Cavitary lung lesions: Melioidosis and pulmonary embolism causing necrotizing pneumonia.

Cavitary lung lesions of various etiologies may be encountered in patients with respiratory symptoms associated with fever. Non-malignant cavitary lesions may mimic malignant lung lesions on most of radiographic modalities including chest radiographs or thoracic computed tomography (CT). Primary lung malignancy can be detected in as high as one-fifths of CT thorax as cavitary lesions and the remaining aetiologies may be due to bacterial, parasitic, and invasive fungal infections, as well as Granulomatosis with polyangiitis (GPA), sarcoidosis, septic thrombo-embolism, and lung metastasis from e

Bacillus cereus necrotizing pneumonia in a patient with nephrotic syndrome.

Bacillus cereus (B. cereus) is a Gram-positive rod that is widely distributed in the environment and can be a cause of food poisoning. We herein present a case of B. cereus necrotizing pneumonia in a patient with nephrotic syndrome under corticosteroid treatment after developing transient gastroenteritis symptoms. B. cereus was isolated from bronchial lavage fluid and transbronchial biopsy specimens. A multiplex polymerase chain reaction analysis of the toxin genes revealed a strain possessing enterotoxicity. The patient recovered after one week of intravenous meropenem followed by a combinati

[Necrotizing pneumonia due to Staphylococcus aureus producing Panton-Valentine toxin].

BACKGROUND: Necrotizing pneumonia caused by Staphylococcus aureus producing Panton Valentine Leukocidine (PVL) has been described recently and is associated with a high mortality (75%). OBSERVATION: We report a case of pneumonia due to PV secreting Staphylococcus aureus in a healthy young adult, complicated by multiple necrotizing lung lesions and major cystic changes, with a favourable final outcome. Acute respiratory failure with haemodynamic failure and ARDS developed a few days after an influenza-like illness. The appearances at fibreoptic bronchoscopy were atypical, consisting of a purul

[Necrotizing pneumonia due to Gemella morbillorum in an immunocompetent patient].

BACKGROUND: Gemella morbillorum is an anaerobic to aerotolerant Gram positive coccus. It is considered a member of the normal upper respiratory tract flora including the oropharynx and also the gastro-intestinal and female genital tract. However severe infections are reported like endocarditis, septic shock and septic arthritis, more often in immunocompromised hosts. It is an infrequently isolated organism and a rare cause of pulmonary or pleural infections. CASE REPORT: We report a case of necrotizing pneumonia due to Gemella morbillorum in a 45-years-old Melanesian, smoker patient. The pres

Extraintestinal pathogenic Escherichia coli-induced acute necrotizing pneumonia in cats.

Extraintestinal pathogenic Escherichia coli (ExPEC) are pathogens involved in several disease conditions, ranging from urinary tract infection to meningitis in humans and animals. They comprise epidemiologically and phylogenetically distinct strains, affecting most species and involving any organ or anatomical site. Here, we report fatal cases of necrotizing pneumonia in cats. Over a 1-week period, 13 cats from an animal shelter in Stamford, Connecticut were presented for necropsy. All had a clinical history of acute respiratory disease. The gross and microscopic findings for all the cats were

Early Prediction of Necrotizing Pneumonia in Children with Mycoplasma Pneumoniae Pneumonia: Development and Temporal Validation of a Clinical Model.

BACKGROUND: Necrotizing pneumonia is a severe complication of Mycoplasma pneumoniae pneumonia (MPP) in children. Early recognition remains challenging because initial clinical manifestations are often non-specific, highlighting the need for a practical tool for early risk stratification. METHODS: We conducted a single-center retrospective study of hospitalized children with MPP. Data from 2015-2023 were used for model development, and patients enrolled in 2024 were reserved for temporal validation. We compared candidate machine-learning algorithms and selected a parsimonious random forest mod

Development and validation of a predictive nomogram for early detection of necrotizing pneumonia in children with refractory Mycoplasma pneumoniae pneumonia.

OBJECTIVE: To identify predictive factors for necrotizing pneumonia (NP) in children with refractory Mycoplasma pneumoniae pneumonia (RMPP) and develop a predictive nomogram. METHODS: A retrospective analysis was conducted on clinical data of children with RMPP admitted to the Affiliated Women and Children's Hospital of Dalian University of Technology between June 2023 and July 2024. The dataset was randomly split into a training set (70%, n = 197) for model development and a test set (30%, n = 77) for internal validation. The χ 2 test and Mann-Whitney U test were used to screen potential pre

An unusual case of necrotizing pneumonia in a healthy apnea diver.

We report a case of necrotizing pneumonia caused by hypervirulent Klebsiella pneumoniae in an athletic, 54-year-old apnea diver. Since hypervirulent Klebsiella pneumoniae strains prevail in Southeast Asia and are mainly associated with "invasive liver abscess syndrome", we considered and tested for hypervirulence only when the necrotizing pneumonia turned out to be refractory to initial antibiotic treatment. Although the hypervirulent Klebsiella pneumoniae strain in our patient did not show multi-drug resistance, a total of 6 months of high-dose antibiotic therapy was necessary to cure the inf

[A multicenter retrospective study on the etiology of necrotizing pneumonia in children].

