Cystic adventitial disease of the popliteal artery: resection and repair with autologous vein patch.

Cystic adventitial disease is rare, but it is one of the well-recognized causes of non-atherosclerotic arterial stenosis or obstruction. Despite one of its most common symptoms being chronic intermittent claudication, it may be misdiagnosed as arterial embolism when presented with acute ischemic symptoms. Surgical resection is recommended because of recurrence or a low success rate with aspiration or endovascular stent. We performed resection and repair with autologous vein patch for cystic adventitial disease of the popliteal artery of a 57-year-old man presenting with pain, pallor, and pares

Traumatic venous aneurysm of the popliteal vein with outcome: a case report and review of the literature.

A case of sudden death due to recurrent pulmonary thromboembolism is described. The fatality took place three and a half weeks following blunt trauma to the left popliteal region. The patient died unexpectedly. Autopsy revealed the source of the emboli as a sacciform venous aneurysm of the popliteal vein, an entity seldom described, but important to consider in cases of soft tissue popliteal masses or unexplained pulmonary embolism, especially in otherwise healthy individuals.

Use of the in-situ lesser saphenous vein bypass from popliteal to peroneal artery.

Lesser saphenous vein grafts have become an important element in limb salvage surgery. Their location frequently dictates that the graft be translocated for use. This incurs the disadvantages of ischemia and increased manipulation of the graft and loses the advantages of an in-situ bypass. The use of the lesser saphenous vein as an in-situ graft for tibial artery reconstruction offers the advantages of reduced vein graft injury and improved patency. Technical requirements for an in-situ lesser saphenous bypass include that graft inflow be based on the popliteal artery, the vein length suffice

Anatomic relationship between the popliteal artery and vein: a guide to accurate angiographic puncture.

The anatomic relationship between the popliteal artery and vein means that an arteriovenous fistula can be created when a popliteal artery approach is used for endovascular interventions. To determine the best site for retrograde puncture of the popliteal artery, we studied six cadaveric specimens, CT scans of 31 patients at 280 levels, and 30 plain radiographs of the knee. In the cadaveric specimens, the vessels were enclosed in a common sheath. In 92% of all levels studied on CT, the artery was anterior to the vein (anterolateral in 9%, anteromedial in 43%, strictly anterior in 40%), and mor

Impact of concomitant popliteal vein thrombosis in patients with acute iliofemoral deep vein thrombosis treated with endovascular early thrombus removal.

Background: Catheter-based thrombus removal (CBTR) reduces the risk of moderate to severe post-thrombotic syndrome (PTS) in patients with ac … rable, regardless of the presence or absence of concomitant popliteal DVT. However, post-thrombotic popliteal vein lesions and reflux are more frequent in IF-DVT patients with popliteal involvement. Their impact on long-term outcomes remains to be investigated.

A Rare Cause of Popliteal Artery and Vein Transection: A Case Report and Literature Review.

We present a rare occurrence of popliteal vascular injury due to blunt trauma. The patient had an isolated blunt lower extremity trauma. The patient subsequently experienced moderate tenderness and non-expanding hematoma at the popliteal fossa, reduced range of motion at the knee, and diminished distal pulses. X-rays showed a patella dislocation and tibial plateau non-displaced fracture but no knee dislocation. CT angiography showed an abrupt non-opacification of the distal portion of the popliteal artery with an overlying large hematoma. Surgical exploration was performed which revealed a con

[An overview of treatment options for a rare disease, aneurysm of the popliteal vein, in connection with a case].

INTRODUCTION: Popliteal vein aneurysm is a rare, but potentially life-threatening condition that can lead to deep vein thrombosis and/or pulmonary embolism. It is often asymptomatic, but symptoms may include pain, post-thrombotic syndrome or chronic venous insufficiency. An experienced physician may be able to detect a palpable mass in the popliteal fossa. Duplex ultrasound is the first line of diagnosis. CT or MR venography play a role in the diagnosis. OBJECTIVE: To review the international literature, explain the possible treatment options, and present our case. CASE REPORT: A 62-year-old…

A giant popliteal artery aneurysm treated with exclusion and bypass using a saphenous vein.

While popliteal artery aneurysm is the most common form of peripheral artery aneurysm, it is a rare condition in the general population. Furthermore, a giant popliteal artery aneurysm has not previously been reported in Korea. A 67-year-old male presented with left thigh pain that had begun 4 months earlier and was aggravated when in a sitting position. We found a giant aneurysm on the left popliteal artery and performed a bypass from the common femoral artery to the distal popliteal artery below the knee, using the autologous greater saphenous vein, and excluded the aneurysm at the sites of a

Popliteal-to-tibial in situ saphenous vein bypass for limb salvage in diabetic patients.

