Accuracy of Point-of-Care Testing for Anemia in the Emergency Department.

BACKGROUND: Pulse oximetry has become the standard of care in emergency medicine, operating rooms, and medical wards for the monitoring of oxygenation, but the use of pulse oximetry for assessment of hemoglobin (Hb) is controversial. The purpose of this study was to compare the accuracy and precision of 2 point-of-care Hb measurement devices, the Pronto-7 and the HemoCue 201+, to laboratory testing. METHODS: We studied a convenience sample of patients in the emergency department who required a complete blood count. We excluded patients in critical condition or those with elevated methemoglobi

Point-of-care-testing of α-amylase activity in human blood serum.

Activity of α-amylase enzyme in human serum indicates the onset of pancreatitis, mumps, cancer, stress, and depression. Herein we design and develop a biosensor for the point-of-care-testing (POCT) of α-amylase concentration in serum. The biosensor is composed of a glass substrate coated with an electrically conducting poly-aniline-emeraldine-salt (PANI-ES) film covered with starch-coated gold nanoparticles (SAuNPs). Addition of different dosage of α-amylase on the biosensor selectively depletes starch stabilized on the SAuNPs, which changes the electrical resistance of the sensor. The change

Towards point-of-care HIV testing: Terahertz PCF sensor integration and miniaturization.

The Human Immunodeficiency Virus, or HIV, is a retrovirus which aims at CD4 cells in particular to destroy the body's natural defenses along with impair its capacity for fighting off illness plus ailments. In this circumstance we suggested a Photonic Crystal Fiber (PCF) sensor for detection of this lethal virus. Our intended sensor exhibits Max Relative Sensitivity (RS) 95.2% & 95% for x & y axis correspondingly when f = 3.0 THz. It also impresses us with minimal Confinement Loss (CL) of 1.06 × 10-7 & 2.04 × 10-7dB/m plus Effective Material Loss (EML) of 0.0078 & 0.0110 cm-1 in that order for

Point-of-care testing for sexually transmitted infections in low-resource settings.

BACKGROUND: Both the global incidence and the prevalence of sexually transmitted infections (STIs) continue to increase, affecting hundreds of millions of individuals, particularly in low-to middle-income countries. Although a definitive diagnosis is desirable to inform STI treatment, syndromic management is the most widely used strategy in resource-limited settings. With the development of point-of-care (POC) tests, it is important to discuss how laboratories will need to adapt to new training and supervisory roles in support of testing, which will largely be performed by peripheral clinical

Scrub typhus point-of-care testing: A systematic review and meta-analysis.

BACKGROUND: Diagnosing scrub typhus clinically is difficult, hence laboratory tests play a very important role in diagnosis. As performing sophisticated laboratory tests in resource-limited settings is not feasible, accurate point-of-care testing (POCT) for scrub typhus diagnosis would be invaluable for patient diagnosis and management. Here we summarise the existing evidence on the accuracy of scrub typhus POCTs to inform clinical practitioners in resource-limited settings of their diagnostic value. METHODOLOGY/PRINCIPAL FINDINGS: Studies on POCTs which can be feasibly deployed in primary he

Point-of-care visco-elastic testing for postpartum haemorrhage: A narrative review.

Postpartum haemorrhage (PPH) remains a leading cause of maternal morbidity and mortality worldwide. While coagulation disorders are seldom the primary cause of PPH and are rare early in PPH, the incidence of coagulation abnormalities increases when blood loss escalates. Acute obstetric coagulopathy (AOC), with an incidence of one in 1000 deliveries, has emerged as a distinct coagulopathy in PPH, highlighting the need for timely coagulation testing and intervention. This narrative review examines the current evidence on the use of visco-elastic haemostatic assays (VHAs) to guide treatment in PP

Point-of-care ionised calcium testing: lab validation and clinical feasibility study.

