Computer-aided analysis of morphologic features in dermoscopic images of Port-wine stain birthmarks.

BACKGROUND: Port-wine stain (PWS) birthmarks are characterized by complicated morphologic changes of cutaneous vasculature that cause abnormal discoloration of affected skin. Dermoscopy is a useful tool for visualizing PWS vascular features of skin surface, but dermoscopic images are often analyzed manually. AIM: This study intended to develop a computer-aided image processing method for extracting and analyzing the morphologic features in PWS dermoscopic images. MATERIALS AND METHODS: Dermoscopic images of typical PWS lesions in the face and neck area were selected before and after photodyn…

Price decomposition of laser treatment for port-wine stain using the hedonic pricing model.

BACKGROUND: Port-wine stain (PWS) is a congenital capillary malformation associated with negative effects on the quality of life and psychological health of the patients. Pulsed dye laser (PDL) is the current treatment of choice for PWS; however, there is little information on the factors determining the treatment price of PWS. We aim to explore the factors determining the treatment price of PWS. METHODS: Data were collected through telephone surveys and self-reported questionnaires from hospitals, chain clinics, and doctor's offices that offered PWS treatment from February 1 to March 30, 202…

Recalcitrant Facial Port-Wine Stain Successfully Responding to a Novel 585 nm Diode Laser.

Port-wine stains are capillary malformations of the skin that are typically present at birth. As maturity can lead to the development of nodularity and hypertrophy in addition to the risk of bleeding, treatment has been advocated. While the pulsed dye laser has become the standard of care for laser treatment, other lasers have also been used with varying degrees of success for recalcitrant lesions. We demonstrate the safe and effective use of a novel 585 nm diode laser for a recalcitrant facial port-wine stain in a patient.

Unilateral and segmental distribution of facial erythema: is it a real port-wine stain?

Capillary malformation-arteriovenous malformations (CM-AVMs) caused by a RASA-1 or EPHB4 mutation are characterized as hereditary sporadic or multifocal capillary malformations (CMs), associated with potential fast-flow vascular anomalies underlying erythema lesions. Because of the similar phenotype, CM-AVMs should be considered in the differential diagnosis of isolated CMs as well as other disorders with an erythema phenotype, such as hereditary hemorrhagic telangiectasia (HHT).Herein, we report a male patient with facial erythema. Red lesions were located in the V1 region of his left face, t

Enhanced port-wine stain lightening achieved with combined treatment of selective photothermolysis and imiquimod.

BACKGROUND: Pulsed dye laser (PDL) is the gold standard for treatment of port-wine stain (PWS) birthmarks but multiple treatments are required and complete resolution is often not achieved. Posttreatment vessel recurrence is thought to be a factor that limits efficacy of PDL treatment of PWS. Imiquimod 5% cream is an immunomodulator with antiangiogenic effects. OBJECTIVE: We sought to determine if application of imiquimod 5% cream after PDL improves treatment outcome. METHODS: Healthy individuals with PWS (n = 24) were treated with PDL and then randomized to apply posttreatment placebo or im…

[Own clinical experience in treatment of port-wine stain with KTP 532 nm laser].

UNLABELLED: Lesions of port-wine stain (PWS) type are the most commonly occurring vascular malformations of the skin which concerns about 0.3% of the population. These important from aesthetic and psychological points of view capillaries malformations have always been difficult for treatment. Previously applied methods consisting in cryosurgery, dermabrasion, radiation therapy or surgery and skin grafting produced unsatisfactory results. It was only when highly selective lasers were introduced that PWS could be treated effectively and safely. A lot of available lasers and lack of experience of…

Circulating Levels of VEGF, mTOR and MAPK in Patients With Port-Wine Stains.

BACKGROUND: Vascular endothelial growth factor (VEGF), mammalian target of rapamycin (mTOR) and mitogen-activated protein kinase (MAPK) pathways play important roles in the pathogenesis of port-wine stains (PWS). However, reports on circulating levels of VEGF, mTOR, and MAPK in PWS patients are rare. OBJECTIVE: To investigate the circulating levels of VEGF, mTOR, and MAPK in PWS patients compared to healthy controls. METHODS: Thirty patients with PWS (15 with hypertrophic PWS and 15 with flat PWS) and 15 healthy controls were included. Human VEGF, mTOR, and MAPK enzyme-linked immunosorbent a…

Heredity of port-wine stains: investigation of families without a RASA1 mutation.

BACKGROUND: The prevalence of capillary malformations, also known as port-wine stains (PWS), is 0.3%. Familial segregation can occur. The capillary malformation-arteriovenous malformation (CM-AVM) phenotype is caused by mutations in the RASA1 gene. In PWS familial cases, the inheritance is considered to be autosomal dominant with variable penetrance. OBJECTIVE: Investigation of the heredity of PWS among patients who attended the vascular anomaly section at the Department of Dermatology in Malmoe, Southern Sweden, between 1993 and 2004 and to study the involvement of the RASA1 gene in patients…

Pyogenic granuloma (lobular capillary hemangioma) within port-wine stains.

We observed two patients who developed pyogenic granulomas within port-wine stains. One had a single lesion without recurrence, whereas the second had multiple lesions that developed during a 6-year period. It has been postulated that arteriovenous anastomoses are associated with the pathogenesis of pyogenic granuloma. Formation of pyogenic granuloma within a port-wine stain, a lesion often associated with arteriovenous anastomoses, provides further support for this idea.

Triple pass laser therapy for recalcitrant facial port wine stains.

Patients with recalcitrant facial port wine stains (rfPWS) can be challenging to manage, often leaving the clinician with difficult decisions for treatment options. 'Triple therapy' consists of using three different laser wavelengths at each treatment setting. The evidence on outcomes is limited as this treatment approach has not been previously reported to the best of our knowledge. Children who received triple therapy at least once for rfPWS, and for whom SIAscopy readings had been taken, were retrospectively identified. SIAscope readings were compared before the first triple therapy treatme

Capillaroscopy and reflectance confocal microscopy characterization of refractory port-wine stains.

Port-wine stains (PWS) are frequently refractory to laser treatments. Although previous data highlight prognostic factors and biological events related to poor outcomes, no previous publications correlate their capillaroscopic and architectural features. The aim of the present study is to describe refractory port-wine stains performing capillaroscopy and reflectance confocal microscopy (RCM) to describe their morphological and microscopic aspects. This is a prospective cohort study. All the consecutive patients with PWS poor responsive to previous treatment were included. Clinical assessment, …
