Patient safety in the rehabilitation of children with spinal cord injuries, spina bifida, neuromuscular disorders, and amputations.

Pediatric patient safety continues to challenge both pediatricians and pediatric physiatrists. While there is a trend toward developing general patient safety initiatives, there is little research on pediatric patient safety. This article identifies major areas of general safety risk, with a focus on timely diagnosis and care coordination to prevent secondary complications that compromise health, function, and quality of life in pediatric neuromuscular disease, spinal cord disorders, and amputation.

[Humero-scapular periarthritis--a diagnosis which does not need to be used].

The periarthritis humeroscapularis (PHS) diagnosis has been used for over a century. In our country, the term PHS is customary and unavoidable in everyday practice. It has been used by everyone, from general practitioners to specialists--physiatrists, radiologists, and orthopaedists. The purpose of this study is to show the inappropriateness of the PHS diagnosis and to point to other clinical conditions with a clear anatomical structure and adequate treatment method.

The Incidence of Physiatry-Relevant Complications in Trauma Patients Admitted to an Urban Canadian Trauma Center.

The objective of this study was to describe the incidence of complications in trauma patients that could be prevented, diagnosed, or managed by a consulting acute care physiatrist. Demographic and complication data were extracted by chart review of adult trauma patients admitted to a Canadian academic trauma center. Subjects were included if they had a diagnosis of traumatic brain injury, spinal cord injury, or multiple injuries resulting in an Injury Severity Score greater than 15. Means and standard deviations were calculated for continuous variables and frequencies for categorical data. Sec

Diagnosis of trochanteric bursitis versus femoral neck stress fracture.

This case report describes a 46-year-old male runner who reported the sudden onset of right hip pain. A diagnosis of trochanteric bursitis was established by a physiatrist, and the patient underwent two courses of physical therapy for moist heat, ultrasound, and exercise. Because minimal improvement was seen in the patient's pain level and gait limitations following physical therapy, a consultation was requested of a fellow staff member in the same physical therapy department, who is both a chiropractor and a physical therapist. The consultant felt that the absence of tenderness to deep palpat

Botulinum toxin for spasmodic dysphonia.

Botulinum toxin is the accepted method of managing SD and has given most patients with adductor SD the ability to develop satisfactory vocal function. Results have been less satisfactory in abductor SD. Management is accomplished best with a team approach. The physiatrist with experience in EMG and botulinum toxin usage can be a prominent member of this team.

Electrodiagnosis in Cancer Rehabilitation.

With numerous advancements in early detection and multimodal therapy, cancer has become a chronic disease. As the number of cancer survivors continues to increase, physiatrists and other neuromuscular disease specialists are more likely to encounter individuals with residual impairments, disabilities, and/or handicaps resulting from cancer or related treatments. The patient with cancer is especially prone to injury directed at the peripheral nervous system at multiple anatomic levels. Electrodiagnosis is an invaluable tool in the evaluation of neuromuscular disorders in this patient population

Piriformis syndrome: assessment of current practice and literature review.

Although piriformis syndrome was initially described in 1928, it has remained a nebulous and controversial diagnosis. A literature review reveals that there is no consensus on the diagnosis or treatment of piriformis syndrome. In addition, a survey of 75 physiatrists revealed that whether the diagnosis of piriformis syndrome even exists is controversial. This survey and literature review examines the diagnosis and treatment of piriformis syndrome. The fact that it remains controversial is likely due to a lack of objective clinical trials.

Multiple sclerosis potpourri. Paroxysmal symptoms, seizures, fatigue, pregnancy, and more.

Several symptom complexes in multiple sclerosis (MS) are found in unusual circumstances but are characteristic of the disease. Most of these are amenable to treatment and will be confronted by the physiatrist treating patients who have MS. This article begins by addressing paroxysmal symptoms such as trigeminal neuralgia, paroxysmal dysarthria and ataxia, parathesia and pain, paroxysmal itching, and akinesia. Seizures, adventitious movements, fatigue, and complications related to pregnancy also are addressed.

The military upper extremity amputee.

Throughout the course of military history, soldiers have continued to sustain amputation injuries during war times and during peacetime and training missions. What has changed over time is the etiology of, indication for, and management of the amputations. Technology has advanced significantly, often with some military connection. More work still needs to be done, especially in the areas of greater prosthetic limb function and usage as well as phantom pain and sensation management. Collaborative efforts among physiatrists, surgeons, prosthetists, and therapists can only benefit the patient.

[A 24-year-old female with acute onset shoulder pain].

A 24-year-old female presents with sudden debilitating right shoulder pain that started 5 days ago, with reduced range of motion (ROM) in shoulder abduction and a scapula alata. These findings are indicative of neuralgic amyotrophy (NA). NA usually has a diagnostic delay which complicates treatment. Long-term impact can be severe which sometimes justifies referral to a physiatrist.

Prescribing Exercise to Cancer Patients Suffering from Increased Bone Fracture Risk Due to Metastatic Bone Disease or Multiple Myeloma in Austria-An Inter- and Multidisciplinary Evaluation Measure.

IntroductionIn the current absence of specific functional fracture risk assessment technology, the planning of physical exercise interventions for cancer patients suffering from increased bone fracture risk remains a serious clinical challenge. Until a reliable, solely technical solution is available for the clinician, fracture risk assessment remains an inter- and multidisciplinary decision to be made by various medical experts. The aim of this short paper is depicting how this challenge should be approached in the clinical reality according to Austrian experts in cancer rehabilitation, prese

Team Approach: Rehabilitation Strategies for Patients After Osteosarcoma Reconstructive Surgery.

The multimodal treatment of osteosarcoma with chemotherapy, surgical resection, and reconstruction has improved outcomes after a limb-salvage surgical procedure. Physical rehabilitation considerations after surgical resection vary, depending on the location of the tumor. Physical medicine and rehabilitation physicians incorporate lymphedema specialists, orthotists, and prosthetists to help to improve limb function. Beyond physical rehabilitation, psychological or behavioral interventions and nutritional rehabilitation are necessary to maximize a patient's return to function.

Studies indicate that cocaine abuse in Denmark is rising. The drug can damage the midface's nasal tissues, cartilage, and bone. Diagnosing the cocaine-induced midline destructive lesions condition is difficult as patients may not admit to drug use. Thus, this review finds that physicians should ask about cocaine abuse in younger patients who present with midline destructions of unknown origin. Mild symptoms are reversible with total abstinence, which is why it is important to involve addiction services early. Besides drug abstinence, comprehensive treatment involves assistance from GPs, physia

Nerve and Tendon Transfers After Spinal Cord Injuries in the Pediatric Population: Clinical Decision Making and Rehabilitation Strategies to Optimize Function.

With improvements in medical care, pediatric patients with spinal cord injuries with tetraplegia are living into adulthood. The goal of rehabilitation following loss of upper extremity function caused by tetraplegia is to maximize function and independence. Physiatrists must be aware of appropriate timing of referral for upper extremity surgery because it can have significant ramifications on the outcome. This article discusses the 2 most commonly used surgical strategies to restore upper extremity function: upper extremity tendon transfer and nerve grafting/transfer. Patient selection, physic
