Experiences of Trans Women and Two-Spirit Persons Accessing Women-Specific Health and Housing Services in a Downtown Neighborhood of Vancouver, Canada.

PURPOSE: Little is known about trans womens' experiences accessing gender-segregated health and housing services, particularly services for marginalized individuals living in poverty. As such, we conducted a qualitative investigation into experiences of accessing women-specific health and housing services among trans women and two-spirit persons in a downtown neighborhood of Vancouver, Canada. METHODS: Between June 2012 and May 2013 interviews were conducted with 32 trans women and two-spirit individuals who had accessed women-specific health and/or housing services. Participants were recruit

Health services use for injury amongst persons experiencing homelessness in Ontario, Canada: a population-based retrospective matched cohort study.

BACKGROUND: Homelessness is an increasingly prevalent public health concern and an important determinant of mental and physical health. The relationships between housing status and injury incidence and outcome are poorly understood. In this study we sought to both establish the epidemiology of injury amongst persons experiencing homelessness (PEH) and to determine the association between the experience of homelessness and utilization of acute care health services for injuries. METHODS: We conducted a retrospective matched cohort study including adults experiencing homelessness between April 1,

Examining the between- and within-person associations among perceived sleep quality and mental health symptoms in emergency medical service clinicians.

Disturbed sleep is a known risk factor for heightened mental health symptoms, and this association may be particularly problematic among emergency medical service (EMS) clinicians. Yet, associations between daily sleep quality and daily mental health symptoms are understudied among this vulnerable group. We used ecological momentary assessments to examine between- and within-person associations between perceived sleep quality and mental health symptoms in 79 EMS clinicians employed at a large agency in central New York. Participants completed eight daily assessments (558 total) on perceived sl

Organising housing and service provision for persons with co-occurring substance use and mental health problems: a scoping review in the ROP Municipal.

BACKGROUND: Persons with co-occurring disorders, both substance use and mental health problems (COP) may be encountered within various disciplinary systems and organisational levels of treatment. In western countries, the most common evidence-based housing programme for persons with complex, long-term needs due to severe mental illness and substance use is Housing First. The context of this study were the Nordic countries, granting universal access to treatment and free public services. The aim was to examine how multidisciplinary and integrated housing services are organised and coordinated f

Delivery of preventive services for low-income persons over age 50: a comparison of community health clinics to private doctors' offices.

This study focused on the use of 14 evidence-based preventive services for the low-income population over age 50: colorectal, breast and cervical cancer screening, cholesterol screening, counseling around diet, exercise, tobacco, alcohol and illicit drugs, and immunizations for influenza, tetanus and pneumonia. Population characteristics and rates of delivery of these preventive services are compared for low-income users of community health clinics vs private doctors' offices/HMOs. Predictors of improvement in personal recovery and activity engagement among mental health service users following a group-based sensory modulation intervention.

BACKGROUND: Mental health service users often have sensory processing difficulties affecting activity engagement and personal recovery. Since interventions targeting this area are scarce, a group-based sensory modulation intervention was developed. To determine in which settings and for whom this intervention has clinical value, this study aimed to investigate whether sociodemographic and clinical baseline characteristics were associated with improvement in personal recovery and activity engagement. METHODS: This study included participants from the intervention group (N = 107) of a trial (re

"There Was a Light Bulb Moment": Using Persona Dyads to Empathise, Explore and Develop Person-Centred Solutions in Mental Health Service Co-Design.

INTRODUCTION: Co-design of health services is an increasingly used method to meaningfully enable patient and public involvement, especially when working with priority populations whose perspectives are often overlooked in traditional top-down approaches. However, differences in power and influence among stakeholders can lead to asymmetry and the marginalisation of certain voices. We used persona dyads (patient and provider) to co-design improvements in Emergency Department care for people presenting with mental health concerns. METHODS: Persona dyads were used in two Experience-Based Co-Desig

Testing a mobile peer support intervention for persons with serious mental illness (DigiPer) in community mental health services: a feasibility study protocol.

