Flaws in advance directives that request withdrawing assisted feeding in late-stage dementia may cause premature or prolonged dying.

BACKGROUND: The terminal illness of late-stage (advanced) Alzheimer's and related dementias is progressively cruel, burdensome, and can last years if caregivers assist oral feeding and hydrating. Options to avoid prolonged dying are limited since advanced dementia patients cannot qualify for Medical Aid in Dying. Physicians and judges can insist on clear and convincing evidence that the patient wants to die-which many advance directives cannot provide. Proxies/agents' substituted judgment may not be concordant with patients' requests. While advance directives can be patients' last resort to at

[Promoting autonomy in the treatment of chronic suicidality and self-harm: a psychotherapeutic perspective].

In the Netherlands, autonomypromoting policy (APP) is increasingly applied to patients with chronic suicidality and self-harming behaviours for whom specialized psychotherapy for personality disorders is not yet feasible. A national clinical framework and a critical evaluation by MIND have recently highlighted the urgent need to articulate the psychotherapeutic principles underpinning APP. In this article we explore the practical implementation of APP, grounded in interventions from various evidence-based psychotherapeutic modalities for personality disorders that emphasize autonomy. When legal worlds collide: from research to treatment in hereditary cancer prevention.

This paper critically examines the legal and ethical dilemma in the institutionalization of a prevention and treatment technology on hereditary non-polyposis colorectal cancer (HNPCC) in Finland. Current medical practice and legislation support the notion of non-directiveness in genetic counselling, patient autonomy and personal privacy. Clinical doctors in Finland, however, proposed that a research register of HNPCC carriers should be nationalized through a legal mandate, which would require all carriers to be identified on the basis of the mutation, but also provide a national healthcare and

Posthumous sperm retrieval for the purpose of later insemination or IVF in Israel: an ethical and psychosocial critique.

In October 2003, the Attorney General of the Government of Israel published guidelines allowing posthumous sperm retrieval for the purpose of later insemination or IVF by the surviving female partner. This paper presents an ethical and psycho-social critique of the guidelines, which challenges their basic premise that personal autonomy over-rides any other ethical principle and argues that the autonomy of the adult should not over-ride the well-being of the offspring. It also shows that, despite the centrality of autonomy in the guidelines, they actually infringe on the autonomy of the decease

From free pastures to penned in: The within-person effects of psychological reactance on side-hustlers' hostility and initiative in full-time work.

Multiple jobholding is increasingly common, particularly among full-time employees who have adopted side-hustles-income-generating work from the gig economy that is performed alongside full-time work. A distinguishing feature of side-hustles is substantial autonomy in the work's timing, location, and method. This autonomy has typically been portrayed as beneficial. We shift this consensus by developing a within-person model that suggests elevated side-hustle autonomy-relative to what is typical for that person-sets the employee on a course to feel "boxed in" by their full-time job. Drawing on

Psychological need satisfaction and well-being in adults aged 80 years and older living in residential homes: using a self-determination theory perspective.

Based on the self-determination theory (SDT), this study aims to examine the psychological needs satisfaction of the elderly living in residential homes and their relationship with indicators of well-being, and then to test the contribution of each need on these indicators. Participants (N=100; Mage=86.7 years, SD=3.78) completed the measures of psychological needs satisfaction, purpose in life, personal growth and geriatric depression. Cluster analyses showed two distinct profiles: one profile with a high satisfaction of the three basic psychological needs and another profile with a low satis

Predictors of posttraumatic stress symptoms among survivors of the Oakland/Berkeley, Calif., firestorm.

OBJECTIVE: The purpose of this study was to examine factors predicting the development of posttraumatic stress symptoms after a traumatic event, the 1991 Oakland/Berkeley firestorm. The major predictive factors of interest were dissociative, anxiety, and loss of personal autonomy symptoms reported in the immediate aftermath of the fire; contact with the fire; and life stressors before and after the fire. METHOD: Subjects were recruited from several sources so that they would vary in their extent of contact with the fire. Of 187 participants who completed self-report measures about their exper

Temporal work stressors and satisfaction with work, life and health among health professionals in Switzerland.

BACKGROUND: Working in a hospital can be both rewarding and stressful. Whether one or the other is dominant depends on a number of factors ranging from shift work, physical demands, responsibilities and time pressure to job autonomy, work climate and leisure time. AIM: This study aimed to examine associations between temporal work stressors and satisfaction with work, life and health among health professionals in general, and nurses and physicians in particular. Associations were further investigated for possible mediating and intervening factors. METHODS: Cross-sectional survey data on 1232

Navigating an ambiguous abortion terrain in Addis Ababa, Ethiopia: autonomy and relational dynamics in pregnancy termination.

BACKGROUND: Ethiopia liberalized its abortion law in 2004 and issued technical guidelines in 2006 to expand access to legal and safe abortion services. However, the implementation of this progressive policy is shaped by stigma, patriarchal norms, and influential actors. As a result, women seeking abortion are required either to negotiate these power dynamics or to go through unsafe procedures. Despite this policy overhaul, there is a paucity of data on how women navigate complex abortion landscape. Therefore, this paper explores how women in Addis Ababa exercise relational autonomy amidst the

Ritualized relational autonomy in Chinese end-of-life care: balancing Confucian benevolence and individual rights.

