Navigating communication barriers in phlebotomy: Strategies and ethical tensions in routine care.

OBJECTIVE: The emotional and ethical experiences of Medical Laboratory Assistants (MLAs) during challenging patient phlebotomy encounters is largely unknown. This study identified verbal and non-verbal strategies used by MLAs and explored how they navigate systemic and interpersonal pressures during patient resistance. METHODS: We held four online focus groups with 12 MLAs working in hospital and community settings in Alberta, Canada. Transcripts were analyzed using a combined inductive and deductive thematic analysis guided by the Framework method. RESULTS: Participants described using plai

Improving the Quality of Venous Blood Sampling Procedure (Phlebotomy): Avoiding Tourniquet Use.

Background  Phlebotomy guidelines discourage tourniquet use whenever possible. We assessed phlebotomists' capability of not using the tourniquet in venous blood sampling, hypothesizing it to be equal to 50% of the patients attended, and identifying the most frequent venipuncture site. Materials and Methods  We assigned two phlebotomists of the same age (41 years) and experience (20 years) to record 10 phlebotomy days, the first with prioritized and the latter with nonprioritized patients. Each acquired daily data for the number of attended patients, age, gender, frequency of nontourniquet usag

[Quantitative liver function tests in porphyria cutanea tarda: improvement under therapeutic phlebotomy].

Quantitative liver function was assessed before and after therapy in 4 patients with porphyria cutanea tarda. Three patients underwent phlebotomy, which produced clinical and biochemical remission. Concomitantly, functional liver cell mass, as measured by galactose elimination capacity, normalized. In the last patient, treated with chloroquine, liver function did not improve during therapy. The aminopyrine breath test, a measure of microsomal function, was inconclusive. These results show that decreased hepatic function in porphyria cutanea tarda is reversible and emphasize the potential role

Rheopheresis, but Not Phlebotomy, Improves Cerebral Vascular Response to Hypercapnia and Neuronal Activation.

Background/Objectives: Several therapeutic approaches, including phlebotomy and rheopheresis, are used to improve hemorheological parameters. While the effects of phlebotomy on cerebral circulation have been described, the impact of rheopheresis on cerebral hemodynamics remains poorly understood. This study primarily aimed to evaluate the within-group effects of phlebotomy and rheopheresis on cerebral blood flow velocity changes evoked by neuronal activation and on cerebrovascular reactivity in patients with elevated hematocrit or hyperviscosity, respectively. Methods: In our present study, we

Treatment of hemodialysis-related porphyria cutanea tarda with small repeated phlebotomies.

Phlebotomy is generally thought to be contraindicated in porphyria cutanea tarda (PTC) associated with hemodialysis for the anemia which is often present in uremic patients. We report a patient with a severe form of hemodialysis-related PTC and biochemical parameters suggestive of iron overload in whom treatment with small repeated phlebotomies was well tolerated without any significant worsening of the anemia and gave marked biochemical improvement and complete clinical remission.

Physiologic effects of hyperventilation and phlebotomy in baboons: systemic and cerebral oxygen extraction.

Eighteen anesthetized baboons were studied to determine the effects of passive hyperventilation and phlebotomy on oxygen transport. After 1 hour of hyperventilation a significant increase in the red cell affinity for oxygen occurred in vivo. This was not associated with any significant changes in cardiac output, oxygen consumption, or in lactic acid production. There was a 40% decrease in cerebral blood flow, a 10 mm Hg decrease in the pulmonary artery Po2 level, and a 17 mm Hg decrease in the jugular venous Po2 level. After 1 hour of hyperventilation, the plasma inorganic phosphorus level dec

Factors associated with phlebotomy failure in an Alzheimer's Disease Research Center cohort.

INTRODUCTION: Venous blood collection through phlebotomy is critical for biomarker development and genetics research on Alzheimer's disease (AD) and related dementias. In this research setting, phlebotomy is often performed by clinical research coordinators (CRCs) rather than professional nurses or phlebotomists. Even though phlebotomy in older adults with cognitive impairment can be empirically challenging, data on the factors associated with phlebotomy failure in this special population are lacking. We addressed this question in the Massachusetts Alzheimer's Disease Research Center (MADRC) L

Diagnosis and management of hereditary hemochromatosis: lifestyle modification, phlebotomy, and blood donation.

The term hemochromatosis refers to a group of genetic disorders characterized by hepcidin insufficiency in the context of normal erythropoiesis, iron hyperabsorption, and expansion of the plasma iron pool with increased transferrin saturation, the diagnostic hallmark of the disease. This results in the formation of toxic non-transferrin-bound iron, which ultimately accumulates in multiple organs, including the liver, heart, endocrine glands, and joints. The most common form is HFE-hemochromatosis (HFE-H) due to p.Cys282Tyr (C282Y) homozygosity, present in nearly 1 in 200 people of Northern Eur

Phlebotomy-induced iron deficiency attenuates the pulmonary toxicity of paraquat in mice.