This study has summarized our results with popliteal-tibial in situ saphenous vein bypass in 26 patients, 25 of whom were diabetic, over a 2 year period. Both above- and below-knee popliteal inflow sites were used for bypass of limb-threatening ischemia. Distal calf or pedal outflow sites were required in all but two patients who had sequential bypass performed to tibial sites. Postoperative ankle-brackial indices were calculated. Eleven patients had transcutaneous mapping surrounding the pedal skin envelope injuries. The mean lowest and highest transcutaneous oxygen values have been reported

Mid and Long-Term Outcomes of Externally Stented Vein Grafts for Popliteal Aneurysm Repair.

BACKGROUND: Popliteal artery aneurysms (PAAs) may result in limb-threatening acute or chronic ischemia if left untreated. Open repair remains the gold standard, with durable long-term outcomes, particularly when an autologous vein graft is available. The FRAME™ external vein graft stent (FEVGS) has been developed to limit vein graft dilatation and maintain favorable hemodynamic conditions over time. This study aims to evaluate the mid- and long-term outcomes of externally stented vein grafts for popliteal aneurysm repair. METHODS: Patients undergoing urgent or elective open PAA repair with FE

Case report of superficial femoral artery and popliteal artery aneurysm repair using brachial vein.

BACKGROUND: Several veins have been well-recognized as acceptable conduits for infrainguinal bypass surgery when the ipsilateral greater saphenous vein is unavailable. However, there is a paucity of literature describing the brachial vein as an adequate alternative. In the absence of other viable autogenous conduits, we describe the use of a brachial vein as a successful alternative for lower extremity revascularization. METHODS: A 70-year-old man presented with a chief complaint of right calf pain. Duplex ultrasound imaging of his right lower extremity revealed right-sided 2.5 cm acutely thr

Results of prosthetic-vein composite graft with remote popliteal arteriovenous fistula in infragenicular bypass.

OBJECTIVE: The purpose of this study is to report the results of a novel procedure for femoral-distal bypass grafting using a composite graft with an adjunctive remote popliteal fistula distal to the prosthetic portion of the graft. This reconstruction was developed for use in limb salvage in the absence of satisfactory autogenous vein. METHOD: Data were collected prospectively on all patients undergoing this procedure from January 1, 1993 to December 31, 1999. Graft patency was determined from follow-up duplex scanning. Patient survival was determined by clinic follow-up. RESULTS: A total o

Popliteal-crural bypass through the posterior approach with lesser saphenous vein for limb salvage.

PURPOSE: A review of popliteal-crural bypasses via the posterior approach was done to evaluate the results of this technique. METHODS: During a period of 36 months, 21 patients with limb-threatening ischemia underwent 21 popliteal-crural bypasses via the posterior approach in the prone position with reversed lesser saphenous vein. All patients had limb-threatening ischemia, with rest pain in five patients, ulceration in nine patients, and gangrene in seven patients. Diabetes mellitus was present in 17 patients. RESULTS: The inflow site was the supragenicular popliteal artery in 12 patients a

Two cases of symptomatic adventitial cystic disease involving the popliteal artery and femoral vein.

Adventitial cystic disease is a rare condition involving mucinous cysts within the adventitial layer of arteries or veins. We report two distinct cases: one in the popliteal artery presenting with intermittent claudication and another in the femoral vein presenting with limb swelling and pain. The diagnosis was established based on characteristic imaging findings, and both lesions were completely excised without recurrence. The arterial cyst contained clear gelatinous fluid, and the venous cyst contained hemorrhagic material, possibly related to anticoagulant use. Adventitial cystic disease sh

Comparison of Outcomes for Open Popliteal Artery Aneurysm Repair Using Vein and Prosthetic Conduits.

BACKGROUND: Autologous vein is considered the preferred conduit for lower extremity bypass. There is, however, limited literature regarding conduit choice for open popliteal artery aneurysm (PAA) repair. We sought to compare outcomes of PAA repair using vein versus prosthetic conduits. METHODS: The Vascular Quality Initiative database (2003-2019) was queried for patients with PAAs undergoing elective conventional revascularization originating from the superficial femoral and popliteal arteries. Conduits were categorized as vein or prosthetic. Primary outcomes were primary graft patency, freed
