of the kidney and a crucial aspect in clinical management of patients on peritoneal dialysis (PD). Volume status has an important impact on the outcome of PD patients. Fluid excess leads to accelerated development of cardiovascular disease. Volume status may also impact on disorders and assessment of nutritional state. While ultrafiltration and residual urine volume have featured prominently in PD research, the importance of fluid intake and thirst in PD patients has received relatively little attention. Despite older studies suggesting that fl

[Peritoneal dialysis in cirrhotic patients].

Renal failure and the need for dialysis worsen the prognosis of patients with combined liver and kidney disease. The choice of an appropriate dialysis technique should improve the life expectancy of these patients. Hypotension, impaired defence against infections, electrolyte and acid-base imbalance, severe protein and caloric malnutrition, hyperammonemia, hyperbilirubinemia, and inadequate response to diuretics present a number of clinical problems in patients with liver insufficiency. Liver failure is therefore considered an important risk factor for any type of dialysis. Theoretically, both
