Social phobias: behavioural and pharmacotherapy.

Five to 10% of the general population suffers from symptoms of an anxiety disorder. Social phobias, while less common than panic disorder, agoraphobia or simple phobias, are just as debilitating. Patients present with somatic, behavioural, mood and cognitive disturbances, of which unrecognized social isolation, depression, loss of employment, and drug and alcohol abuse can be the result. A symptomatic approach can be implemented through the use of education, insight, support, behaviour therapy, cognitive therapy, and pharmacological intervention.

Alprazolam in the treatment of social phobia.

Pharmacological treatment of social phobia has not been extensively studied. Recent reports suggest that social phobia may be treated with monoamine oxidase inhibitors, tricyclic antidepressants, beta-blockers, or clonidine. The authors describe four patients with social phobia who responded moderately to markedly well when treated with the triazolobenzodiazepine alprazolam. Patients with social phobia and patients with agoraphobia with panic attacks differ in the focus of anxiety, sensitivity to lactate infusion, and the pattern of symptoms during anxiety episodes. However, there is significa

Steroid phobia on social media platforms.

Topical corticosteroids (TCS) are the most commonly prescribed treatment for children with atopic dermatitis and are supported by the American Academy of Dermatology (AAD) atopic dermatitis treatment guidelines with level I strength A evidence; however, fear regarding their use, coined "steroid phobia," is widespread. In this study, we analyzed steroid phobia-related content on popular social media platforms. We found much of this content consists of patients describing negative personal experiences with TCS and subsequently discouraging viewer use. We conclude that social media may contribute

State personality disorder in social phobia.

To examine the effects of state on personality disorder characteristics, we compared individuals with social phobia before and after psychological intervention. Administration of the Personality Disorder Questionnaire (PDQ-4) before and after treatment allowed for the identification of three groups of patients: (1) individuals who showed elevated PDQ-4 scores but little changes from intake to post treatment (Trait PD group; n = 28); (2) individuals who showed a decrease in PDQ-4 scores from intake to post treatment (State group; n = 33); and (3) a group with no significant personality disorder

Social phobia: prevalence and diagnostic threshold.

This article reviews the literature on the prevalence and demographic features of social phobia in both community and general medical settings. The age at onset of social phobia is examined, as are comorbid conditions. Important differences between social phobia as it appears in the community and in primary care settings are explored. We conclude that social phobia is common and associated with significant impairment in a number of life areas. We discuss the diagnostic threshold of social phobia and potential difficulties in differentiating this disorder from other mental disorders.

Negative interpretation bias in social phobia.

We examined the hypothesis that, compared to non-anxious controls (NACs) and individuals with other anxiety disorders (e.g., individuals with obsessive compulsive disorder; OCs), individuals with generalized social phobia (GSPs) would tend to interpret ambiguous social scenarios as negative when provided with various alternative interpretations. Participants were presented with 22 ambiguous scenarios each followed by three possible interpretations: positive, negative, and neutral. Fifteen scenarios were socially relevant and the remaining 7 were nonsocially relevant. Participants were asked to

Autonomic responsivity in generalized social phobia.

To determine whether patients with generalized social phobia exhibit evidence of abnormal autonomic nervous system (ANS) functioning, 15 non-depressed, medication-free subjects with DSM-IV social phobia (generalized type) and 15 healthy control subjects participated in a series of autonomic function tests. Generalized social phobics exhibited increased blood pressure responsivity to Valsalva and exaggerated vagal withdrawal in response to isometric exercise, but normal cardiovascular responsivity to all other tasks. Plasma norepinephrine and epinephrine levels were also normal. Studies with la

Social phobia: biological aspects and pharmacotherapy.

1. Social phobia is one of the anxiety disorders that until recently, had not been thoroughly investigated. 2. Social phobia is a relatively common anxiety disorder that appears to have a genetic basis. 3. There are certain physiological aspects of social phobia that separate it from the other anxiety disorders. 4. Support for a dopaminergic abnormality related to social phobia is supported by investigation studies and pharmacotherapy. 5. There are a number of studies reporting success in the treatment of social phobia with medications.

Depersonalization/derealization during acute social stress in social phobia.

The present study aimed at investigating how frequently and intensely depersonalization/derealization symptoms occur during a stressful performance situation in social phobia patients vs. healthy controls, as well as testing hypotheses about the psychological predictors and consequences of such symptoms. N=54 patients with social phobia and N=34 control participants without mental disorders were examined prior to, during, and after a standardized social performance situation (Trier Social Stress Test, TSST). An adapted version of the Cambridge Depersonalization Scale was applied along with mea

Social Self-Reappraisal Therapy for Social Phobia: Preliminary Findings.

The recent psychopathology literature suggests that individuals with social phobia overestimate social standards and are deficient in setting and attaining social goals, have a negative perception of themselves as social objects and show heightened self-focused attention when confronted with social threat. They further overestimate the potential cost of a social encounter, experience their anxiety as uncontrollable and visible to others, view their social skills as inadequate, rely on safety behaviors and avoidance strategies to control their anxiety, and engage in post-event rumination. Profile of a large sample of patients with social phobia: comparison between generalized and specific social phobia.

This study examines a large cohort of subjects with social phobia, as part of a larger naturalistic and longitudinal study of 711 subjects with anxiety disorders. We focused on 176 subjects who were in an episode of social phobia at intake. We were particularly interested in evaluating the diagnostic distinction between generalized and specific social phobia. We compared these two groups along demographic characteristics, comorbidities, psychosocial functioning (health, role functioning, social functioning, and emotional functioning) and global assessment scores. We found that generalized soci

Assessing reliable and clinically significant change in social phobia: validity of the social phobia and anxiety inventory.

The ability of the Social Phobia and Anxiety Inventory (SPAI) to measure change as a function of treatment for social phobia was assessed. In addition to determining treatment sensitivity of the SPAI, changes over the course of treatment were examined to determine if they were reliable and clinically significant. The results indicated that although a number of measures showed statistically significant change, only on the SPAI was that change reliable and clinically significant. The use of outcome measures that are reliable and clinically significant for social phobia as well as other disorders

Frequency and comorbidity of social phobia and social fears in adolescents.

This report presents findings on the frequency, comorbidity and psychosocial impairment of social phobia and social fears among 1035 adolescents, aged 12-17 years. The adolescents were randomly selected from 36 schools in the province of Bremen, Germany. Social phobia and other psychiatric disorders were coded based on DSM-IV criteria using the computerized Munich version of the Composite International Diagnostic Interview. Seventeen (1.6%) of the adolescents met the DSM-IV criteria for social phobia sometimes in their life. More girls than boys received the diagnosis of social phobia and the
