The words we work with that work on us: clinical paradigm and cumulative relational trauma.

This paper addresses a gap between analytic clinical theory and practice which emerges when examining the words we work with via textual and narrative research of case histories. Both subject matter and methodology fit with the remit of conceptual research in psychoanalysis, currently ranging from inductive to nomothetical approaches. Research of clinical language reveals an implicit account of human nature and the world which undergirds clinical practice. Based in the critical philosophy of the previous century, this is termed clinical paradigm. Such implicit views are induced rather than exp

Is stuttering a contraindication to psychotherapy?

This is a case report of a patient who stutters and was treated for problems of self-esteem with focused brief psychotherapy. The literature by and large is against the use of individual psychotherapy for neurotic problems in patients who stutter. The countertransference may account for the unwarranted pessimism in the literature about such patients. In this case the successful resolution of countertransference feelings facilitated the achievement of the goals of therapy. Further reports on such treatment attempts are warranted. Speech therapy may be facilitated by brief focused psychotherapy

Distinct temperament and character profiles in first onset vitiligo but not in alopecia areata.

Alopecia areata (AA) and vitiligo (V) are diseases that are correlated with psychiatric disorders before, during and after diagnosis. The Temperament and Character Inventory (TCI) is a well-established approach for investigating personality traits in various psychosomatic diseases. The aim of this study is to compare and investigate the differences in the TCI between patients with first onset AA, patients with V and healthy controls (HC). Participants in the study included 42 patients with first onset AA, 50 adult patients with V and 60 HC who had no history or diagnoses of psychiatric or derm

Pre-dementia Alzheimer's trials: overview.

A series of negative clinical trials of disease-modifying agents for Alzheimer's disease has increased pessimism regarding the prospects for important therapeutic advances. But limited efficacy may be attributed in part to the advanced degree of neurodegeneration present at the onset of dementia. To optimize the likelihood of success, it is essential to develop the methodology to allow testing of disease-modifying treatments at an early stage of pathology, when modulation of pathophysiologic mechanisms may yield major clinical benefits.

A Personalized, Transdiagnostic Smartphone Intervention (Mello) Targeting Repetitive Negative Thinking in Young People With Depression and Anxiety: Pilot Randomized Controlled Trial.

BACKGROUND: Repetitive negative thinking (RNT) is a key transdiagnostic mechanism underpinning depression and anxiety. Using "just-in-time adaptive interventions" via smartphones may disrupt RNT in real time, providing targeted and personalized intervention. OBJECTIVE: This pilot randomized controlled trial evaluates the feasibility, acceptability, and preliminary clinical outcomes and mechanisms of Mello-a fully automated, personalized, transdiagnostic, and mechanistic smartphone intervention targeting RNT in young people with depression and anxiety. METHODS: Participants with heightened de

The effect of positive and negative evaluation on state anxiety and repetitive negative thinking in social evaluative situations: An experimental study.

The Bivalent Fear of Evaluation (BFOE) Model suggests that fears of negative and positive evaluation are core cognitive vulnerability and maintenance factors for social anxiety disorder The aim of this study was to experimentally assess cognitive and emotional effects of positive and negative feedback in social anxiety. False positive or negative feedback (vs no feedback) was provided following a speech task to high and low socially anxious individuals to investigate impacts on state anxiety and repetitive negative thinking (N = 152, Mage = 22.13, 69 % female). Measures were completed prior to

The effect of psychological treatment on repetitive negative thinking in youth depression and anxiety: a meta-analysis and meta-regression.

BACKGROUND: Depression and anxiety are prevalent in youth populations and typically emerge during adolescence. Repetitive negative thinking (RNT) is a putative transdiagnostic mechanism with consistent associations with depression and anxiety. Targeting transdiagnostic processes like RNT for youth depression and anxiety may offer more targeted, personalised and effective treatment. METHODS: A meta-analysis was conducted to examine the effect of psychological treatments on RNT, depression and anxiety symptoms in young people with depression or anxiety, and a meta-regression to examine relation

Measurement Invariance of Disorder-Specific and Transdiagnostic Measures of Repetitive Negative Thinking.

BACKGROUND: Repetitive negative thinking is conceptualized to be a transdiagnostic process linked to the development and maintenance of psychopathology. Prior research distinguishes between disorder-specific exemplars (worry, rumination) and transdiagnostic measures of repetitive negative thinking with differences across disorders reported. However, establishing the measurement invariance of these measures is necessary to support meaningful comparisons across clinical groups. METHOD: Bayesian structural equation modelling was used to assess the approximate invariance of the Ruminative Respons

Toward a better understanding of the influence of repetitive negative thinking in alcohol use disorder: An examination of moderation effect of metacognitive beliefs and gender.

Repetitive negative thinking (RNT), the most studied forms of which are depressive ruminations and anxious worry, is a unique transdiagnostic process responsible for the development and maintenance of many mental disorders. Over the past decade, studies have shown that RNT could be involved in the development and maintenance of alcohol use disorder (AUD). However, to date, little is known about the factors that can influence this relationship, even though some theoretical and empirical arguments suggest that variables such as gender or metacognitive beliefs may determine the role of RNT in the

Emotional Awareness in Daily Life: Exploring Its Potential Role in Repetitive Thinking and Healthy Coping.

Cross-sectional research has found that emotional awareness, which is composed of one's emotional clarity and attention to emotions, is associated with psychopathology, emotion regulation, and coping. Not surprisingly then, emotional awareness is often a target of cognitive and behavioral therapies. However, little is known about what emotional awareness is related to in daily life, which would inform how and for whom emotional awareness training should be conducted in therapeutic settings. The goal of the current studies was to examine associations among facets of emotional awareness and repe

What Can Affect Competition Anxiety in Athletes? The Role of Self-Compassion and Repetitive Negative Thinking.