Phlebotomy is an effective method in the prevention and treatment of some poisonings, among which iron deficiency is a well-known consequence. Given the role of iron in paraquat (PQ) toxicity, the present study investigated the effectiveness of phlebotomy in PQ pulmonary toxicity. After conducting preliminary studies, the duration time of phlebotomy was set to be seven days. Then, the mice were divided into nine separate groups. Groups 1-3 received a single dose of normal saline, and 5 and 10 mg/kg of PQ, respectively, and phlebotomy was not performed on them (NPG status). The animals in group

Aceruloplasminemia with Abnormal Compound Heterozygous Mutations Developed Neurological Dysfunction during Phlebotomy Therapy.

Aceruloplasminemia is an autosomal recessive inherited disorder caused by ceruloplasmin gene mutations. The loss of ferroxidase activity of ceruloplasmin due to gene mutations causes a disturbance in cellular iron transport. We herein describe a patient with aceruloplasminemia, who presented with diabetes mellitus that was treated by insulin injections, liver hemosiderosis treated by phlebotomy therapy, and neurological impairment. A genetic analysis of the ceruloplasmin gene revealed novel compound heterozygous mutations of c.1286_1290insTATAC in exon 7 and c.2185delC in exon 12. This abnorma

Platelet count increase following phlebotomy in iron overloaded patients with liver cirrhosis.

Thrombocytopenia is a frequent hematological complication in patients with liver cirrhosis, but its pathogenesis is not clearly understood. We evaluated the effect of iron depletion by phlebotomy on platelet count in 62 consecutive iron overloaded patients with liver cirrhosis and thrombocytopenia. After a median follow-up of 30.2 months we observed a significant increase of platelet count in all patients (from mean baseline levels of 110.1 up to 168.22109/l at the end of follow-up, P<0.001) with platelet count normalization in 42 of them (67.7%). In addition, we observed a significant improve

Influence of phlebotomy treatment on abnormal hypothalamic-pituitary function in genetic hemochromatosis.

To test the hypothesis that deficiencies in hypothalamic-pituitary function in genetic hemochromatosis result from cellular injury by iron deposits, we conducted provocative tests in 11 men with genetic hemochromatosis before and after iron depletion by serial phlebotomy and in 10 control subjects. We gave combination intravenous injections of insulin (0.15 U/kg), luteinizing hormone releasing hormone (LHRH, 100 micrograms), and thyrotropin releasing hormone (400 micrograms) and then measured plasma glucose, growth hormone, corticosteroids, follicle-stimulating hormone, luteinizing hormone, pr

ACQUIRED PORPHYRIA CUTANEA TARDA: REPORT OF A CASE SUCCESSFULLY TREATED BY PHLEBOTOMY.

CURRENTLY, THE PORPHYRIAS ARE CLASSIFIED IN FOUR MAIN GROUPS: congenital porphyria, acute intermittent porphyria, porphyria cutanea tarda hereditaria, and porphyria cutanea tarda symptomatica. The acquired form of porphyria (porphyria cutanea tarda symptomatica) occurs in older males and is nearly always associated with chronic alcoholism and hepatic cirrhosis. The main clinical changes are dermatological, with excessive skin fragility and photosensitivity resulting in erosions and bullae. Biochemically, high levels of uroporphyrin are found in the urine and stools. Treatment to date has been

The Resolution of a Cerebrovascular Accident With Phlebotomy in a Hemodialysis Patient With Sickle Cell Disease.

Anemia is commonly observed in patients with end-stage renal disease (ESRD) on maintenance hemodialysis (HD) and can be quite severe, particularly when there is an additional comorbidity. With the use of erythropoietin-stimulating agents (ESAs), anemia is effectively treated, but the complete normalization of hemoglobin is not recommended since these agents increase the risk of thrombosis. With improvements in the therapy of sickle cell disease (SCD), patients now survive longer and may more frequently reach end-stage renal disease and require renal replacement therapy. Their anemia can be sev

Effects of phlebotomy on the growth of ferric nitrilotriacetate-induced renal cell carcinoma.

The ferric nitrilotriacetate-induced carcinogenesis model is unique in that reactive oxygen species-free radicals are involved in the carcinogenic process. But the effects of iron-withdrawal in the progression of renal cell carcinoma are not well understood. We performed repeated phlebotomies on animals that had been administered ferric nitrilotriacetate in the initiation stage of renal cell carcinoma (phlebotomy group), and compared the development of renal tumors with those not receiving repeated phlebotomies (non-phlebotomy group). Ferric nitrilotriacetate-treated male Wistar rats were rand

The response of various hematologic parameters in the young bovine subjected to multiple phlebotomies.

Specific guidelines for the optimal collection of blood from calves maintained as donors do not exist. This investigation was conducted to document erythrocytic changes in calves subjected to controlled blood loss. Two groups of Holstein calves ranging in age from 2 to 5 months were compared. Six animals were designated controls and six were subjected to a series of three blood collections over an 8 day period. Each collection was roughly 50% of the total circulating blood volume (30 ml/kg of body weight). Hematologic values were compared for 18 days. All calves survived the study. There was n