INTRODUCTION: Persons with serious mental illness (SMI) often have coexisting medical conditions and experience a significantly reduced life expectancy compared with the general population. Peer support is considered an effective care approach for this population, and with rapid technological advancements, digital peer support, such as the DigiPer mobile application, can be a feasible self-management tool for persons with SMI. The study aims to assess the feasibility of DigiPer for persons with SMI in the Norwegian community mental health service settings. METHODS AND ANALYSIS: This feasibili

Correlates of secondhand tobacco smoke exposure among persons with severe and persistent mental illness (SPMI) accessing community mental health services.

To examine the correlates of secondhand tobacco smoke (SHS) exposure in a sample of individuals with severe and persistent mental illness (SPMI) accessing community mental health services. Cross-sectional data were collected from a voluntary sample of 788 individuals with SPMI. Nonsmokers were more likely to be exposed on the street or at a bus stop, in a park or public place; whereas smokers were more likely to report SHS exposure where they lived and at their friend's homes. In a multivariate model, only number of acquaintances who smoked and the number of sources of SHS exposure remained si

"Making a homeless person even more homeless … it's a necessary evil": The health and safety complexities of service restrictions as perceived by emergency shelter staff.

Service restrictions involve people experiencing homelessness being banned from an emergency shelter in response to a violation of program policies. Their use can be a pathway into unsheltered homelessness and reliance on other institutional services. However, the prevalence of service restriction use and the perspectives of shelter staff toward the practice are unknown. Accordingly, this mixed methods study used a one-phase QUAL(quan) embedded design to examine the reasons for implementing service restrictions, as recorded by shelter staff in administrative data and their perceptions of the p

The mental health recovery movement and family therapy, part I: consumer-led reform of services to persons diagnosed with severe mental illness.

In 2004, the U.S. Department of Health and Human Services issued a consensus statement on mental health recovery based on the New Freedom Commission's recommendation that public mental health organizations adopt a "recovery" approach to severe and persistent mental illness, including services to those dually diagnosed with mental health and substance abuse issues. By formally adopting and promoting a recovery orientation to severe mental illness, the United States followed suit with other first-world nations that have also adopted this approach based on two decades of research by the World Hea

Disruption of Opioid Treatment Program Services Due to an Extreme Weather Event: An Example of Climate Change Effects on the Health of Persons Who Use Drugs.

Climate change and the opioid epidemic in combination may pose significant challenges for individuals with opioid use disorder due to potential disruptions in access to essential addiction treatment services caused by extreme weather events. Despite concerns over the escalating health impacts of climate change, limited research has documented and explored the vulnerability of patients enrolled in opioid treatment programs to disruptions caused by climate change and particularly extreme cold events. In this commentary, we describe the impact of a catastrophic flooding event during record-settin

Assessing the quality of the care offer for people with personality disorders in Italy: the QUADIM project. A multicentre research based on the database of use of Mental Health services.

BACKGROUND: Italy can be viewed as a laboratory to assess the quality of mental healthcare delivered in a community-oriented system, especially for severe mental disorders, such as personality disorders. Although initiatives based on clinical indicators for assessing the quality of mental healthcare have been developed by transnational-organisations, there is still no widespread practice of measuring the quality of care pathways delivered to patients with severe mental disorders in a community-oriented system, especially using administrative healthcare databases. The aim of the study is to eva

Clinical Stage Transitions in Persons Aged 12 to 25 Years Presenting to Early Intervention Mental Health Services With Anxiety, Mood, and Psychotic Disorders.

IMPORTANCE: The large contribution of psychiatric disorders to premature death and persistent disability among young people means that earlier identification and enhanced long-term care for those who are most at risk of developing life-threatening or chronic disorders is critical. Clinical staging as an adjunct to diagnosis to address emerging psychiatric disorders has been proposed for young people presenting for care; however, the longer-term utility of this system has not been established. OBJECTIVES: To determine the rates of transition from earlier to later stages of anxiety, mood, psych
