[Obstructive sleep apnea syndrome and the upper airway muscles].

Pharyngeal muscles are prone to distorsion during inspiratory negative pressure. Some pharyngeal muscles, called pharyngeal dilatators, exhibit tonic and/or phasic inspiratory activity. At the velar level, tensor palatini, glossopharyngeus, palatopharyngeus and musculus uvula drive the airflow towards nasal or buccal breathing. Genioglossus and geniohyoid muscles exert a forward propulsion to the tongue. The contraction of the dilatators muscles precedes the diaphram contraction. These muscles show a poor endurance. In obstructive sleep apnea syndrome, they contract during unfavorable metaboli

Contributions of the glossopharyngeal nerve and the pharyngeal branch of the vagus nerve to the swallowing process in dogs.

The separate contributions of the glossopharyngeal nerve and the pharyngeal branch of the vagus nerve to the innervation of the pharyngeal muscles were studied first in 10 canine cadavers by dissection of the pharyngeal plexus and the pharyngeal muscles. In 10 other dogs, the parent trunks and 1st division of the glossopharyngeal nerve and the pharyngeal branch of the vagus nerve were electrically stimulated. The evoked stimulation potentials were recorded from the stylopharyngeal, hyopharyngeal, thyropharyngeal, and cricopharyngeal muscles. One of the parent trunks was then transected, and th

Functional Anatomy of the Pharynx of Glycera tridactyla Schmarda, 1861 (Annelida: Glyceriformia: Glyceridae).

The pharyngeal muscular system of polychaetes is highly complex and shows remarkable variation in structure and function among taxa. In this study, the functional anatomy of the pharynx of G. tridactyla was investigated using micro-computed tomography (micro-CT). Sixteen Glycera specimens were imaged in different pharyngeal positions to examine the roles of key muscular structures in pharyngeal movement. The high-resolution imaging provided by micro-CT allowed visualisation of the pharyngeal muscles in their original topography, revealing essential components for pharyngeal function, including

New Insights into the Diversity of Branchiomeric Muscle Development: Genetic Programs and Differentiation.

Branchiomeric skeletal muscles are a subset of head muscles originating from skeletal muscle progenitor cells in the mesodermal core of pharyngeal arches. These muscles are involved in facial expression, mastication, and function of the larynx and pharynx. Branchiomeric muscles have been the focus of many studies over the years due to their distinct developmental programs and common origin with the heart muscle. A prerequisite for investigating these muscles' properties and therapeutic potential is understanding their genetic program and differentiation. In contrast to our understanding of how

Dual muscarinic and nicotinic action on a motor program in Drosophila.

The effect of cholinergic agonists and antagonists on the central pattern generator of the pharyngeal muscles has been studied in third instar larvae of Drosophila. The pharyngeal muscles are a group of rhythmically active fibers involved in feeding. Bath application of the cholinergic agonists carbachol, muscarine, pilocarpine, and acetylcholine (ACh) to a semiintact preparation including the pharyngeal muscles and the central nervous system (CNS), initiated long-lasting endogenous-like bursting activity in the muscles. The muscarinic antagonists, atropine and scopolamine, blocked these respo

Effects of drugs and electrical field stimulation on isolated muscle strips from rabbit pharyngoesophageal segment.

Muscle activity of isolated muscle strips from the middle pharyngeal constrictor, the inferior pharyngeal constrictor, the cricopharyngeal muscle, and the cervical esophagus of the rabbit was studied in organ baths in response to drugs and electrical field stimulation. These muscles of the pharyngoesophageal segment seem to lack an autonomic adrenergic and cholinergic innervation, as determined by responses to specific drugs. The nerve-evoked contractile responses of the two lower muscles of the segment were almost completely abolished by the curarelike drug gallamine, suggesting a somatic cho

Mechanisms of airway protection during retching, vomiting, and swallowing.

We investigated the mechanisms of airway protection and bolus transport during retching and vomiting by recording responses of the pharyngeal, laryngeal, and hyoid muscles and comparing them with responses during swallowing and responses of the gastrointestinal tract. Five dogs were chronically instrumented with electrodes on the striated muscles and strain gauges on smooth muscles. Retching and vomiting were stimulated by apomorphine (5-10 ug/kg iv). During retching, the hyoid and thyroid descending and laryngeal abductor muscles were activated; between retches, the hyoid, thyroid, and pharyn

Motor control of the pharyngeal musculature and implications for the pathogenesis of obstructive sleep apnea.

Obstructive sleep apnea is a common breathing problem that results in recurrent episodes of nighttime hypoxemia, hypercapnia, bradytachycardia, and hypertension, as well as sleep disturbance and daytime hypersomnolence. The obstruction is located in the oropharynx and is caused by hypotonia of the pharyngeal dilator muscles. In this paper, the various mechanisms affecting motor output to the upper airway muscles are reviewed. In particular, the respiratory function of the pharyngeal dilator muscles, the various reflex mechanisms underlying their control, and the effects of sleep on these mecha

Is the pharynx a muscular hydrostat?

