RHD allelic identification among D-Brazilian blood donors as a routine test using pools of DNA.

BACKGROUND: RHD alleles leading to a reduced expression of D antigen of the red blood cell (RBC) surface may be erroneously typed as D- by serology and may cause anti-D immunizations when transfused to recipients. METHODS: To determine the occurrence of such alleles among apparent D- blood donors, molecular typing was implemented as a routine test using a pool of DNA. A total of 2,450 pretyped D- samples were tested in pools of 10 for the RHD-specific polymorphism in intron 4 and exon 7. Samples in polymer chain reaction (PCR) positive pools were individually reevaluated by exon-specific PCRs

Pooled analysis of the accuracy of five cervical cancer screening tests assessed in eleven studies in Africa and India.

Cervical cancer is the main cancer among women in sub-Saharan Africa, India and other parts of the developing world. Evaluation of screening performance of effective, feasible and affordable early detection and management methods is a public health priority. Five screening methods, naked eye visual inspection of the cervix uteri after application of diluted acetic acid (VIA), or Lugol's iodine (VILI) or with a magnifying device (VIAM), the Pap smear and human papillomavirus testing with the high-risk probe of the Hybrid Capture-2 assay (HC2), were evaluated in 11 studies in India and Africa. Clinical sensitivity and time-to-result of a cascaded pooled testing approach for assessing the prevalence and intensity of Schistosoma haematobium infection.

BACKGROUND: This study compared the clinical sensitivity and the time-to-result of an individual testing (IT) and a cascaded pooled testing approach (CPT; a positive test result in a pooled sample triggers examination of smaller-sized pools or individual samples) for assessing the prevalence and the intensity of Schistosoma haematobium infection. We also compared the sensitivity of the CPT in detecting S. haematobium infection when deploying urine filtration microscopy (UFM) vs. urine reagent strips (URS), and testing 10 mL vs. 15 mL of urine. A novel strategy to avoid sensitivity loss in pooled testing for SARS-CoV-2 surveillance: validation using nasopharyngeal swab and saliva samples.

At the peak of the COVID-19 pandemic, pooled surveillance strategies were employed to alleviate the overwhelming demand for clinical testing facilities. A major drawback of most pooled-testing methods is the dilution of positive samples, which leads to a loss of detection sensitivity and the potential for false negatives. We developed a novel pooling strategy that compensates for the initial dilution with an appropriate concentration during nucleic acid extraction and real-time PCR. We demonstrated the proof of principle using laboratory-created 10-sample pools with one positive and correspond

Efficient high-risk human papillomavirus screening through pooled testing: a simulation study based on real-world data in low- and middle-income countries.

BACKGROUND: The World Health Organization recommends high-risk human papillomavirus (hrHPV) testing as the primary method for cervical cancer screening. Combining samples in a pool can reduce the expense associated with large numbers of commercially available tests. This study simulated the potential test-saving impact of pooled hrHPV testing using real-world data from two low- and middle-income countries (LMICs). METHODS: We simulated a two- and three-stage hrHPV pooling in Bangladesh (hrHPV prevalence 2.10%) and Uganda (hrHPV prevalence 22.52%) using real-world laboratory data. Measurements

Pooled prevalence, spatial variation and associated factors of HIV testing uptake among multiple sexual partners in Sub Saharan Africa: Spatial and multilevel analysis.

BACKGROUND: Uptake of HIV testing is vital for the early diagnosis of HIV infection and initiation of treatment, which are used to eliminate the disease's progression and reduce HIV-related mortality. Even if determining HIV testing is imperative to prevent HIV/AIDS among multiple sexual partners who are at higher risk of sexually transmitted infections, most of the countries in Sub Saharan Africa did not fulfil the global targets of UNAIDS. Moreover there is a paucity of literature on spatial variation and factors associated with HIV testing among high-risk groups in SSA. This study aimed to

Sample pooling approaches simulated under resource scarcity, lapses in testing capacity, and rapid processing demands for surveillance testing: a data-driven performance comparison.

BACKGROUND: Sample pooling is a critical strategy to meet increased testing demand and conserve resources in surveillance testing. Much of its effectiveness depends on how well optimized the pool size is to the prevalence of infection in the sampled population, which can be difficult to anticipate in many circumstances. Multiple methods exist to better optimize pooling, with unique trade-offs. METHODS: Pooling optimization methods were simulated to examine trade-offs between surveillance priorities and operational characteristics using SARS-CoV-2 surveillance data and workflows generated by th

Temporal Trends and Determinants of HIV Testing at Antenatal Care in Sub-Saharan Africa: A Pooled Analysis of Population-Based Surveys (2005-2021).

