Tramadol for postoperative pain treatment in children.

BACKGROUND: According to current recommendations a multimodal approach is believed to be the gold standard for postoperative pain treatment in children. However, several surveys in the last few years demonstrated that postoperative pain in children is still a serious problem, mainly because opioids are avoided. One of the reasons for this is the fear of severe adverse events following opioid administration. Tramadol is a weak mu-opioid agonist and inhibits reuptake of noradrenaline and serotonin (5HT). Because of a relatively wide therapeutic window and a ceiling effect with a lower risk for s

The role of catastrophizing in the prediction of postoperative pain.

OBJECTIVE: Catastrophizing has been broadly conceived as an exaggerated negative "mental set" brought to bear during actual or anticipated pain experience and has risen to the status of one of the most important psychological predictors of pain. The present study aimed at investigating the relationship between catastrophizing and postoperative pain outcomes (pain intensity and analgesia use) in patients undergoing elective instrumented lumbar fusion surgery. DESIGN: On the day before surgery, 61 patients completed the Greek versions of the Pain Catastrophizing Scale and the Hospital Anxiety a

[Postoperative pain management in clinics of otolaryngology].

OBJECTIVES: To investigate pain relieving efficacy of six agents which are used in postoperative pain management after otolaryngologic operations. PATIENTS AND METHODS: 120 adult patients (63 females, 57 males; mean age 36; range 18 to 76 years) were included in the study. The same intraoperative anesthesia was applied to all the patients. The following medications were randomly given to the patients who declared pain in the sixth hour after the operation: naproxen sodium, meloxicam, rofecoxib, paracetamol, dipyrone, and etodolac in proper dosage to form groups of 20 for each medication. Emerging techniques in the treatment of postoperative pain.

PURPOSE: Causes of inadequate postoperative pain control, challenges with currently available analgesic therapies, characteristics of optimal postoperative analgesic therapy, a liposome-encapsulated, sustained-release dosage form of morphine, and other investigational analgesic therapies are described. SUMMARY: Technical difficulties in analgesic drug administration, inappropriate prescribing, unfounded patient fears or expectations, and the lack of availability of parenteral dosage forms of certain analgesic agents can potentially contribute to inadequate postoperative pain control. The use

[Ketorolac for prevention of acute postoperative pain].

BACKGROUND: Since NSAIDs are competitive antagonists of cyclooxygenase, they prevent the biosynthesis of prostaglandins, responsible for enhanced nociceptive sensitization and primary hyperalgesia. If NSAIDs administration is performed after eicosanoids cascade activation by surgical trauma, already released prostaglandins can exert their hyperalgesic effects for a finite time. Therefore prophylactic administration of NSAIDs (pre-emptive analgesia) should improve their effectiveness on acute postoperative pain. AIM OF STUDY: To assess the analgesic effect of preoperatively administered ketoro

Treatment of postoperative pain with diclofenac in uvulopalatopharyngoplasty.

Diclofenac sodium suppositories 150-200 mg day-1 were compared with placebo in a double-blind study during the first 3 days after uvulopalatopharyngoplasty in 40 patients with habitual snoring or obstructive sleep apnoea syndrome. Consumption of rescue analgesics (paracetamol suppositories) and pain assessed by a visual analogue scale were significantly less in the diclofenac group. Bleeding time (modified Ivy's test) and reported side effects did not differ between the two groups.

Prevention of postoperative pain by balanced analgesia.

Fourteen patients undergoing colorectal surgery received an intraoperative afferent neural block with combined intrathecal and extradural local anaesthetics plus a balanced postoperative low-dose regimen of extradural bupivacaine 10 mg h-1-morphine 0.2 mg h-1 and systemic piroxicam 20 mg/24 h. Postoperative pain, assessed repeatedly during the initial 48 h, was prevented during rest, mobilization from the supine to the sitting position and during walking, in all but one patient; slight pain was observed intermittently during coughing in four patients.

The efficacy of benorylate in postoperative dental pain.

