Of nine hemiplegic patients having foot-drop who were test-treated with the Philips functional electronic peroneal stimulator, only three who tolerated continued use of the stimulator for five weeks had improvement in function. Findings in these cases indicate that to be selected for trial use of a stimulator a hemiplegic patient should fulfill the following criteria: (1) be in the hospital, (2) be ambulant with or without a cane and/or short leg brace, (3) have no severe contracture of the ankle joint, (4) have no severe spasticity, (5) have no obvious lower motor neuron lesions, (6) be coope

PURPOSE: The common peroneal nerve (CPN) and the superficial peroneal nerve (SPN) are among the most commonly iatrogenically injured peripheral nerves. The aim of this study was to localize the division point of the CPN into the deep peroneal nerve (DPN) and SPN and the exit point of the SPN through the crural fascia in relation to the length of the fibula.
METHODS: 101 lower extremities were included in this given study. The distance between the apex of the fibular head and the distal tip of the lateral malleolus was defined as the fibular length (FL). Measurements were taken from the apex o

The foot and ankle specialist will frequently encounter patients with dorsal midfoot pain in clinic. In the presence of midfoot pain and/or paraesthesia, nerve entrapment must be considered. The authors report the outcome of a case series of patients who underwent surgical release of the DPN. Between 2011-2017, a single surgeon operated on seven patients with a diagnosis of DPN entrapment. A retrospective review of the patient's clinical notes was performed, including the operative findings. The average age at presentation was 47 years (range, 31-70 years), and the left foot was affected in fo

This report describes a case of complete rupture of the peroneal nerve as a consequence of low velocity trauma. A 54-year-old physically fit patient suffered a complex trauma with complete nerve discontinuity as a result of knee joint distortion without external force. The initial medical findings were unremarkable, in particular neither the accident medical history nor the initial sensitivity impairment suggested the presence of serious knee damage; however, during clinical diagnostics a complex trauma with rupture of the peroneal nerve was found. Accordingly, an extensive revision with nerve

We introduce a case report of a woman that was operated of foot surgery under locoregional anesthesia with an echo-guided peripheral sciatic-popliteal nerve block. As post operatory complication a peroneal nerve injury was noticed. We revised differential diagnosis of peripheral nerve block and therapeutic strategy we should take. In our case the finding was a high degree axonotmesis secondary to extrinsic compressure due to pneumatic tourniquet placed in the ankle used during surgery. There was no relationship with the anesthetic technique.

The fabella is a normal anatomical variant and has been found in up to 30% of the general population. We present the case of a 67-year-old man with pain down the lateral aspect of the left lower limb of 18 months duration. A clinical examination revealed a palpable fabella and nerve conduction studies confirmed a common peroneal nerve neuropathy at its level. Dynamic ultrasound scan and MRI of the knee showed the fabella to be impinging on the common peroneal nerve. Operative excision of the fabella was performed with significant improvement at 3 months and full recovery at 1 year. A literatur

Peroneal neuropathy is one of the common focal mononeuropathies in the lower extremities occurring in both adults and children. Foot drop due to weakness of ankle dorsiflexion is the most common presentation of a peroneal neuropathy. It may also result from other causes involving the upper or lower motor neurons. Disorders that must be distinguished from peroneal neuropathy include sciatic mononeuropathy, lumbosacral plexopathy, motor neuron disease, polyneuropathy, and an L5 radiculopathy. To establish a diagnosis, electrodiagnostic studies have been used to localize the level of the abnormal

The accessory deep peroneal nerve (ADPN) is a common variant branch of the superficial peroneal nerve. It unrarely participates in the innervation of the extensor digitorum brevis muscle and interferes with the differential diagnosis of peroneal nerve lesions. Several electrophysiological and anatomical studies have been conducted in order to document the topography, characteristics and prevalence of ADPN, presenting significantly different results. ADPN existence is of great clinical and surgical importance, thus the aim of this study is to select and present all the relevant data available i

The history of functional motor recovery after reconstruction of traction injury to the common peroneal nerve is poor, regardless of technique and regardless of the experience of the surgeon doing the reconstruction. The hypothesis tested is that the failure of functional motor recovery after common peroneal nerve traction injury is because the zone of injury extends beyond the visible region of peroneal nerve in continuity injury and into the muscle entry zone of the motor nerve terminations. The opportunity arose to examine pathologically this suspected distal zone of injury in one patient.