BACKGROUND: In sub-Saharan Africa (SSA), integrating HIV testing into antenatal care (ANC) has been crucial toward reducing mother-to-child transmission of HIV. With the introduction of new testing modalities, we explored temporal trends in HIV testing within and outside of ANC and identified sociodemographic determinants of testing during ANC. METHODS: We analyzed data from 139 nationally representative household surveys conducted between 2005 and 2021, including more than 2.2 million women aged 15-49 years in 41 SSA countries. We extracted data on women's recent HIV testing history (<24 mon

Evaluating the Impact of Ferritin Testing: Iron Status of the Donor Pool, Changes to Hemoglobin Deferrals, and Effects on Product Availability.

Blood collectors in many jurisdictions globally have implemented ferritin testing of their donor populations as an additional donor safety measure. Where performed, ferritin testing is generally conducted on a selective basis, restricted to certain populations perceived to face higher risk for or from iron deficiency or applied to periodic rather than all donations from donors. The wide array of ferritin testing configurations that currently exist, together with underlying differences in donation eligibility policies, suggests the impact of ferritin testing on blood center and donor outcomes-b

Pathogen performance testing of a natural swimming pool using a cocktail of microbiological surrogates and QMRA-derived management goals.

In recent decades, natural swimming pools (NSPs) have gained popularity in Europe, especially in Germany and Austria. NSPs differ from swimming pools in that they utilize biological treatment processes based on wetland processes with no disinfection residual. However, data are missing on the specific log-reduction performance of NSPs to address enteric virus, bacteria, and parasitic protozoa removal considered necessary to meet the North American risk-based benchmark (<35 illnesses per 1,000 swimming events) set by the USEPA for voluntary swimming. In this study, we examined Canada's first NSP

Evaluation of Sample Pooling Strategies for Point-of-Care HPV DNA Testing Using the STANDARD™ M10 Platform.

Human papillomavirus (HPV) DNA testing is the recommended primary screening method for cervical cancer prevention. However, implementation in resource-limited and geographically challenging settings is constrained by cost, logistics, and time-sensitive screen-and-treat workflow. Sample pooling may improve testing efficiency, but evidence for its feasibility on point-of-care molecular platforms remains limited. We evaluated the analytical performance and feasibility of sample pooling for high-risk HPV (hrHPV) detection using the STANDARD™ M10 HPV point-of-care assay. Archived self-collected vag

Underscoring the effect of swab type, workflow, and positive sample order on swab pooling for COVID-19 surveillance testing.

Sample pooling is a promising strategy to facilitate COVID-19 surveillance testing for a larger population in comparison to individual single testing due to resource and time constraints. Increased surveillance testing capacity will reduce the likelihood of outbreaks as the general population is returning to work, school, and other gatherings. We have analyzed the impact of three variables on the effectiveness of pooling test samples: swab type, workflow, and positive sample order. We investigated the performance of several commercially available swabs (Steripack polyester flocked, Puritan nyl

The Performance of Pooled 3 Anatomic Site Testing for Chlamydia trachomatis and Neisseria gonorrhoeae Among Men Who Have Sex With Men and Transgender Women.

BACKGROUND: Although molecular testing for Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) is highly sensitive, the cost can be prohibitive. Those high costs are amplified when the recommended screening approach is used, which requires separate testing of specimens from 3 anatomic sites (rectal, pharyngeal and urogenital). Although individual molecular testing is standard of care, pooled testing may offer a cost-saving alternative. METHODS: Using the Xpert® CT/NG assay (Cepheid, Sunnyvale, CA) we tested urine, rectal and pharyngeal swabs for CT and NG in a high-risk cohort of partic

The impact of HIV stigma and discrimination on HIV testing, antiretroviral treatment, and viral suppression in Africa: a pooled analysis of population-based surveys.

BACKGROUND: UNAIDS' ending AIDS targets include ensuring that less than 10% of people with HIV, at risk of HIV, or affected by HIV experience HIV stigma and discrimination. We aimed to estimate the effect of HIV stigma and discrimination on the HIV care cascade in eastern, southern, central, and western Africa-regions with the largest HIV burden worldwide. METHODS: We used population-based surveys in countries in eastern, southern, central and western Africa from 2000 onward identified through previous reviews and data catalogues.