This article develops a hypothesis-and-theory framework for ethical tensions in contemporary Chinese end-of-life care and family mourning. It examines the relationship between Confucian ren'ai (benevolence), expressed through embodied familial care, and modern rights-based commitments to bodily autonomy, dignity, and informed choice. The article offers a conceptual and normative analysis informed by interpretive cultural inquiry and existing empirical research on family-centered decision-making, advance care planning, caregiver burden, and hospice and palliative care in China. It argues that t

Parent-perceived autonomy-supportive experiences and basic psychological needs of people with complex support needs: Development and preliminary psychometric evaluation of two questionnaires.

Background: Understanding and supporting basic psychological needs of persons with complex support needs is important but difficult because of communicative challenges . We developed and tested questionnaires to obtain parents' perspectives on autonomy support and basic psychological needs of autonomy, competence, and relatedness. Method: Two parent-informant questionnaires were developed, administered, and subjected to psychometric property analyses. Participants were 63 Dutch parents of persons diagnosed with severe or profound intellectual and multiple disabilities. Results: Principal compo

Between "a lot of room for it" and "it doesn't exist"-Advancing and limiting factors of autonomy in birth as perceived by perinatal care practitioners: An interview study in Switzerland.

BACKGROUND: Numerous studies show that negative birth experiences are often related to birthing people's loss of autonomy. We argue that a fetal-focused decision-making framework and a maternal-fetal conflict lens are often applied, creating a false dichotomy between autonomy and fetal beneficence. Given the high prevalence of autonomy-depriving decision-making, it is important to understand how autonomy can be enhanced. METHODS: We interviewed 15 Swiss perinatal care practitioners (eight midwives, five physicians, and two doulas) and employed reflexive thematic analysis. We offer a reflectio

A systematic review of post-secondary transition interventions for youth with disabilities.

Purpose: Youth with disabilities have lower rates of enrollment and completion of post-secondary education compared with youth without disabilities. The objective of this systematic review is to understand the best practices and components of post-secondary transition programs for youth with disabilities. Method: Systematic searches of six international databases identified 18 studies meeting our inclusion criteria (youth with a disability, aged 15-30; focusing on post-secondary education program or intervention, published from 1997 to 2017). These studies were analyzed with respect to the cha

Suggestion of one-hand type power-assisted wheelchair and driving control for persons with hemiplegia.

Elderly people and disabled people use the wheelchair as a transportation device. However, the manual wheelchair has a large physical burden in order to use own power, and the activity on the outdoors is difficult. Moreover, the electric wheelchair causes the fall of muscular power by operation with joy stick. We suggest the one-hand type power-assisted wheelchair for persons with hemiplegia. This paper describes the driving control of straight and revolution by one-hand. The simulation was performed about straight and revolution. The result showed the validity of driving control.

A perspective on homelessness, ethics, and medical care.

People who are homeless are known to have high rates of morbidity and mortality. Yet their attitudes, values, and desires regarding end-of-life care have not been studied. This article reports on the findings from interviews with groups of homeless individuals in the Twin Cities performed by University of Minnesota researchers. It explores the implications of homelessness on two commonly discussed ethical principles-autonomy and justice.

Medical science and the madness of crowds; context for our COVID pandemic years.

Individual autonomy is fragile. We must ask whether we have been swept up with ' a crowd '; perhaps trapped within a ' thought window ', carried by mass narrative, or entranced by the certainty of the totalitarised mind.Loss of the individual to the group can occur on any scale, from ' group-think ' cognitive bias, through the eager madness of a literal crowd, to the modern global reach of propaganda and technocratic totalitarianism.Milgram electrocution experiments show potential for the mind to be rapidly totalitarised - the hypnotic, focal enthusiasm of ' engaged followers '.This work was pe

Navigating Autonomy: Enhancing Accuracy in Medical Decision-making Capacity Assessment.

Evaluating medical decision-making capability (DMC) is critical for maintaining patient autonomy and informed consent in health care. DMC refers to a patient's ability to comprehend information about their treatment, recognize its implications, reason through the options, and articulate their decision. Several assessment techniques, including the Aid to Capacity Evaluation, the Hopkins Capacity Assessment Test (HCAT), Understanding Treatment Disclosure (UTD), and the MacArthur Competence Assessment Tool, have been discussed in this review. The legal distinction between decisional ability and c

Is the categorical denial of pentobarbital for assisted suicide a violation of the constitutional right to a self-determined death in Germany?

Aiding in suicide is no criminal offence under German law. In addition, a constitutional right to a self-determined death exists, including relying on third-party assistance, where offered. To exercise such a constitutional right, persons require access to effective lethal medication. Pentobarbital is a substance commonly used in jurisdictions allowing assisted dying. Yet, in Germany, the Federal Institute for Drugs and Medical Devices, based on the Narcotic Drugs Act, categorically rejects pentobarbital licence applications with the consequence that pentobarbital is not available in Germany f