Identifying the correlates of competition anxiety is fundamental to athletes' well-being and performance. Self-compassion is gaining attention in sport psychology because of its associations with positive outcomes, while repetitive negative thinking (i.e. worry and rumination) is associated with negative conditions, such as anxiety. Building on previous evidence in the general population of associations between self-compassion, repetitive negative thinking, and trait anxiety, we investigated these relations in athletes of different sports. A total of 263 athletes (Mage = 23.72, SD = 6.97, 141

The neuroanatomical correlates of repetitive negative thinking: A systematic review.

Repetitive negative thinking (RNT) is a cognitive process characterised by intrusive, repetitive, and difficult-to-disengage-from negative thoughts. Heightened RNT levels are prevalent across clinical disorders and have been associated with ill-health (e.g. cardiovascular disease), even at lower, non-clinical levels. Identifying the neuroanatomical correlates of RNT could help characterise structural alterations that transcend diagnostic boundaries and further understanding of the pathogenesis of clinical disorders. We therefore conducted a systematic review to investigate associations between

Rumination as a transdiagnostic process: The role of rumination in relation to antisocial and borderline symptoms.

The present study examined the influence of several forms of repetitive negative thinking on antisocial and borderline symptoms to identify underlying mechanisms that may contribute to the presentation and cooccurrence of these symptoms. Participants (N = 403), completed the Personality Assessment Inventory, the Personality Diagnostic Questionnaire-4, and several measures of rumination/worry. Path analyses were conducted in which different forms of rumination/worry predicted antisocial and borderline symptoms across two personality disorder measures. Across both models tested, anger rumination

Associations Between Psychological Factors and Adherence to Health Behaviors After Percutaneous Coronary Intervention: The Role of Cardiac Rehabilitation.

BACKGROUND: Cardiac rehabilitation (CR) participation after percutaneous coronary intervention (PCI) for coronary heart disease lowers the disease burden and risk of recurrent cardiac events. Examining psychological factors may improve post-PCI health behavior adherence. PURPOSE: To determine whether psychological factors are associated with post-PCI health behavior adherence, and the role of CR participation. METHODS: Data from 1,682 patients (22.1% female, Mage = 64.0, SDage = 10.5 years) from the THORESCI cohort were included. Adjusted mixed models were used to examine associations betwee

Advancing Our Understanding of Protective Maternal Immunity as a Guide for Development of Vaccines To Reduce Congenital Cytomegalovirus Infections.

Human cytomegalovirus (HCMV) is the most common congenitally transmitted pathogen worldwide, impacting an estimated 1 million newborns annually. Congenital HCMV (cCMV) infection is a major global contributor to long-term neurologic deficits, including deafness, microcephaly, and neurodevelopmental delay, as well as to fetal loss and occasional infant mortality. Accordingly, design of a maternal vaccine to prevent cCMV continues to be a top public health priority. Nevertheless, we remain without a licensed vaccine. Maternal immunity provides partial protection, as the risk of vertical HCMV tran

Optimistic, pessimistic, realistic: Event-related potential evidence for how depressive symptoms influences expectation formation in the Human brain.

Recent research suggested a link between the prediction mechanism and depressive symptoms. While healthy people tend to maintain unrealistic optimism in the face of reality challenging their beliefs, depressed people show systematic pessimism. However, it remains unclear at which stage these individual differences in optimism/pessimism arise in the brain. In the current study we designed a simple gambling task with two difficulty levels, the easy game and the hard game. Participants were required to press one of four keys to gain a bonus signalled by a sinusoidal tone. For three of the four ke

Work engagement among breast cancer survivors and the referents: the importance of optimism and social resources at work.

INTRODUCTION: Despite a growing number of cancer survivors returning to work, little is known about their well-being at work and the work and personal characteristics related to it. The aim of the present study was to investigate personal resources (optimism and pessimism) and job-related resources (organizational climate, social support and avoidance behaviour) as antecedents of work engagement among female breast cancer survivors and their referents. METHODS: A community-based postal survey was conducted among female breast cancer survivors and their referents. The final study population co

Optimism, hostility, and adjustment in the first year of high school.

BACKGROUND: The period of transition to high school is marked by large variations in student adjustment patterns. Many students make successful adjustments but others do not and are at risk of developing hostile attitudes towards their school environment. AIMS: This study sought to investigate the role of optimism and pessimism in predicting the extent of hostility students express towards school. SAMPLE: The sample consisted of 102 students who completed questionnaires at the beginning of the school year, and also in the last term of their first year at high school. METHODS: Students respo

[Psychological predictors of rehabilitation efficiency of patients with radiculopathy after neurosurgical correction of compression diskogenic syndrome].

Sixty four osteochondrosis patients after neurosurgical correction of the compression syndrome were examined psychologically before and after the course of rehabilitation. It was established that considerable relief of pain in the course of physiotherapy was seen in patients who, before the treatment, had less severe impairment of the psychological status, no significant symptoms of hypochondria, anxiety, depression. Psychologically unfavourable predictors of low efficiency of physiotherapy were psychic tension, excitability, low stress resistance, pessimism, high anxiety, intrapersonality con

Use of percutaneous nephrostomy in malignant ureteric obstruction.

Seventeen patients with malignant disease underwent percutaneous nephrostomy to relieve renal failure secondary to ureteric obstruction. Renal function improved in 88%. The median survival time was 18 weeks and 58% left the primary hospital to return home or to a terminal care hospice. Minor complications occurred in 58%. The use of bilateral nephrostomy tubes conferred no greater benefit than a unilateral tube. Intervention in malignant ureteric obstruction may confer some benefit and should not necessarily be viewed with pessimism.