BACKGROUND: Patients undergoing thyroid and parathyroid surgery require frequent assessment of blood calcium levels to guide their management, and currently such measurements are performed mostly in hospital environments on main laboratory platforms. The aim of this study was to explore the potential use of a non-medical LAQUA device designed to measure ionised calcium in environmental samples as a pocket-size point-of-care device able to measure blood calcium concentration in patients after thyroid and parathyroid surgery. METHODS: The protocol of the study consisted of three distinctive pha

Implementing point-of-care hemoglobin A1C testing in an obstetrics outpatient clinic.

BACKGROUND: A1C ≥6.0% is associated with increased risk of adverse outcomes in pregnant diabetic patients. A1C testing is recommended by the American Diabetes Association as a secondary measure of glycemic control in pregnant patients. OBJECTIVE: To determine the utility of A1C point-of-care testing (POCT) during pregnancy to facilitate rapid counseling and diabetes care, particularly in relatively low-income transient patient populations. METHODS: We performed a single-center, retrospective analysis of patients presenting to an outpatient obstetrics office with routine, in-laboratory A1C te

Near Point-of-Care HIV Viral Load: Targeted Testing at Large Facilities.

INTRODUCTION: Point-of-care (POC) technologies in resource-limited settings can circumvent challenges of centralized laboratory testing, improving clinical management. However, higher device costs and uncertain indications for use have inhibited scaling up POC modalities. To address this gap, we investigated the feasibility and cost of targeted near-POC viral load (VL) testing in 2 large HIV clinics in Lilongwe, Malawi. METHODS: VL testing using GeneXpert was targeted for patients suspected of treatment failure or returning to care after a previously elevated VL (>1000 copies/mL). Descriptive

A Cell-Phone-Based Acoustofluidic Platform for Quantitative Point-of-Care Testing.

Acoustofluidic methods, with advantages including simplicity of device design, biocompatible manipulation, and low power consumption, have been touted as promising tools for point-of-care (POC) testing. Here, we report a cell-phone-based acoustofluidic platform that uses acoustic radiation forces to enrich nanoscale analytes and red and green fluorescence nanoparticles (SiO2@R and G@SiO2) as probes for POC visual testing. Thus, the color signals from the fluorescent probes are enhanced, and colorimetric sensitivity is significantly improved. As a POC demonstration, the acoustofluidic platform

Point-of-care testing for chlamydia and gonorrhoea: implications for clinical practice.

OBJECTIVES: Point-of-care (POC) testing for chlamydia (CT) and gonorrhoea (NG) offers a new approach to the diagnosis and management of these sexually transmitted infections (STIs) in remote Australian communities and other similar settings. Diagnosis of STIs in remote communities is typically symptom driven, and for those who are asymptomatic, treatment is generally delayed until specimens can be transported to the reference laboratory, results returned and the patient recalled. The objective of this study was to explore the clinical implications of using CT/NG POC tests in routine clinical c

Coeliac disease case finding and diet monitoring by point-of-care testing.

BACKGROUND: Immunoglobulin A class transglutaminase autoantibodies are highly predictive markers of active coeliac disease, a disorder difficult to recognize solely on clinical grounds. AIMS: To develop and evaluate a simple rapid test for point-of-care detection of coeliac autoantibodies. METHODS: The novel whole blood test utilizes the patient's endogenous transglutaminase in red blood cells for detection of transglutaminase-specific immunoglobulin A antibodies present in the blood sample, with normal plasma immunoglobulin A detection as positive test control. We evaluated 284 patients und

Client, health care provider, and policymaker perspectives on point-of-care/near-point-of-care testing for sexually transmitted infections in community pharmacies in Uganda.

INTRODUCTION: Point-of-care or near-point-of-care (POC/near-POC) tests for sexually transmitted infections (STIs) are often unavailable and unaffordable in low-resource settings, despite the World Health Organization's (WHO) recommendation that such testing be offered, leading to empiric treatment. Private community pharmacies (CPs) are often the first point of contact for people with symptoms of STIs, where pharmacists dispense treatment without diagnostic testing or prescriptions. This study evaluated clients,' providers,' and policymakers' perspectives on POC/near-POC STI testing in communi
