Thrombosis is a major complication of polycythemia vera (PV) and also a well-known complication of thalassemia. We reported a case of non-atherosclerotic ST-segment elevation myocardial infarction (STEMI) in a 17-year-old man with concurrent post-splenectomized hemoglobin H-Constant Spring disease and JAK2 V617F mutation-positive PV. The patient initially presented with extreme thrombocytosis (platelet counts greater than 1,000,000/µL) and three months later developed an acute STEMI. Coronary artery angiography revealed an acute clot in the right coronary artery without atherosclerotic plaque.

Acute leukemias with thrombocytosis have been recently linked with structural abnormalities of the short arm of chromosome 3. Essential thrombocythemia (ET) is one of the myeloproliferative disorders characterized by an elevated platelet count, usually greater than 1,000 X 10(9)/L. It may be associated with either hemorrhagic or thrombotic tendencies. Although it usually affects older people, we have recently encountered two young women with ET who had a total of six spontaneous abortions before the end of the first trimester. This report describes an unusual case of an apheresis platelet (AP) unit in platelet additive solution (PAS) exhibiting marked icteric discoloration and bag staining, despite sterile cultures and initially preserved platelet morphology. Donor evaluation revealed mild unconjugated hyperbilirubinemia, implicating donor-derived pigments as the source of discoloration. By Day 5, reduced platelet function was observed. Therapeutic thrombocytapheresis has been used is an attempt to obtain a rapid cytoreduction in thrombocythemic patients, in order to prevent or reduce thrombosis or bleeding complications. BACKGROUND: The biologic mechanisms of allergic transfusion reactions (ATRs) are largely unknown. We sought to compare the atopic predisposition of platelet (PLT) recipients who experienced an ATR to nonreactive control recipients.