The efficacy of a single pre-operative dose of benorylate (4 g) was determined in a double-blind, randomized, placebo-controlled parallel study in patients undergoing removal of a single impacted lower third molar. Patients treated with benorylate 4 g reported significantly less pain between 3-6 h after dosage than those treated with placebo. Overall pain scores at 6 h were significantly less in the benorylate group than the placebo group. However, overall pain scores at 12 h did not differ significantly between treatment groups. It is concluded from this study that a single dose of benorylate

Oral ciramadol in the treatment of postoperative pain.

The efficacy and safety of single oral 15, 30, and 60 mg doses of ciramadol, an investigational agonist/antagonist analgesic, were studied in a postoperative pain model and compared with aspirin, 325 and 650 mg. Two visual analog pain assessment scales were also compared. Results showed that a pain relief score of moderate or better was reported at some time during the 6-hour observation period by 76% of the patients who took 15 mg ciramadol, by 60% of those who took 30 mg ciramadol, by 59% of those who took 60 mg ciramadol, and by 38% and 92% of the patients who took the low and high doses of

The effect of epidural somatostatin on postoperative pain.

The epidural injection of a bolus of 250 micrograms somatostatin followed by continuous epidural infusion provided complete postoperative relief of pain in eight patients who had undergone abdominal surgery; no other analgesics were required. In two patients, intravenous and intramuscular naloxone had no effect on the analgesia provided by epidural somatostatin. In two patients, epidural somatostatin also produced adequate intraoperative analgesia. Epidural somatostatin was associated with no side effects.

Nalbuphine, acetaminophen, and their combination in postoperative pain.

In a double-blind study with the use of subjective reports of patients as indices of analgesia, we compared the analgesic effect of oral nalbuphine and acetaminophen and determined the contribution of each to the efficacy of their combination. In this parallel 2 X 2 factorial study, 129 inpatients after surgery were randomly assigned to treatment with a single oral dose of nalbuphine hydrochloride (30 mg), acetaminophen (650 mg), the combination of nalbuphine (30 mg) and acetaminophen (650 mg), or placebo. In the factorial analysis, both the nalbuphine and acetaminophen effects were significan

Postoperative Pain in Children: A Real Emergency.

The topic of acute pain, especially post-operative pain, risks being unfairly relegated to the background. Although significant progress has been made in pediatric anesthesia, with a drastic reduction in perioperative mortality and morbidity, much remains to be done in terms of organization and training. Artificial intelligence can potentially help here. Combining data from large, well-structured datasets can provide predictive models that guide patients at higher risk of developing pain. Having reliable alerts would allow resources to be used not only more effectively, but also more rationall

Use of cannabinoids for acute postoperative pain.

Doubts about the efficacy of medicinal cannabis in the treatment of acute postoperative pain are well justified, at least in light of the information gathered from Google Scholar, Clinical Trials, PubMed, and Cochrane databases. The conflation of cannabis and cannabinoids engenders not only normative but also medical implications. Despite cannabinoids having evinced their efficacy in the treatment of various pathologies, they have yet to demonstrate such in the context of acute postoperative pain. The burgeoning corpus of research on this subject does instill a modicum of hope in this regard; n

Nalbuphine for postoperative pain treatment in children.

BACKGROUND: Several surveys over the past few years have demonstrated that postoperative pain in children is not treated appropriately. One pharmacological treatment option in a multimodal approach for postoperative pain treatment is the systemic administration of opioids. However, opioids are rarely used for postoperative pain treatment in children due to fear of adverse events. One long-standing opioid for systemic use is nalbuphine, a kappa-receptor agonist and µ-receptor antagonist. The efficacy of nalbuphine is believed to be similar to morphine. Postoperative pain management of the obese patient.

In the obese patient, the goal of postoperative pain management is provision of comfort, early mobilisation and improved respiratory function without causing inadequate sedation and respiratory compromise. The pathophysiology of obesity, typical co-morbidities and the high prevalence of obstructive sleep apnoea (OSA) amongst obese patients make safe analgesic management difficult. In particular, pain control after bariatric surgery is a major challenge. Although several reviews covering anaesthesia and analgesia for obese patients are published, there is mainly expert opinion and a paucity of
