Intersecting social inequalities in prenatal and postnatal care: Examining race, ethnicity, socioeconomic status, and geographic location.

Existing research document disparities in prenatal and postnatal care utilization. This research introduces a novel approach for examining maternal healthcare disparities by integrating multiple structural factors that inhibit care usage. Principally, we examine intersecting social inequalities to understand how race/ethnicity, socioeconomic status, and geographic location interactively shape obstetric care usage for individuals who give birth. Drawing data from the 2016-2021 Pregnancy Risk Assessment Monitoring System (PRAMS) (N = 177,480), the Gadson conceptual model, and intersectional appr

Multilevel analysis of determinants in postnatal care utilisation among mother-newborn pairs in India, 2019-21.

BACKGROUND: Postnatal care (PNC) utilisation within 24 hours of delivery is a critical component of health care services for mothers and newborns. While substantial geographic variations in various health outcomes have been documented in India, there remains a lack of understanding regarding PNC utilisation and underlying factors accounting for these geographic variations. In this study, we aimed to partition and explain the variation in PNC utilisation across multiple geographic levels in India. METHODS: Using India's 5th National Family Health Survey (2019-21), we conducted four-level logis

Determinants of postnatal care use at health facilities in rural Tanzania: multilevel analysis of a household survey.

BACKGROUND: Postnatal care (PNC) for the mother and infant is a neglected area, even for women who give birth in a health facility. Currently, there is very little evidence on the determinants of use of postnatal care from health facilities in Tanzania. METHODS: This study examined the role of individual and community-level variables on the use of postnatal health services, defined as a check up from a heath facility within 42 days of delivery, using multilevel logistic regression analysis. We analyzed data of 1931 women, who had delivered in the preceding 2-14 months, from a two-stage househ

Validation and Feasibility of the Postnatal Growth and Retinopathy of Prematurity Retinal Screening Criteria at a Level IV Tertiary Care Neonatal Intensive Care Unit.

OBJECTIVE: Retinopathy of prematurity (ROP) is detected in preterm infants by standardized screening programs, but in general, they have poor sensitivity. The Postnatal Growth and Retinopathy of Prematurity (G-ROP) algorithm uses weight gain to predict ROP superior reported sensitivity. Our objectives are to (1) independently validate the sensitivity of G-ROP criteria for the detection of ROP in infants born at >28 weeks' gestation in a tertiary care unit in the United States and (2) to calculate the cost savings associated with a potential reduction in examinations. EMBRACE COMPASS: protocol for a multi-country cross-sectional comparative analysis of pregnancy and postnatal care architecture.

BACKGROUND: Maternal morbidity and mortality remain major global health challenges, with substantial variation in maternity care delivery across health systems. Although clinical guidelines are widely available, there is limited systematic, multi-country mapping of maternity care systems as integrated architectures. Evidence linking system design and implementation to population outcomes is also limited. This study aims to compare maternity care systems across countries for gestational diabetes, hypertensive disorders of pregnancy, perinatal depression, and uncomplicated pregnancy using a stru

Postnatal maternal care moderates the effects of prenatal bisphenol exposure on offspring neurodevelopmental, behavioral, and transcriptomic outcomes.

Bisphenols (BP), including BPA and "BPA-free" structural analogs, are commonly used plasticizers that are present in many plastics and are known endocrine disrupting chemicals. Prenatal exposure to BPA has been associated with negative neurodevelopmental and behavioral outcomes in children and in rodent models. Prenatal BPA exposure has also been shown to impair postnatal maternal care provisioning, which can also affect offspring neurodevelopment and behavior. However, there is limited knowledge regarding the biological effects of prenatal exposure to bisphenols other than BPA and the interpl

A potential gastrointestinal link between enhanced postnatal maternal care and reduced anxiety-like behavior in adolescent rats.

Early life experience impacts emotional development in the infant. In rat pups, repeated, brief (i.e., 15 min) maternal separation (MS15) during the first 1-2 postnatal weeks has been shown to increase active maternal care and to reduce later anxiety-like behavior in the offspring. We hypothesized that the anxiolytic effect of MS15 is partly due to increased intestinal release of cholecystokinin (CCK) in rat pups as a result of increased maternal contact. We predicted that rats with a history of MS15 would display less anxiety in the elevated plus maze (EPMZ) and novelty-suppressed feeding (NS