Objective: To investigate the etiology of necrotizing pneumonia (NP) in children and the clinical characteristics of NP caused by different pathogens in China. Methods: A retrospective, case-control study was performed in children with NP who were admitted to 13 hospitals in China from January 2008 to December 2019. The demographic and clinical information, laboratory data, etiological and radiological findings were analyzed. The data were divided into three groups based on the following years: 2008-2011, 2012-2015 and 2016-2019, and the distribution characteristics of the pathogens in differe

Klebsiella pneumoniae cervical necrotizing fasciitis originating as an abscess.

Cervical necrotizing fasciitis is a rapidly progressive and devastating infection that is usually caused by a polymicrobial infection including group A Streptococcus and anaerobes. We present a case of a newly diagnosed diabetic man who underwent transoral drainage of a Klebsiella pneumoniae paraglottic abscess, which, despite culture-directed antibiotics, progressed to cervical necrotizing fasciitis with descending mediastinitis. The patient required 12 surgical debridements and sternotomy, but survived and was discharged on hospital day 40. To our knowledge, this is the first case of cervica

Klebsiella pneumoniae necrotizing fasciitis in a Latin American male.

Necrotizing fasciitis, caused by Klebsiella pneumoniae, is a rare and life-threatening bacterial infection. Most documented cases have been reported from Asia, particularly associated with diabetes mellitus. The prevalence of this infection in the USA is rare, especially among persons of non-Asian descent and those without travel to Asia. We report a case of disseminated necrotizing fasciitis, caused by K. pneumoniae, in a Latin American male with diabetes mellitus. Given our review of the literature, this is the only case report, to our knowledge, of a Latin American patient with Klebsiella n

Exploring the pathogenesis of necrotizing fasciitis due to Streptococcus pneumoniae.

Monobacterial necrotizing fasciitis is a rare form of soft tissue infection usually caused by the group A beta-hemolytic Streptococcus. Soft tissue infection is an uncommon clinical manifestation of invasive disease due to Streptococcus pneumoniae. We describe 3 cases of pneumococcal necrotizing fasciitis and explore potential pathogen-specific mechanisms of pathogenesis. The clinical characteristics of necrotizing fasciitis due to S. pneumoniae and group A beta-hemolytic Streptococcus appear to overlap. The similarities include predominant occurrence in elderly adults with underlying chronic

A case of necrotizing pneumonia leading to respiratory failure and tracheostomy.

Necrotizing pneumonia is a severe lung infection characterized by pulmonary necrosis and high mortality rates of up to 50 %. It is typically caused by toxin-producing pathogens, such as Staphylococcus aureus and Klebsiella pneumoniae. Limited guidelines exist for its management, making its treatment challenging. Case report: We discuss a case of a 58-year-old Malay woman with no significant comorbidity but developed necrotizing pneumonia. The diagnosis was confirmed using computed tomography (CT) of the thorax. Treatment with carbapenem, oxazolidinone and corticosteroid led to significant reco

Case Report of Necrotizing Fasciitis Associated with Streptococcus pneumoniae.

Necrotizing fasciitis, caused by Streptococcus pneumoniae, is an extremely rare and life-threatening bacterial soft tissue infection. We report a case of early necrotizing fasciitis associated with Streptococcus pneumoniae infection in a 26-year-old man who was immunocompromised with mixed connective tissue disease. The patient presented with acute, painful, erythematous, and edematous skin lesions of his right lower back, which rapidly progressed to the right knee. The patient underwent surgical exploration, and a diagnosis of necrotizing fasciitis was confirmed by pathological evidence of ne

Streptococcus pneumoniae necrotizing fasciitis in systemic lupus erythematosus.

Necrotizing fasciitis is a rapidly progressive destructive soft tissue infection with high mortality. Streptococcus pneumoniae as etiologic agent of necrotizing fasciitis is extremely unusual. The increased susceptibility to Streptococcus pneumoniae infection in patients with systemic lupus erythematosus is probably a multifactorial phenomenon. We report a case of a patient, a 36-year-old Caucasian female with 8-year history of systemic lupus erythematosus who presented a fatal Streptococcus pneumoniae necrotizing fasciitis. The role of computed tomography and the high performance of blood cul

Necrotizing or cavitating pneumonia due to Streptococcus Pneumoniae: report of four cases and review of the literature.

Streptococcus pneumoniae is seldom considered as an etiologic agent of necrotizing or cavitating pneumonia. However, during a 5-month period we encountered four patients, bacteremic with S. pneumoniae, with such a pulmonary process. Review of the older literature indicates that this association may be more frequent than is commonly assumed. Anatomic, physiologic, and immunologic alterations of the pulmonary defense mechanisms prior to and during the infection as well as virulence factors of S. pneumoniae (i.e., rapid multiplication, accumulation of capsular polysaccharides, and inhibition of p
