Microendoscopic Surgery of Middle Ear and Petrous Bone: Benefits Analysis.

OBJECTIVES: Endoscopy has become routinely used in middle ear surgery. The aim of this study is to analyze where this tool may complement the traditional microscopic approach. This is a retrospective study done in single tertiary hospital. METHODS: We reviewed 342 middle ear/petrous bone surgical procedures performed between 2005 and 2015. Only cases in which both microscopic and endoscope-assisted techniques were used have been included. Sixty-six patients received this double-technique surgery: 51 (77.2%) had middle ear/mastoid and petrous bone cholesteatomas (46 middle ear and mastoid and

[Uncommon tumors of the petrous bone. 4 cases (author's transl)].

Four cases of uncommon tumours of the petrous bone treated by surgery alone or combined with radiotherapy are reported. There was one case embryonic sarcoma in a 3-year-old female child and one case each of chondroid chordoma, isolated plasmocytoma and malignant fibroid chondrosarcoma or histiocytoma with giant cells. The signal symptoms (pseudo-petrositis, isolated paralysis of a vocal cord, sudden deafness) may be misleading. The most helpful diagnostic methods are frontal tomography and CT scanning.

[Surgical management of petrous bone cholesteatoma and facial nerve protection].

Objective: To discuss the therapeutic scheme of petrous bone cholesteatoma(PBC) and the technique of facial nerve reconstruction. Method: The data of 28 patients who underwent surgery for PBC in our center were analyzed retrospectively. All patients were diagnosed radiologically with PBCs and reconfirmed pathologically after surgery. The surgical approach was discussed basing Sanna's classification of PBCs, and the facial nerve outcomes were analyzed moreover. Result: PBC cases 11 were supralabyrinthine, 4 infralabyrinthine, 3 infralabyrinthine-apical, 10 massive and none apical. The facial ne

Endoscope-Assisted Surgery for Petrous Bone Cholesteatoma with Hearing Preservation.

OBJECTIVES: Petrous bone cholesteatoma is a rare pathologic entity and may be a difficult surgical challenge because of potential involvement of the facial nerve, carotid artery, dura mater, otic capsule and superior petrosal or lateral sinus. The objective of this article is to present the endoscope-assisted surgery for petrous bone cholesteatoma. MATERIALS AND METHODS: Eight patients (nine ears) who underwent endoscope-assisted petrous bone surgery for cholesteatoma. Pure tone audiogram, magnetic resonance imaging were performed at preoperatively, and at approximately 12 months postoperativ

Surgical Strategy and Facial Nerve Outcomes in Petrous Bone Cholesteatoma.

OBJECTIVE: To review the classification and management of petrous bone cholesteatomas (PBCs) at our center and the outcomes of facial nerve (FN) management in these lesions. METHODS: This was a retrospective study. The setting was a quaternary referral center for skull base pathology in Italy. A total of 200 patients with 201 PBCs were included in the study. All patients diagnosed radiologically with PBCs were classified according to the Sanna classification. All patients were surgically treated and followed up with radiology. The main outcome measures - classification of PBCs, the surgical a

Meningiomas of the posterior petrous bone: functional outcome after microsurgery.

OBJECT: The aim of this study was to analyze a subgroup of patients harboring cerebellopontine angle meningiomas originating from the posterior petrous bone in regard to clinical presentation, surgical anatomy, complications, and long-term functional postoperative results. METHODS: Data in a series of 51 patients with meningiomas of the posterior petrous bone who had undergone microsurgical treatment at the authors' institution between 1989 and 2002 were retrospectively reviewed. The patient population consisted of 46 women and five men with a mean age of 53 years (range 22-70 years). The mai

Extensive Temporal Bone Necrosis in Petrous Bone Cholesteatoma Complicated by Actinomycosis: A Case Report.

BACKGROUND Temporal bone necrosis is a rare condition, most commonly caused by radiation therapy, bisphosphonate treatment, and osteomyelitis. Cholesteatoma can contribute to temporal bone necrosis, particularly in the presence of secondary ear infections. We report a case of extensive temporal bone necrosis with intracranial involvement in a patient with mastoid and petrous apex cholesteatoma, further complicated by actinomycosis. CASE REPORT A 47-year-old woman presented with persistent otorrhea and right-sided conductive hearing loss. Imaging demonstrated diffuse necrotic tissue extending f

Ultralow-dose CT of the petrous bone using iterative reconstruction technique, tin filter and high resolution detectors allows an adequate assessment of the petrous bone structures.

OBJECTIVES: To assess image quality and radiation dose in computed tomography (CT) studies of the petrous bone done with a scanner using a tin filter, high-resolution detectors, and iterative reconstruction, and to compare versus in studies done with another scanner without a tin filter using filtered back projection reconstruction. MATERIAL AND METHODS: Thirty two patients (group 1) were acquired with an ultra-low dose CT (32-MDCT, 130kV, tin filter and iterative reconstruction). Images and radiation doses were compared to 36 patients (group 2) acquired in a 16-MDCT (120kV and filtered back-

Decision making for the surgical approach of posterior petrous bone meningiomas.

OBJECT: The authors undertook this study to examine the surgical approaches used to treat posterior petrous bone meningiomas at a single institution and retrospectively evaluate their surgical strategy based on a previously published classification. METHODS: Cases in which craniotomies were performed to treat posterior petrous bone meningiomas between 2002 and 2010 were retrospectively reviewed. Data were examined from 57 patients who were treated for 59 tumors. The tumors were classified into 3 types according to the location of their primary dural attachment: Type A, located around the poru

A case of petrous bone myxoid chondrosarcoma associated with cerebellar hemorrhage.

We report a rare case in which a myxoid chondrosarcoma originated from the petrous bone and invaded the cerebellar hemisphere with hemorrhage. Neuroimaging showed the characteristic feature of multiple small cysts along the solid tumor, and the cystic formation was confirmed as a mucoid secretion by Alcian blue staining and electron microscopic examination. This tumor recurred following partial removal and stereotaxic radiosurgery.

Rupture of an internal carotid artery aneurysm in the petrous temporal bone.

A case of otorrhagia following the rupture of an aneurysm of the intrapetrous portion of the internal carotid artery is presented. The aneurysm was successfully treated by balloon occlusion of the parent artery. The problems associated with diagnosis and clinical management of these lesions is discussed and possible solutions suggested.

[Mondini-type petrous bone dysplasia as a cause of spontaneous otoliquorrhea].

A case of fistula formation in a congenitally deformed petrous bone is described. This case demonstrates the diagnostic problem of deafness with spontaneous cerebrospinal fluid otorrhoea causing recurrent meningitis. Methods of diagnosis and therapy are discussed and the value of hypocycloidal polytomography is stressed.