Peroneal nerve injury usually results from fracture of the fibula neck or direct pressure from an ill-fitting plaster cast. Clinically, these patients present with "drop foot." Compression neuropathy caused by a ganglion is rarely encountered. This report concerns the clinical and radiologic evaluation of a 57-year-old man admitted to our clinic with left drop foot. A mass compressing the peroneal nerve at the level of the fibula neck was excised totally and diagnosed as a ganglion.

A histomorphometric analysis of the superficial peroneal nerve was made in 15 cases of leprosy. Thirteen patients presented clinical signs of leprous neuropathy, while the other two showed only cutaneous signs of leprosy. The presence of M. leprae in all the nerves sampled, and the appearance of the histologic lesions, made it possible to confirm the diagnosis of leprosy and to specify the type of leprosy in each case, even in the absence of clinical signs of leprous neuropathy. Correlation of the histomorphometric results with the duration of development of the disease and with the time elaps

The peroneal nerve is one of the most commonly injured nerves of the lower extremity. Nerve grafting has been shown to result in poor functional outcomes. The aim of this study was to evaluate and compare anatomical feasibility as well as axon count of the tibial nerve motor branches and the tibialis anterior motor branch for a direct nerve transfer to reconstruct ankle dorsiflexion. In an anatomical study on 26 human body donors (52 extremities) the muscular branches to the lateral (GCL) and the medial head (GCM) of the gastrocnemius muscle, the soleus muscle (S) as well as the tibialis anter

Different mechanisms including knee dislocation, replacement surgery, nerve tumor, lumbar disc herniation, sharp injury, and gunshot wound lead to foot drop. Several surgical techniques have been used for treatment of foot drop, however, they have had sub-optimal outcomes. Soleus branch of tibial nerve is a good donor for nerve transfer for treatment of foot drop. In this is retrospective study, we reviewed medical records of 6 consecutive patients with sustained foot drop following injury to lumbar root or peroneal nerve, who underwent transfer of the soleus branch of tibial nerve to deep per

BACKGROUND: Patients in whom conventional peroneal nerve repair surgery failed to reconstitute useful foot lift need to be evaluated for their suitability to undergo a concomitant tendon transfer procedure or nerve transfers.
OBJECTIVE: To report our first clinical experience with nerve transfers for persistent traumatic peroneal nerve palsy.
METHODS: Between 2007 and 2013, 8 patients were operated on for foot drop after unsuccessful nerve surgery. Six patients without fatty degeneration of the anterior tibial muscle and proximal lesion of the peroneal nerve were oriented for tibial to peron

BACKGROUND: Theoretically, sensory nerve action potential (SNAP) of the superficial peroneal nerve (SPN) should be preserved when L5 roots are damaged. Previous study indicated that SNAP of SPN was lost or reduced in amplitude in patiens with L5 radiculopathy. To address this issue, this study compared results of SPN sensory conduction studies between healthy subjects and patients with L5 radiculopathy.
METHODS: Ninety-four healthy subjects were enrolled and assigned to two age groups: group I (< or = 60 years, n=61) and group II (> 60 years, n=33). Forty-one patients with unilateral L5 radic

BACKGROUND: Partial tibial nerve transfer to the motor branches of tibialis anterior is an emerging reconstructive technique for the treatment of traumatic common peroneal nerve (CPN) injury; however, few papers in the literature describe clinical outcomes.
METHODS: A prospective single-surgeon series of nine consecutive patients who underwent partial tibial nerve transfers to the motor branches of tibialis anterior for traumatic CPN injuries between 2008 and 2014. Eight patients were male and the average age at operation was 28.2 years old (range 21-39). All nine patients experienced high-en
