Gender confirmation surgery has a crucial role among transgender individuals. Phalloplasty is a procedure that uses flaps for phallic shaft creation. Flaps can be classified in free flaps or pedicle flaps and can be obtained from different donor sites such as forearm, thigh, abdomen, groin, and leg, and upper back. Penile reconstruction remains a difficult surgical problem. To produce a phallus capable of erection and with a water-tight urethra to the tip has not been solved satisfactorily even by the introduction of free flap transfers. We present a method used in four cases, three transsexuals and one pseudohermaphrodite, in which a phallus was successfully constructed using the deep inferior epigastric flap.

An aesthetically appealing neophallus with an urethral meatus at its tip is one of the goals of phalloplasty in female-to-male transsexuals. From July 1993 to December 1996, 20 cases of female-to-male transsexuals who had received sculpting of glans in four different ways were enrolled. Their photographs were collected and were scored by independent surgical and non-surgical assessors. Three weeks after radial forearm free flap phalloplasty, a 28-year-old female-to-male transsexual presented with ischaemia of the neophallus. A salvage procedure was started 7 hours after onset of ischemia. Thrombectomy of the arterial pedicle did not result in venous return of the arterial inflow. Perfusion of the flap with streptokinase restored venous return. Flap perfusion was confirmed by intraoperative direct angiography of the neophallus. Six months after this salvage procedure, the flap is doing well and tactile sensibility has occurred.

Phalloplasty following penectomy for penile cancer presents a complex reconstructive challenge, requiring restoration of urinary and sexual function while addressing anatomical and psychological sequelae. Unlike gender-affirming phalloplasty, this procedure is complicated by previous surgery and potential anatomical deficits. However, limited data exist to guide reconstruction in this cohort. This narrative review summarises available evidence on phalloplasty post-penectomy. A systematic search identified six relevant studies, including 48 patients, with follow-up ranging from 1 to 150 months.