Failure to maintain the patency of the pharyngeal airway during sleep is central to the pathogenesis of obstructive sleep apnoea (OSA). This failure is hypothesised to be due to the combination of a small pharyngeal airway and inadequate state-dependent neuro-mechanical control. Little is known of how the pharyngeal muscles function in an integrated function to alter the size and shape of the pharyngeal airway. We hypothesise that the muscles of the pharynx function as a muscular hydrostat. Muscular hydrostats are organs that are composed almost entirely of muscle, with a complex muscular arra

Evaluation of velopharyngeal function using high-speed cine-magnetic resonance imaging based on T2-weighted sequences: a preliminary study.

The objective was to introduce a new technique for visualizing the three-dimensional (3D) movements of velopharyngeal-related muscles using high-speed cine-magnetic resonance imaging (MRI) based on T2-weighted sequences. The evaluation of phonation- and water swallowing-related events was performed in 11 healthy subjects. Specifically, whether cine-MRI could precisely visualize normal velopharyngeal function during these two events was examined. The 3D movements of the soft palate, superior pharyngeal constrictor muscles, and levator veli palatini muscles were visualized in all 11 subjects. A

Nerve fiber analysis of the lingual branch of the glossopharyngeal nerve in human adult subjects.

PURPOSE: Fibers of the glossopharyngeal part of the superior constrictor muscle are connected with fibers of the transverse lingual muscle, forming a ring of muscle at the base of the tongue. This group of muscles constrict the midpharyngeal cavity during retrusive movement of the tongue. The purpose of this study is to identify the contribution of the lingual branch of the glossopharyngeal nerve to the neuro-motor control of three muscles: the glossopharyngeal part of the superior pharyngeal constrictor muscle, the palatopharyngeal and the palatoglossus muscles. METHODS: Six en bloc samples

Gross anatomical observations of attachments of the middle pharyngeal constrictor.

The pharyngeal muscles overlap each other and some of their parts have different areas of origin. Such arrangements make the interrelationships among pharyngeal muscles complicated. This study investigated the attachments of the middle constrictor to clarify its configuration and re-examine its functions. The gross anatomies of the pharyngeal and neighboring muscles were examined in 41 cadavers. The middle constrictor arose from the stylohyoid ligament and the hyoid bone, and its fibers were divided into three overlapping groups. The anterosuperior group ascended posterosuperiorly from the lig

Cardiopharyngeal mesoderm origins of musculoskeletal and connective tissues in the mammalian pharynx.

Cardiopharyngeal mesoderm (CPM) gives rise to muscles of the head and heart. Using genetic lineage analysis in mice, we show that CPM develops into a broad range of pharyngeal structures and cell types encompassing musculoskeletal and connective tissues. We demonstrate that CPM contributes to medial pharyngeal skeletal and connective tissues associated with both branchiomeric and somite-derived neck muscles. CPM and neural crest cells (NCC) make complementary mediolateral contributions to pharyngeal structures, in a distribution established in the early embryo. We further show that biallelic e

Morphologic characteristics of palatopharyngeal muscle.

In an effort to clarify the morphologic characteristics of the palatopharyngeal muscle, we examined its origin, insertion, and positional relationship with other muscles. We found that the origin of the palatopharyngeal muscle was both the oral and the nasal side of the soft palate; it was also attached to both the palatal aponeurosis and the soft palate median. However, in some cases the muscle originated on the nasal side was lacked. When the palatopharyngeal muscle originated from both the oral and the nasal side, it traveled through its insertion via the levator muscle of the palatine velu

Nerve supply to the soft palate muscles with special reference to the distribution of the lesser palatine nerve.

OBJECTIVE: Descriptions of the innervation of the soft palate muscles in previous studies have varied according to the author. In the present study, distribution of the lesser palatine nerve, through which motor fibers of the facial nerve are considered to reach soft palate muscles, and that of the pharyngeal plexus in the soft palate were investigated in order to reexamine the innervation of the soft palate muscles according to anatomical evidence. RESULTS: Observations suggested that the levator veli palatini and palatopharyngeus were doubly innervated by branches of the lesser palatine ner

[The clinical picture and diagnosis of the pharyngofacial form of myasthenia].

In the clinic, 564 patients with the different forms of myasthenia were operated on. A pharyngo-facial form of myasthenia was revealed in 65 (11.5%) of them. There are the following clinical peculiarities of a pharyngo-facial form of myasthenia: involvement of the pharyngeal and facial muscles, impaired phonation, jerky speech, sensation of the tongue heaviness. The direction of process generalization is commonly from the centre towards periphery, towards the skeletal muscles. Involvement of the muscles of the soft palate, epiglottis causes saliva and pharyngeal secret flowing into the bronchi

A new computed tomography-based approach to quantify swallowing muscle volume by measuring tongue muscle area in a single slice.

BACKGROUND: Measuring the swallowing muscle mass with volume measurements is complex and time intensive; therefore, it is not used in clinical practice. However, it can be clinically relevant, for instance, in the case of sarcopenic dysphagia. The aim of the study was to develop a feasible and clinically applicable method to measure swallowing muscle mass. METHODS: Data from 10 head and neck cancer patients were collected from the Oncological Life Study data-biobank of the University Medical Center Groningen. The pharyngeal constrictor, genioglossus, mylohyoid and geniohyoid complex muscles,
